Summary: The study verified and harmonized chemiluminescent immunoassays for the detection of MPO- and PR3-ANCA, and evaluated their clinical performance. The results showed high agreement between QUANTA Flash assays and ELISAs, and similar diagnostic sensitivity and specificity. By calculating likelihood ratios, the utility of likelihood ratios for harmonization of ANCA results was confirmed.

Summary: This study investigated the clinical characteristics and prognosis of patients with anti-neutrophil cytoplasmic antibody-associated vasculitis (AAV) presenting as central diabetes insipidus (CDI). The results showed that AAV patients with CDI had more ear, nose, and throat involvement and less renal impairment compared to the control group. Patients in Asian countries tended to be older men and have higher MPO-ANCA positivity, and PR3-ANCA positivity may predict disease recurrence.

Summary: This study evaluates the efficacy and safety of protein A immunoadsorption (PAIA) and therapeutic plasma exchange (TPE) for ANCA-associated vasculitis (AAV) patients with severe renal involvement. The results show that PAIA treatment effectively removes MPO-ANCA and IgG, and is superior to TPE in clearing MPO-ANCA. Furthermore, PAIA treatment has an advantage in reducing mortality. Higher serum creatinine, lower serum albumin or hemoglobin levels, age >60, and failure to achieve remission at 6 months independently predict the risk of end-stage renal disease (ESRD) or death in AAV patients.

Summary: Hypertrophic pachymeningitis (HP) is an inflammatory disorder characterized by intracranial and spinal thickened dura mater, leading to neurological manifestations. Antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) is linked to the development of immune-mediated HP. HP can be observed throughout the clinical course of AAV, with ear, nose, and throat manifestations and mucous membranes/eyes manifestations being associated with HP. Corticosteroids are the first-line therapy for ANCA-related HP, and immunosuppressive agents can be used for achieving remission.

Summary: A GWAS identified an association between PR3-ANCA and a SNP (rs62132293) upstream of the PRTN3 gene. The variant (G allele) was shown to be an expression quantitative trait locus in healthy controls, but its clinical impact is still unknown. Patients carrying the variant allele had elevated leukocyte PRTN3 expression compared to non-carriers, while healthy controls had low PRTN3 regardless of genotype.

Summary: A 77-year-old woman with sinusitis developed embolic stroke and splenic infarctions, and was diagnosed with eosinophilic granulomatosis with polyangiitis characterized by eosinophilic infiltration. Cardiac imaging also revealed evidence of endo-myocarditis. The patient received immunosuppressive and anticoagulation therapy, and no recurrence of infarctions was observed.

Summary: This study found that the glycosylation changes of IgG are associated with disease flares in patients with ANCA-associated vasculitis. In addition, among the glycosylation changes, alterations in Fc bisection and fucosylation levels of IgG are correlated with long-term treatment outcome and impending relapse.
