TumorTracer: a method to identify the tissue of origin from the somatic mutations of a tumor specimen

Background: A substantial proportion of cancer cases present with a metastatic tumor and require further testing to determine the primary site; many of these are never fully diagnosed and remain cancer of unknown primary origin (CUP). It has been previously demonstrated that the somatic point mutations detected in a tumor can be used to identify its site of origin with limited accuracy. We hypothesized that higher accuracy could be achieved by a classification algorithm based on the following feature sets: 1) the number of nonsynonymous point mutations in a set of 232 specific cancer-associated genes, 2) frequencies of the 96 classes of single-nucleotide substitution determined by the flanking bases, and 3) copy number profiles, if available. Methods: We used publicly available somatic mutation data from the COSMIC database to train random forest classifiers to distinguish among those tissues of origin for which sufficient data was available. We selected feature sets using cross-validation and then derived two final classifiers (with or without copy number profiles) using 80 % of the available tumors. We evaluated the accuracy using the remaining 20 %. For further validation, we assessed accuracy of the without-copy-number classifier on three independent data sets: 1669 newly available public tumors of various types, a cohort of 91 breast metastases, and a set of 24 specimens from 9 lung cancer patients subjected to multiregion sequencing. Results: The cross-validation accuracy was highest when all three types of information were used. On the left-out COSMIC data not used for training, we achieved a classification accuracy of 85 % across 6 primary sites (with copy numbers), and 69 % across 10 primary sites (without copy numbers). Importantly, a derived confidence score could distinguish tumors that could be identified with 95 % accuracy (32 %/75 % of tumors with/without copy numbers) from those that were less certain. Accuracy in the independent data sets was 46 %, 53 % and 89 % respectively, similar to the accuracy expected from the training data. Conclusions: Identification of primary site from point mutation and/or copy number data may be accurate enough to aid clinical diagnosis of cancers of unknown primary origin.

The ratio of adaptive to innate immune cells differs between genders and associates with improved prognosis and response to immunotherapy

Summary: Immunotherapy has revolutionized cancer treatment, but not all patients benefit. Current stratification strategies based on PD1 status and mutation burden are imperfect. It has been found that a high activation of the innate immune response relative to the adaptive response may hinder tumor neoantigen identification and decrease the anticancer response. The adaptive-to-innate immune ratio is associated with patient prognosis and response to immunotherapy, showing potential relevance to patient stratification and a gender bias.

Summary: Immunotherapy has dramatically improved the treatment outcomes for metastatic melanoma patients, but it also brings immune-related adverse events. Identifying nonresponding patients early is crucial, and this study explores the use of circulating tumor DNA (ctDNA) analysis as a minimally invasive tool. In a cohort of 24 melanoma patients, the presence of ctDNA was associated with poor prognosis, suggesting its potential to be used in clinical practice.

Summary: This study reveals that lung adenocarcinomas in both humans and mice elicit local germinal center responses and tumour-binding antibodies, with endogenous retrovirus (ERV) envelope glycoproteins as the dominant anti-tumour antibody target. ERV-targeting B cell responses are enhanced by immune checkpoint blockade (ICB) and targeted inhibition of KRAS(G12C). ERV-reactive antibodies have anti-tumour activity and improve survival in a mouse model, and ERV expression predicts the response to ICB in human lung adenocarcinoma. Furthermore, the study demonstrates that effective immunotherapy in the mouse model requires CXCL13-dependent tertiary lymphoid structure (TLS) formation, and therapeutic CXCL13 treatment enhances anti-tumour immunity and synergizes with ICB. These findings provide a potential mechanistic basis for the association between TLS and immunotherapy response.

Summary: Through longitudinal evolutionary analysis of 126 non-small cell lung cancer tumors, it was found that 25% of cases showed early metastatic divergence before the last clonal sweep in the primary tumor, which was enriched in patients who were smokers at the time of initial diagnosis. Polyclonal dissemination was associated with tumor recurrence, while primary lymph node disease played a limited role in metastatic relapse. Our study highlights the importance of selection in metastatic clone evolution within untreated primary tumors.

Summary: In this study, intratumour transcriptomic diversity in non-small cell lung cancer tumours was investigated using paired whole-exome and RNA sequencing data. It was found that the transcriptome plays a major role in driving intratumour heterogeneity, lung cancer evolution, and metastasis. The study also revealed the involvement of allele-specific expression and RNA-editing enzymes in these processes.

Summary: Circulating tumour DNA (ctDNA) can be used to detect and profile residual tumour cells after surgery. A study involving a large number of early-stage non-small-cell lung cancer (NSCLC) patients found that preoperative absence of ctDNA detection is associated with better clinical outcome. Postoperative plasma analysis revealed ctDNA detection in 25% of patients within 120 days after surgery, and 49% of patients who experienced clinical relapse were identified.

Summary: This study analyzed 1,644 tumor regions and found various gene mutations and evolutionary processes in lung adenocarcinoma. It was discovered that clonal expansion and whole genome doubling play important roles in the development and relapse of non-small cell lung cancer. The findings also revealed similarities in etiology and pathogenesis between lung adenocarcinoma in smokers and never-smokers.

Summary: Cancer-associated cachexia (CAC) is a significant factor in the morbidity and mortality of non-small cell lung cancer patients. The distribution of skeletal muscle and adipose tissue at the time of diagnosis is associated with survival rates. Individuals who develop CAC exhibit distinct tumor genomic and transcriptomic profiles, including enrichment of inflammatory signaling and epithelial-mesenchymal transitional pathways. The protein GDF15 is identified as a potential therapeutic target for managing CAC.

Summary: Intratumor heterogeneity is crucial for the evolution and selection of subclonal tumor cell populations. However, accurately inferring tumor subclonal architecture and reconstructing tumor evolutionary histories from bulk DNA sequencing data remains challenging. In this article, the authors propose a new computational framework, CONIPHER, which can accurately infer subclonal structure and phylogenetic relationships from multisample tumor sequencing, accounting for both copy number alterations and mutation errors.
