Summary: This study aims to evaluate the association between genetic variants of TLRs genes and the response to treatment of ENL. The results suggest that TLRs polymorphism might play a role in the response to ENL treatment, however, in the clinical setting, they cannot be considered as useful biomarkers due to small differences in medication doses.

Summary: This article describes four cases of leprosy patients with chronic recalcitrant erythema nodosum leprosum (ENL) treated with cyclophosphamide pulse therapy. The pulse therapy resulted in partial or complete regression of symptoms and allowed for a reduction in medication dosage, effectively controlling the disease.

Summary: This study assessed the efficacy and safety profiles of combined treatment of prednisolone with thalidomide or prednisolone with clofazimine in patients with erythema nodosum leprosum (ENL) or type 2 lepra reactions. The results showed that the combination of thalidomide and steroid was more effective than the combination of clofazimine and steroid in the treatment of ENL, and both treatment regimens had few tolerable side effects.

Summary: This study investigated the activation of the type I interferon (IFN-I) pathway in leprosy patients with erythema nodosum leprosum (ENL). The results showed higher activation of the IFN-I pathway in ENL patients, which was reduced during thalidomide treatment. Peripheral pDCs were found to be involved in the production of IFN-I during ENL. These findings contribute to the identification of biomarkers for early diagnosis and the discovery of new therapeutic targets for the management of ENL reactions.

The patient presented with a one-week history of fever and painful erythematous nodules along her upper and lower extremities. Histological examination confirmed the diagnosis of leprosy with abundant bacilli. The patient was treated with multidrug antibiotic therapy.

Summary: Erythema nodosum leprosum (ENL) is an immunological complication seen in 50% of lepromatous and 10% of borderline lepromatous leprosy. It presents as a multisystem disease with papulo-nodular skin lesions and fever. Rarely, a pure rheumatologic presentation of lepromatous leprosy complicated by ENL can mimic connective tissue disease and requires treatment with steroids.

Summary: The study evaluated selected serum mediators involved in immune responses to identify potential immunomarkers for Erythema nodosum leprosum (ENL). Results showed significantly higher levels of serum interleukin-6 in ENL patients, which decreased after specific therapy, indicating a role for IL-6 in pathogenesis and its potential as a biomarker for ENL.
