Incidence and Persistence of Viral Shedding in COVID-19 Post-acute Patients With Negativized Pharyngeal Swab: A Systematic Review

After the global spread of a severe acute respiratory syndrome caused by a coronavirus (SARS-CoV-2), factors that influence viral diffusion have gained great attention. Human-to-human transmission mainly occurs through droplets, but viral RNA clearance in different biological fluids in coronavirus disease 2019 (COVID-19) remains unclear. We aimed to correlate the presence and the relevant temporal patterns of SARS-CoV-2 viral RNA in biological specimens (stool, urine, blood, and tears) of the transmission with clinical/epidemiological features in patients with COVID-19. We focused on the time window between the positivity of reverse transcriptase-polymerase chain reaction (RT-PCR) tests from different specimens. We used the Mantel-Cox log rank test to verify the differences in terms of viral shedding duration, while we employed the Mann-WhitneyU-test for subgroup analysis. This review protocol was registered with PROSPERO number: CRD42020183629. We identified 147 studies; we included 55 (1,348 patients) for epidemiological analysis, of which we included 37 (364 patients) for statistical analysis. The most frequently used specimens other than respiratory tract swabs were stool samples (or anal/rectal swabs), with a positivity rate of 48.8%, followed by urine samples, with a positivity rate of 16.4%; blood samples showed a positivity rate of 17.5%. We found that fecal positivity duration (median 19 days) was significantly (p< 0.001) longer than respiratory tract positivity (median 14 days). Limited data are available about the other specimens. In conclusion, medical and social communities must pay close attention to negativization criteria for COVID-19, because patients could have longer alternative viral shedding.

The impact of cognitive function deficits and their recovery on functional outcome in subjects affected by ischemic subacute stroke: results from the Italian multicenter longitudinal study CogniReMo

Summary: This study aimed to evaluate the relationships between impairments in cognitive functions and recovery of functional independence in stroke patients during the subacute phase. The results identified several predictors of functional recovery, including functional status, lower limb functioning, attention, and executive functions. Patients who recovered deficits in executive functions had the same recovery as those without deficits, while those who maintained deficits had a smaller recovery.

Musculoskeletal Pain Management and Thermotherapy: An Exploratory Analysis of Italian Physicians' Attitude, Beliefs, and Prescribing Habits

Sclerostin: clinical insights in muscle-bone crosstalk

Summary: Sclerostin, encoded by the SOST gene, is an important regulator of bone homeostasis by inhibiting the Wnt signaling pathway. Recent studies have also suggested a potential role of sclerostin in myogenesis, affecting the interaction between bone and muscle. This review provides an overview of the clinical implications of sclerostin modulation on skeletal muscle mass and function, and bone metabolism. Understanding the muscle-bone crosstalk may have significant implications for the development of therapeutic strategies for musculoskeletal disorders, particularly osteosarcopenia.

A Nutritional Supplement as Adjuvant of Gabapentinoids for Adults with Neuropathic Pain following Spinal Cord Injury and Stroke: Preliminary Results

Summary: This study aimed to investigate the effect of NACVAN (R) as an adjuvant treatment to gabapentinoids on pain symptomatology in CNP patients. Preliminary results showed significant reductions in pain intensity, neuropathic symptoms, and evoked pain with NACVAN (R) treatment, while other neuropathic pain dimensions and quality-of-life did not significantly improve.

New insights in lower limb reconstruction strategies

Summary: High Energy Musculoskeletal Traumas (HEMTs) are a significant problem for healthcare systems due to high social costs, and the high morbidity and mortality. Complications such as bone infection, fracture malunion, and non-union contribute to poor outcomes associated with HEMT. Treatment of these complications can be challenging, with limb reconstruction often requiring multiple procedures. Amputation may theoretically offer better function and lower complications, but also comes with its own set of complications and long-term social costs. A comprehensive literature review was conducted to propose ways to optimize limb preservation surgeries for HEMT complications and improve their management.

Bone fragility during the COVID-19 pandemic: the role of macro- and micronutrients

Summary: Bone fragility is associated with aging, comorbidities, disability, poor quality of life, and increased mortality. International guidelines recommend a nutritional approach for managing patients with bone fragility, focusing on optimal protein, calcium, and vitamin D intake. Patients with major fragility fractures often have poor dietary habits and lack intake of nutrients that affect bone health, which can be further worsened during the COVID-19 pandemic. This narrative review discusses the role of macro- and micronutrients in patients with bone fragility during this pandemic.

Nutritional and physical activity issues in frailty syndrome during the COVID-19 pandemic

Summary: 'Frailty' is a state of increased vulnerability caused by impairment of homeostasis due to stress. Frail individuals are more likely to experience significant health decline and have a higher risk of adverse outcomes such as osteoporosis, falls, and disability. Frailty is becoming increasingly important due to global population aging. The pathophysiology of frailty involves cumulative molecular and cellular damage and is partially independent of aging but closely related. Uncorrected nutrition and sedentary behavior worsen frailty.

A comprehensive and interdisciplinary approach involving a dedicated healthcare team can improve the standard of care.
