Staphylococcus aureus enterotoxin B disrupts nasal epithelial barrier integrity

This study found that SEB damages nasal polyp epithelial cell integrity by triggering TLR2, worsening the condition of CRSwNP. Interfering with TLR2 triggering might offer a way to prevent the pathophysiological consequences of Staphylococcus aureus-induced inflammation in CRSwNP.
Background Staphylococcus aureus colonization and release of enterotoxin B (SEB) has been associated with severe chronic rhinosinusitis with nasal polyps (CRSwNP). The pathogenic mechanism of SEB on epithelial barriers, however, is largely unexplored. Objective We investigated the effect of SEB on nasal epithelial barrier function. Methods SEB was apically administered to air-liquid interface (ALI) cultures of primary polyp and nasal epithelial cells of CRSwNP patients and healthy controls, respectively. Epithelial cell integrity and tight junction expression were evaluated. The involvement of Toll-like receptor 2 (TLR2) activation was studied in vitro with TLR2 monoclonal antibodies and in vivo in tlr2(-/-) knockout mice. Results SEB applied to ALI cultures of polyp epithelial cells decreased epithelial cell integrity by diminishing occludin and zonula occludens (ZO)-1 protein expression. Antagonizing TLR2 prevented SEB-induced barrier disruption. SEB applied in the nose of control mice increased mucosal permeability and decreased mRNA expression of occludin and ZO-1, whereas mucosal integrity and tight junction expression remained unaltered in tlr2(-/-) mice. Furthermore, in vitro SEB stimulation resulted in epithelial production of IL-6 and IL-8, which was prevented by TLR2 antagonization. Conclusion & Clinical relevance SEB damages nasal polyp epithelial cell integrity by triggering TLR2 in CRSwNP. Our results suggest that SEB might represent a driving factor of disease exacerbation, rather than a causal factor for epithelial defects in CRSwNP. Interfering with TLR2 triggering might provide a way to avoid the pathophysiological consequences of S. aureus on inflammation in CRSwNP.

Summary: This study aims to establish an international chronic rhinosinusitis outcome registry based on real-world data collection enabled by mobile health technology. A collaborative scientific network of 17 university ear-nose-throat clinics from 10 European countries has been established to collect data in a longitudinal and standardized manner. Up to 300 patients have already been included.

Reported Cases and Diagnostics of Occupational Insect Allergy: A Systematic Review

Summary: A significant proportion of adult-onset asthma is caused by occupational exposure to various agents. Insect allergies can occur in several professions, with respiratory symptoms such as asthma being the most common. The recent approval of insects for human consumption by the European Union may increase the risk of occupational insect allergies. This review highlights the need for better diagnostics and primary prevention measures to protect workers in the developing insect industry.

Histamine skin prick tests: from established diagnostic technique to advanced experimental biomarker

Summary: This study investigated the measurement method of histamine-induced skin blood flow and flare area, as well as the effects of certain drugs on histamine-induced blood flow and flare. The results showed inconsistent correlation between ruler and laser Doppler imaging assessments of the flare area, and the reproducibility and histamine-dependency of the measurements were not obvious. The study suggested that a lower dose of histamine might be more suitable as a quantitative biomarker.

Summary: Organomegaly, including splenomegaly, hepatomegaly, and/or lymphadenopathy, is an important diagnostic and prognostic feature in patients with cutaneous mastocytosis (CM) or systemic mastocytosis (SM). The number of organomegalies is adversely associated with overall survival (OS), and lymphadenopathy is frequently detected in patients with SM, particularly in advanced SM (AdvSM). The presence of lymphadenopathy in AdvSM is associated with inferior outcomes compared to AdvSM patients without lymphadenopathy.

The main action points to further optimize the Belgian clinical practice of HAE include working towards total disease control, informing patients about new long-term prophylactic therapies, ensuring availability of on-demand therapy, implementing a more comprehensive assessment in daily clinical practice, and expanding the existing patient registry.

Summary: Deficiency of human adenosine deaminase type 2 (DADA2) is a rare autoimmune disorder characterized by vasculopathy, immune dysregulation, and hematologic abnormalities. Neurological manifestations, particularly strokes, are a significant feature of DADA2, with brain stem and deep gray matter being commonly affected. Other neurological involvements include neuropathies, focal deficits, ophthalmological findings, convulsions, and headaches. Recognizing the diverse neurological manifestations of DADA2 is crucial for accurate diagnosis and appropriate management.
