Background Pemphigus encompasses a group of life-threatening autoimmune bullous diseases characterized by blisters and erosions of the mucous membranes and skin. Before the era of immunosuppressive treatment, pemphigus was almost always fatal. Due to its rarity, only few randomized controlled therapeutic trials are available. Recently, rituximab has been approved as first-line treatment for moderate and severe pemphigus vulgaris in Europe and the United States. Objectives TheAutoimmune blistering diseasesTask Force of the European Academy of Dermatology and Venereology (EADV) has initiated a throughout update of the guideline for the management of patients with pemphigus. Results The guidelines for the management of pemphigus were updated, and the degree of consent among all task force members was included. The final version of the guideline was consented by the European Dermatology Forum (EDF) and several patient organizations.

Summary: This study investigated the levels of Fab glycosylation on autoantibodies associated with various B cell-mediated autoimmune diseases and found that elevated Fab glycosylation is present in chronic autoimmune diseases. The increased Fab glycosylation was not limited to a specific subclass of IgG and was not caused by chronic antigen-stimulation. These findings suggest that disease-specific autoantibodies in chronic autoimmune diseases are enriched with Fab glycans.

Summary: Isotretinoin does not increase the risk of Crohn's disease, but it may be associated with a slight and temporary increase in the risk of ulcerative colitis. However, it can lower the risk of irritable bowel syndrome.

A human skin organ culture (HSOC) model has been established as an alternative to animal models for studying pemphigus, replicating key features of the disease.

Summary: This study summarizes the different outcomes and outcome measurement instruments reported in clinical research on epidermolysis bullosa (EB) over the past 30 years and identifies a significant heterogeneity. Adequate evaluation and comparison of EB clinical studies require well-defined and consensus-endorsed outcomes and measurement instruments. This scoping review is the first step towards harmonization of EB outcomes, which is crucial for the clinical translation of novel treatments.

Summary: This study used a DELPHI exercise to establish a multidisciplinary expert consensus on the management of SJS/TEN sequelae. After the survey, a consensus was obtained for 100% of the initially proposed statements. The final consensus will help guide physicians in conducting a prolonged multidisciplinary follow-up of sequelae in SJS-TEN.

Impact of diet and host genetics on the murine intestinal mycobiome

Summary: In this study, 591 mice were used to demonstrate that fungi are regulated by host genetics and that diet has a regulatory role in the composition of gut fungi. The role of fungi in complex interactions involving host genetics, bacteria, and environmental factors remains understudied. Using whole genome sequencing and genotyping, quantitative trait loci associated with various fungal species were mapped in mice. Additionally, the study identified fungal indicator species associated with different dietary regimes.

EGFR inhibition leads to enhanced desmosome assembly and cardiomyocyte cohesion via ROCK activation

Summary: Arrhythmogenic cardiomyopathy (AC) is a familial heart disease caused by impaired desmosome turnover, and stabilizing desmosome integrity through EGFR and ROCK inhibition may offer new treatment options. In this study, EGFR inhibition enhanced cardiomyocyte cohesion and increased DSG2 recruitment and binding at cell borders. Additionally, EGFR inhibition led to the upregulation of ROCK, and ROCK inhibition abolished the effects of EGFR inhibition on desmosome assembly and cardiomyocyte cohesion. These findings suggest that targeting EGFR and ROCK could be potential therapeutic strategies for AC.

Pruritus Is Associated with an Increased Risk for the Diagnosis of Autoimmune Skin Blistering Diseases: A Propensity-Matched Global Study

Summary: Autoimmune bullous skin diseases (AIBDs), such as bullous pemphigoid (BP) and pemphigus, are characterized by autoantibodies targeting structural proteins. Pruritus is common in patients with BP and pemphigus, and can be the sole symptom in some cases. The temporal relationship between pruritus and AIBDs is unclear, and the presence of pruritus in other AIBDs is also unknown.
