Reduced PICU respiratory admissions during COVID-19

During the COVID-19 pandemic in South America, there was a significant reduction in PICU admissions due to viral LRTIs, as well as admissions related to respiratory syncytial virus and influenza, compared to previous years.
Background The COVID-19 pandemic reached the Southern Hemisphere in the autumn of 2020, thus coinciding with its expected annual viral respiratory season. The potential impact of national strategies aimed at mitigating COVID-19 during the pandemic on the incidence of other critical viral lower respiratory tract infections (LRTIs) in children is unknown. Methods We analysed admission data for LRTIs from 22 paediatric intensive care units (PICUs) in four countries, part of a large international Latin American registry of children with acute respiratory failure (Red Colaborativa Pediatrica de Latinoamerica [LARed Network]). Results Between January and August, there were 83% fewer PICU admissions for LRTIs in 2020 compared to the 2018/2019 average over the same period. Similar decreases were noted for PICU admissions due to respiratory syncytial virus and influenza (92% and 78%, respectively). Conclusion We observed a striking reduction in PICU admissions due to viral LRTIs over winter, during the COVID-19 pandemic in South America.

A systematic review of clinical study evidence for pulmonary vasodilator therapy following surgery with cardiopulmonary bypass in children with CHD

Summary: Complications from pulmonary hypertension are a major cause of morbidity and mortality in children with congenital heart disease (CHD) post-cardiopulmonary bypass surgery. The use of pulmonary vasodilator therapy after surgery is common, but the optimal target patient population, therapy choice, timing, and duration are unclear. This review identified 26 studies involving 42,971 children with CHD, and found that inhaled nitric oxide, iloprost, and sildenafil showed improved haemodynamics in specific cohorts of children with post-operative pulmonary hypertension. However, further research is needed to reduce post-operative morbidity and mortality.

Summary: Pediatric-specific ventilator liberation guidelines are lacking and have led to unnecessary variation in methods used to assess readiness for extubation. An international panel of experts developed evidence-based recommendations for best practices related to pediatric ventilator liberation, including screening, spontaneous breathing trials, respiratory muscle strength assessment, and prevention of upper airway obstruction. However, most recommendations are conditional and based on low certainty of evidence.

Summary: This study provides the first global case series of children dying with nosocomial RSV infection. Infant-targeted immunization strategies in the future could prevent the majority of nosocomial RSV-related deaths. Although nosocomial RSV deaths are expected to occur at highest rates in low- and lower-middle-income countries (LMICs), the reported number of these deaths was low in these countries. Hospital-based surveillance is needed to capture the full burden of nosocomial RSV mortality in LMICs.

A Network Meta-analysis of Dexamethasone for Preventing Postextubation Upper Airway Obstruction in Children

Summary: In this study, the effectiveness of different corticosteroid regimens in preventing upper airway obstruction (UAO) and reintubation in children was compared. The results showed that early initiation of dexamethasone, ideally more than 12 hours before extubation, is more important than the dose in preventing UAO. However, there is no clear optimal regimen for preventing reintubation. Therefore, individualized steroid strategies should consider the potential adverse events associated with dexamethasone and the individual risk of UAO and reintubation.

Reducing Avoidable Transfer Delays in the Pediatric Intensive Care Unit for Status Asthmaticus Patients

Summary: This study aimed to reduce avoidable delays in pediatric intensive care unit (PICU) discharge for patients with status asthmaticus by standardizing transfer criteria and implementing multidisciplinary strategies. The results showed a significant reduction in the time from transfer criteria fulfillment to discharge, as well as a decrease in readmissions for asthma exacerbations.
