Association between white matter impairment and cognitive dysfunction in patients with ischemic Moyamoya disease

Background Ischemic Moyamoya disease is one of the important causes of stroke, which leads to severe impairment in cognitive functions. This cognitive impairment occurs prior to stroke. However, the cognitive functions that are impaired and the mechanisms of these impairments have not been determined. Methods We analyzed 12 patients with Moyamoya disease and 12 controls. All participants underwent cognitive tests and magnetic resonance imaging (MRI) scans. The diffusion tensor imaging (DTI) data was processed using Tract-Based Spatial Statistics (TBSS). Significantly different white matter areas were correlated with different cognitive functions. Results There were significant differences in intelligence and subtraction between the patients and controls (p < 0.05). The parameters of DTI such as fractional anisotropy (FA), mean diffusivity (MD), axial diffusivity (AD), and radial diffusivity (RD) have different changes in anterior thalamic radiation, inferior fronto-occipital fasciculus (IFO), superior longitudinal fasciculus (SLF), uncinate fasciculus (UF), inferior longitudinal fasciculus, forceps minor, and other regions between the two groups. Conclusion Left UF and IFO may be the key brain regions affecting arithmetic function, while bilateral IFO has an effect on intelligence. RD and AD may be better indicators for early prediction of chronic white matter damage than FA, while MD tends to have a comprehensive indirect change. There is cognitive impairment in ischemic MMD, which is closely related to white matter impairment.

Summary: Direct bypass surgery improves blood flow and flow direction in patients with moyamoya disease, significantly increasing blood flow in the M4 segment. Postoperatively, blood flow direction typically changes to a bi-directional pattern, with a significant difference in flow volume between the distal and proximal sides of the M4 segment. Additionally, patients with a change in flow direction in the distal M4 segment are associated with lower risks of postoperative infarctions.

Summary: This study revealed significant declines in short-term memory and lower brain network connectivity in asymptomatic carotid artery stenosis (aCAS) patients compared to controls, with the decreased connectivity significantly correlated with memory impairment. These results suggest that DNA methylation is involved in the pathogenesis of MMD, and new pathogenic genes are proposed as biological markers.

Evaluation of a convolutional neural network to identify scaphoid fractures on radiographs

The findings suggest the potential utility of exosomal RNAs as biological markers in MMD.

A Novel Mini-Invasive Technique of Arthroscopic-Assisted Reduction and Robot-Assisted Fixation for Trans-Scaphoid Perilunate Fracture Dislocations

Association between DIAPH1 variant and posterior circulation involvement with Moyamoya disease

Summary: This study investigated the DIAPH1 gene mutation in an Asian population and compared the angiographic features of MMD patients with and without the mutation. DIAPH1 gene mutation was detected in 18% of the MMD patients, and there was a significantly higher rate of posterior cerebral artery involvement in the mutation positive group. DIAPH1 gene mutation is not a major genetic risk gene for Asian patients with moyamoya disease but may play an important role in the involvement of posterior cerebral artery.
