The outbreak of coronavirus disease 2019 (COVID-19), caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), has rapidly spread worldwide. A number of serious effects on various organs and systems have been reported in humans, and recently emerging evidence on the potential association between the infection and thyroid dysfunction are attracting attention from the scientific community. This editorial critically summarizes the main findings on this topic published so far and defines research lines according to the translational approach from the bench to the bed to epidemiological studies and back again, aimed at patient care and effective public health measures.

Summary: This study assessed influenza vaccine uptake during the 2020/2021 flu season and compared it with the previous season. The results showed that educational level and socio-economic deprivation were associated with flu vaccine uptake. These findings highlight the importance of considering social determinants when planning influenza vaccination campaigns.

Risk Associations between Vehicular Traffic Noise Exposure and Cardiovascular Diseases: A Residential Retrospective Cohort Study

Elisa Bustaffa, Olivia Curzio, Gabriele Donzelli, Francesca Gorini, Nunzia Linzalone, Marco Redini, Fabrizio Bianchi, Fabrizio Minichilli

Summary: A retrospective residential cohort study in Pisa investigated the relationship between vehicular traffic noise pollution and cardiovascular disease. The study found that noise exposure was associated with increased mortality and hospitalization rates for cardiovascular diseases, particularly among women. The authors recommend implementing noise prevention measures and monitoring the risk profile of the Pisa population.

Risk Factors for Lung Cancer in the Province of Lecce: Results from the PROTOS Case-Control Study in Salento (Southern Italy)

Summary: A higher incidence of lung cancer among men in the province of Lecce, southern Italy, compared to regional and national data was reported. In a specific area within the province, the incidence and mortality of lung cancer were even higher. A case-control study called PROTOS was conducted to examine potential risk factors for lung cancer in the province. The study found that excessive alcohol use in women, family cancer history, pack/year of cigarette smoking, higher industrial exposure in the cluster area, and pesticide use in agriculture without personal protective equipment all increased the risk of lung cancer. The higher incidence of adenocarcinoma in both sexes suggests that factors other than smoking, such as environmental, occupational, and lifestyle factors, may contribute to the increased risk of cancer and should be further investigated.

Healthcare Burden of Rare Diseases: A Population-Based Study in Tuscany (Italy)

Silvia Baldacci, Michele Santoro, Anna Pierini, Lorena Mezzasalma, Francesca Gorini, Alessio Coi

Summary: This retrospective cohort study aimed to analyze the healthcare burden of patients with rare diseases (RDs) using a multi-database approach. The study found a decrease in the hospitalization rate over the years and observed variations in the average length of stay. Patients with RDs related to metabolism and the genitourinary system had the highest hospitalization rates, while those with rare immune system disorders and diseases of the skin and subcutaneous tissue had the longest average length of stay.

Selenium and Selenoproteins at the Intersection of Type 2 Diabetes and Thyroid Pathophysiology

Francesca Gorini, Cristina Vassalle

Summary: Type 2 diabetes is a major global public-health concern, affecting over 400 million people worldwide. Selenium has recently been identified as a determinant of T2D pathogenesis and progression, playing a crucial role in antioxidant response, thyroid hormone regulation, and glucose metabolism.

Lipids in Atherosclerosis: Pathophysiology and the Role of Calculated Lipid Indices in Assessing Cardiovascular Risk in Patients with Hyperlipidemia

Melania Gaggini, Francesca Gorini, Cristina Vassalle

Summary: Lipids play a crucial role in the development of atherosclerosis, and the traditional lipid profile is an established tool for assessing cardiovascular risk. However, non-traditional lipids have emerged as potential predictors of cardiometabolic risk, as they provide a more comprehensive understanding of lipid/lipoprotein interactions.

Lipoprotein(a), Cardiovascular Events and Sex Differences: A Single Cardiological Unit Experience

Beatrice Dal Pino, Francesca Gorini, Melania Gaggini, Patrizia Landi, Alessandro Pingitore, Cristina Vassalle

Summary: This study evaluated the relationship between Lp(a) and cardiovascular disease in different genders and cardiometabolic risk factors, especially in terms of prognosis. It found that elevated levels of Lp(a) were associated with worse prognosis in females with type 2 diabetes, but not in other high-risk cardiovascular states.

FIB-4 Index and Neutrophil-to-Lymphocyte-Ratio as Death Predictor in Coronary Artery Disease Patients

Melania Gaggini, Fabrizio Minichilli, Francesca Gorini, Serena Del Turco, Patrizia Landi, Alessandro Pingitore, Cristina Vassalle

Summary: This study aimed to evaluate the predictive value of neutrophil-to-lymphocyte ratio (NLR) and fibrosis-4 index (FIB-4) on coronary artery disease (CAD) mortality. The study found that elevated NLR and FIB-4 were associated with increased mortality in CAD patients. Categorization of CAD patients based on FIB-4/NLR could provide valuable information for risk stratification and reduction of residual risk.

Vitamin D Determinants, Status, and Antioxidant/Anti-inflammatory-Related Effects in Cardiovascular Risk and Disease: Not the Last Word in the Controversy

Giulia Della Nera, Laura Sabatino, Melania Gaggini, Francesca Gorini, Cristina Vassalle

Summary: In addition to its role in calcium homeostasis, vitamin D has a significant impact on the cardiovascular system. Low vitamin D levels are associated with increased cardiovascular risk, morbidity, and mortality. The antioxidative and anti-inflammatory properties of vitamin D contribute to these effects. However, the optimal vitamin D status for cardiovascular health remains controversial, and this review will discuss confounding factors and the debate on the minimum blood 25(OH)D level required for optimal cardiovascular health.

Prescriptions for insulin and insulin analogues in children with and without major congenital anomalies: a data linkage cohort study across six European regions

Joanne Given, Joan K. Morris, Ester Garne, Elisa Ballardini, Laia Barrachina-Bonet, Clara Cavero-Carbonell, Mika Gissler, Francesca Gorini, Anna Heino, Sue Jordan, Amanda J. Neville, Anna Pierini, Ieuan Scanlon, Joachim Tan, Stine K. Urhoj, Maria Loane

Summary: This study aimed to evaluate the prescription rates of insulin/insulin analogues in children aged 0-9 years with and without major congenital anomalies. The results showed that children with chromosomal anomalies had an increased risk of developing diabetes requiring insulin therapy, while children with non-chromosomal anomalies had a similar risk to the reference group. Female children had a reduced risk, and preterm births without congenital anomalies were more likely to be prescribed insulin/insulin analogues.
