Therapeutic Effects of Lactoferrin in Ocular Diseases: From Dry Eye Disease to Infections

Lactoferrin is a naturally occurring iron-binding glycoprotein, produced and secreted by mucosal epithelial cells and neutrophils in various mammalian species, including humans. It is typically found in fluids like saliva, milk and tears, where it reaches the maximum concentration. Thanks to its unique anti-inflammatory, antioxidant and antimicrobial activities, topical application of lactoferrin plays a crucial role in the maintenance of a healthy ocular surface system. The present review aims to provide a comprehensive evaluation of the clinical applications of lactoferrin in ocular diseases. Besides the well-known antibacterial effect, novel interest has been rising towards its potential application in the field of dry eye and viral infections. A growing body of evidence supports the antimicrobial efficacy of lactoferrin, which is not limited to its iron-chelating properties but also depends on its capability to directly interact with pathogen particles while playing immunomodulatory effects. Nowadays, lactoferrin antiviral activity is of special interest, since lactoferrin-based eye drops could be adopted to treat/prevent the new severe acute respiratory syndrome coronavirus type 2 (SARS-CoV-2) infection, which has conjunctivitis among its possible clinical manifestations. In the future, further data from randomized controlled studies are desirable to confirm the efficacy of lactoferrin in the wide range of ocular conditions where it can be used.

Widefield and Ultra-Widefield Retinal Imaging: A Geometrical Analysis

Summary: Diabetic retinopathy (DR) can cause various lesions in the peripheral retina, which may go unnoticed with traditional fundus cameras. Widefield (WF) and Ultra-Widefield (UWF) technologies aim to expand the photographable retinal field. Through geometrical analysis, we found that WF and UWF imaging significantly increase the field of view, allowing detailed visualization of the peripheral retina. This has a significant impact on the diagnosis and management of DR.

AAPOS Adult Strabismus Committee?s Report on the Status of Adult Strabismus Telemedicine

Clinical Results of Accelerated Iontophoresis-Assisted Epithelium-on Corneal Cross-linking for Progressive Keratoconus in Children

Summary: The clinical characteristics of pediatric patients with progression of keratoconus after I-ON CXL were evaluated and the efficacy and safety of retreatment using epi-OFF CXL were assessed. Two years after I-ON CXL, 12 patients showed progression of keratoconus, whereas 4 patients were stable. I-ON CXL was ineffective in younger children, but had a 2-year efficacy in older children. Retreatment using epi-OFF CXL was effective in halting the progression of keratoconus after I-ON CXL failure.

Early diagnosis and appropriate surgical intervention can lead to favorable motor and sensory outcomes.

This article describes both low-tech and high-tech approaches for detecting post-keratoplasty astigmatism, as well as the management strategies involving suture manipulation.

Multimodal imaging in early onset non-neovascular pigment epithelial detachments

Summary: This article describes the multimodal imaging of bilateral non-vascularized pigment epithelial detachments in two young patients with long-term follow-up. It was observed that both patients had a choroidal layer thicker than 420μm and no choroidal neovascularization was detected. During the follow-up period, one eye showed a retinal pigment epithelium tear and all eyes showed the presence of apical sub-retinal fluid and hyperreflective material. No signs of atrophy were observed in either patient during the follow-up period.

Outcomes of large-diameter deep anterior lamellar keratoplasty and converted two-piece mushroom penetrating keratoplasty for herpetic corneal scars

Summary: This study reports the clinical outcomes of large diameter deep anterior lamellar keratoplasty (DALK) and converted two-piece microkeratome-assisted mushroom keratoplasty (MK) for herpetic corneal scars. The results showed that DALK yields excellent visual and clinical outcomes, and in case of intraoperative complications, DALK can be converted to MK to maximize the refractive benefit and minimize the risk of graft failure.

The State of Intraoperative OCT in Vitreoretinal Surgery: Recent Advances and Future Challenges
