The new 'normal': Rapid adoption of telemedicine in orthopaedics during the COVID-19 pandemic

Background: Telemedicine provides a safe and effective means for the delivery of care by physicians amongst many subspecialties. Historically, orthopaedic practices in the United States have not widely utilized telemedicine for the delivery of orthopaedic care. As technology improves the adoption and utilization of telemedicine will likely grow, especially in light of the novel coronavirus (COVID-19) pandemic. Our study aims to assess patient and surgeon satisfaction and efficacy of telemedicine during a rapid adoption due to the global pandemic. Methods: All patients who completed a telemedicine encounter (telephone or video) with an orthopaedic surgeon were contacted. Patients were individually contacted after their visit, and a standardized validated post-visit satisfaction survey was completed. Orthopaedic surgeons completed a standardized postencounter survey after each visit. Pre-COVID-19 patient satisfaction data was used for comparison. Results: Orthopaedic surgeons completed 612 telehealth encounters either via phone or video consultation between April 6, 2020 and May 22, 2020. 95% of patients rated both surgeon sensitivity to their needs and response to their concerns as 'good' or 'very good.' 93% of patients reported they would participate in a telemedicine encounter again. Surgeons reported high satisfaction with telemedicine encounters (80%, 86% phone and video respectively), and that 78.4% of the time a telemedicine encounter was successful in replacing an in-person visit. Conclusion: Patients and orthopaedic surgeons documented high levels of satisfaction with telehealth encounters during the novel coronavirus (COVID-19) pandemic. Telemedicine does not appear to be a replacement for all in-person clinic encounters, however, when used in the appropriate context demonstrated favourable results. YPublished by Elsevier Ltd.

Bilateral femur fractures are associated with concomitant femoral neck fracture

Summary: The purpose of this study was to determine the rate of femoral neck fractures in patients with bilateral femur fractures compared to patients with unilateral femur fractures. A retrospective review of an institutional trauma database was conducted, and the results showed an association between bilateral femur fractures, open femur fractures, and associated femoral neck fractures.

Effect of nailing technique on length of stay in isolated ballistic femoral shaft fractures

Summary: This study investigated the length of stay, postoperative mobilization, and discharge disposition following intramedullary nailing of ballistic femoral shaft fractures stratified by nailing technique. The results showed that patients who underwent antegrade nailing had a significantly shorter length of stay compared to patients who underwent retrograde nailing. Patients in the antegrade cohort also had greater mobilization in the immediate postoperative setting. There was no significant difference in VAS pain scores between the two cohorts.

Gunshot-related lower extremity nerve injuries

Summary: This retrospective study aimed to investigate the frequency of nerve injury associated with lower extremity ballistic trauma and its relationship with skeletal and soft tissue injuries, as well as the rate of neurologic recovery. The findings indicated a high rate of concomitant nerve injuries in patients with lower extremity ballistic trauma, with an associated diagnosis of vascular injury indicating a higher risk of neurologic injury.

American Academy of Orthopaedic Surgeons Appropriate Use Criteria: Prevention of Surgical Site Infections After Major Extremity Trauma

Summary: The study developed Appropriate Use Criteria for the prevention of surgical site infections (SSIs) after major extremity trauma, using evidence-based information and the clinical expertise of physicians. The criteria were created by identifying clinical indications typical of patients suspected of developing an SSI and were evaluated by a multidisciplinary voting panel. A total of 588 patient scenarios and 14 treatments were assessed using a 9-point scale.

A global perspective on gun violence injuries

Total hip arthroplasty after acetabular fractures in the older population: timing of intervention may improve patient outcomes

Summary: This study evaluates the complication rates of different treatment modalities for acetabular fractures in older patients undergoing THA. The results showed that patients who received acute THA or delayed THA had significantly higher complication rates compared to those who underwent THA for non-fracture causes. Patients who underwent simultaneous ORIF and THA or THA after ORIF also had significantly higher rates of revision, infection, and DVT. Acute THA patients had significantly higher rates of readmission and DVT compared to delayed THA patients.

Isolated ballistic femoral condyle fractures: a case series of eighteen patients

A comparison between ilioinguinal and modified Stoppa approach in anterior column acetabular fractures

Summary: This study compares the outcomes and complications of Ilioinguinal versus modified Stoppa approach in Open Reduction and Internal Fixation (ORIF) of anterior column acetabulum fractures. The results show that the modified Stoppa approach has a shorter operative time, less intra-operative blood loss, and fewer complications. The ilioinguinal approach achieves better anatomic reduction. However, there were no significant differences in terms of vascular lesions, clinical and functional outcomes between the two groups.

Analysis of dental traumatic patterns in standing electric scooter-related accidents

Summary: This study aims to investigate dental traumatic injuries related to standing electric scooters and compare them with injuries caused by other factors. The results indicate that crown-root fractures and avulsions are more common in electric scooter accidents. Additionally, relatively minor dental injuries, such as concussions and subluxations, are more likely to occur as combined injuries in electric scooter accidents. The study suggests the need for strengthened policies and enforcement of laws to prevent severe dental and craniofacial trauma associated with electric scooter use.

A new method regulates bone fracture tissue exosome lncRNA-mRNA to promote mesenchymal stem cell proliferation and migration

Summary: This study discussed the role of fracture site tissue exosomes lncRNAs-mRNAs networks on post-injury adaptation (PIA) promoting bone mesenchymal stem cells (BMSCs) proliferation and migration. The results confirmed that PIA accelerated BMSCs proliferation and migration, and identified tissue exosomes as the key factor in this process. By analyzing the tissue exosomes lncRNAs-mRNAs networks, several hub genes, such as Akt1, Actb, and Uba52, were identified, and Kif11 was found to be a key gene in BMSCs regulated by tissue-derived exosomes of PIA treated rats.

Preoperative COVID-19 infection status negatively impacts postoperative outcomes of geriatric hip fracture surgery

Summary: A retrospective propensity score matched cohort study was conducted using the American College of Surgeons National Surgical Quality Improvement Program (NSQIP) database to compare outcomes for patients with recently diagnosed COVID-19 infection and those without COVID-19 infection undergoing operative treatment of hip fractures. The results showed that COVID-19-positive patients had a higher risk of 30-day mortality, pneumonia, unplanned intubation, septic shock, longer length of hospital stay, and discharge to an acute care hospital. Active COVID-19 infection is an independent risk factor for complications and increased resource utilization in these patients.

Calcaneal tuberosity avulsion fractures - A review

Summary: Calcaneal tuberosity avulsion fracture, an extra-articular injury, is a rare fracture caused internally by intense contraction of the gastrocnemius-soleus complex, and externally by low-energy (possibly high-energy). The risk of injuries to the skin and Achilles tendon around the calcaneal tuberosity is closely related to the Lee classification and Carnero-Martin de Soto Classification of this type of fracture. Diagnosis is typically confirmed through X-ray, digital imaging, and CT, but MRI should also be used to assess the soft tissue. Recent advances in understanding this fracture have led to the development of different internal fixation devices and surgical procedures that provide stable fracture reduction and resistance to Achilles tendon forces. This article reviews the new insights into the anatomy, classification, risk factors, and treatment modalities of calcaneal tuberosity avulsion fracture in recent years.

In-hospital complications of work-related musculoskeletal injuries

Summary: This study aimed to identify the incidence and factors associated with in-hospital complications of work-related musculoskeletal injuries. The results showed that age, gender, area-level disadvantage, hospital type, comorbidity, emergency admissions, anesthesia, and the type of injury were all associated with in-hospital complications. The most common complications were cardiovascular, gastrointestinal complications and adverse drug events.

Healed ORIF in a fibula fracture - Hardware removal or hardware retention for the fibula in a patient with minor symptoms?

She had ORIF surgery and recovered well, but has developed vague pain and hardware sensation at the lateral ankle 6 months later. Her past medical history includes breast reduction surgery and vaginal delivery, and she has a social history of alcohol consumption and past recreational drug use.

Compression screw internal fixation versus non-weight-bearing cast immobilization for Jones fractures in children

Summary: This study compared the clinical outcomes of compression screw and non-weight-bearing techniques in treating pediatric Jones fractures. The results showed that compared to non-weight-bearing techniques, compression screw fixation can shorten the radiographic union time, immobilization time, and facilitate earlier return to full weight bearing and daily life. We recommend the use of compression screw fixation for widely displaced fractures and school-age active adolescents.
