Visit-to-Visit Blood Pressure Variability Is Associated With Cognitive Decline and Incident Dementia The S.AGES Cohort

To investigate the impact of visit-to-visit systolic blood pressure variability (BPV), diastolic BPV, mean arterial pressure variability, and pulse pressure variability on cognitive decline and incident dementia in noninstitutionalized patients aged >= 65 years. A total of 3319 subjects from the S.AGES (Sujets AGeS-Aged Subjects) cohort underwent clinical examinations every 6 months during 3 years. Variability was evaluated using standard deviation (SD), coefficient of variation, average real variability, successive variation, variation independent of mean, and residual SD. Cognition was assessed using the Mini-Mental State Examination and dementia with the Diagnostic Statistical Manual of Mental Disorders. Linear mixed models and Cox proportional hazards models were used. Higher systolic BPV was associated with poorer cognition independently of baseline SBP: adjusted 1-SD increase of coefficient of variation: beta (SE)=-0.12 (0.06),P=0.04. Similar results were observed for diastolic BPV and mean arterial pressure variability: beta (SE)=-0.20 (0.06),P<0.001 for both. Higher pulse pressure variability was no longer associated with cognitive function after adjustment for age, except with residual SD (P=0.02). Among the 3319 subjects, 93 (2.8%) developed dementia. Higher systolic BPV was associated with greater dementia risk (adjusted 1-SD increase of coefficient of variation: hazard ratios=1.23 [95% CI, 1.01-1.50],P=0.04). Similar results were found for diastolic BPV and mean arterial pressure variability (P<0.01). Pulse pressure variability was not associated with dementia risk. Beyond hypertension, higher BPV is a major clinical predictor of cognitive impairment and dementia. Further studies are needed to assess whether controlling BP instability could be a promising interventional target in preserving cognition among older adults.

Summary: This study found that levels of A beta 42 and pTau in cerebrospinal fluid are more direct indicators of the pathogenesis of Alzheimer's disease than clinical diagnosis. Through a collaborative effort, novel associations with AD risk loci and multiple biological pathways related to A beta 42 and pTau were identified.

Blood pressure variability: A potential marker of aging

Summary: Aging is associated with increased Blood Pressure Variability (BPV), which is linked to organ damage and various health outcomes. Aging and altered BPV share the same molecular mechanisms, including subclinical inflammation, altered endothelial function, and increased production of ROS. Arterial stiffness and autonomic dysfunction are also associated with impaired BPV in elderly patients. Furthermore, molecular changes in cardiovascular aging and altered BPV are related to the hallmarks of aging. The interplay between BPV and the pathophysiology of aging suggests that BPV may serve as a new marker of aging.

Summary: This study compared the costs of healthcare resource use, stroke/systemic thromboembolism, and major bleedings between non-valvular atrial fibrillation patients who initiated apixaban and other oral anticoagulants. The results showed that patients initiating apixaban had lower costs in terms of healthcare resource use and event-related costs compared to other oral anticoagulants. Apixaban may be cost-saving compared to VKAs and cheaper than other DOACs, although the cost differences are limited.

Association between car driving and successful ageing. A cross sectional study on the S.AGES cohort

Summary: This study investigates whether driving status can be considered a proxy of successful ageing (SA) and identifies determinants associated with driving status in the 65+. The results show that 23.8% of drivers are classified as successful agers, compared to only 6.8% of non-drivers. The logistic model suggests that SA is associated with driver status. Driving reflects elders' independence, cognitive ability, and a means to maintain social interactions.

Summary: This study compared the clinical performance of Lumipulse and Simoa assays in predicting the conversion to dementia in patients with mild cognitive impairment (MCI). The results showed that both assays had similar abilities, but Lumipulse was less affected by comorbidities. Plasma pTau181 was associated with renal dysfunction, cardiovascular factors, nutrition, and inflammatory markers. The study suggests that Lumipulse could be used as a noninvasive diagnostic tool for the management of Alzheimer's disease.
