Summary: This study aimed to identify the predictors of in-hospital mortality in patients with nonbacterial thrombotic endocarditis. The results showed that factors such as older age, presence of antiphospholipid syndrome, splenic/renal infarct, lower limb thromboembolism, pulmonary embolism, myocardial infarction, and mitral valve regurgitation were significantly associated with increased risk of in-hospital mortality.

Summary: Nonbacterial thrombotic endocarditis (NBTE) is a rare condition associated with advanced malignancy, causing noninfectious vegetative lesions on heart valves. This case describes a patient with atrial flutter who underwent transesophageal echocardiogram (TEE) cardioversion, but it was aborted due to the discovery of large mobile vegetation on the mitral valve leaflet. Further investigation revealed esophageal adenocarcinoma with metastases to the liver, adrenal glands, and perirectal lymph nodes. This case highlights the importance of TEE prior to cardioversion and EGD before and after gastric sleeve surgery to evaluate for esophageal cancer.

Summary: Based on the study findings, we suggest that all patients with triple positive APS, especially those with secondary APS and SLE, should undergo bridging therapy with intravenous UFH to reduce time without anticoagulation and minimize the risk of thrombotic complications. Furthermore, we propose that NICE include APS in the list of medical conditions which are high risk for thrombotic complications and require bridging therapy.

Summary: Systemic lupus erythematous (SLE) is an autoimmune disease that has a wide range of cardiovascular complications. The main manifestations include diseases of the coronary arteries, valves, pericardium, and myocardium. Multimodality cardiovascular imaging techniques are crucial in evaluating the extent of cardiac manifestations in SLE patients and providing valuable prognostic information. However, their utility has not been clearly defined before. This review provides an up-to-date overview of the cardiovascular manifestations of lupus, as well as the role of multimodality cardiac imaging in guiding the management of SLE patients. Summary: Spontaneous coronary artery dissection (SCAD) is a rare but increasingly recognized cause of acute coronary syndrome. There is currently no consensus on the treatment of SCAD due to its low incidence rate. Most of the information on SCAD comes from case reports and series, and there have been no prospective randomized studies to guide treatment. This article presents a case series of five SCAD patients and reviews the clinical presentation, incidence, diagnostic modalities, management, and risk of recurrence in patients diagnosed with SCAD.

Summary: The objective of this study was to develop a multi-parametric MAC score using CT features for predicting outcomes in patients undergoing mitral valve surgery. The MAC score, ranging from 2 to 12, was based on Agatston calcium score, MAC circumferential angle, involvement of trigones, myocardial infiltration, and aorto-mitral curtain extension. The MAC score was found to be significantly associated with operation time, cross-clamp time, in-hospital complications, and time to all-cause death.

Summary: This study investigated the long-term outcomes of septal reduction therapies in elderly Medicare patients with oHCM, as well as the relationship between hospital volume and outcomes. The results showed that septal myectomy was associated with lower mortality and need for redo procedures compared to alcohol septal ablation. Additionally, higher-volume centers had better outcomes. However, most patients underwent treatment at low-volume centers.

Summary: Renal transplant recipients are more susceptible to infections due to immunosuppression. The literature regarding infective endocarditis (IE) in these recipients is limited. A study of the National Inpatient Sample in the United States analyzed the epidemiology and outcomes of IE in renal transplant patients. The study identified risk factors for inpatient mortality and IE development in these patients. The analysis showed that Staphylococcal IE, shock and stroke were associated with higher inpatient mortality, while Streptococcal endocarditis was associated with lower mortality.

Summary: English Summary: The diagnosis of infective endocarditis (IE) can be challenging, especially in cases of prosthetic valve endocarditis (PVE) or cardiac device-related endocarditis (CDIE). While echocardiography is crucial for identifying IE, there are situations where transesophageal echocardiography (TEE) may not be conclusive or feasible. Intracardiac echocardiography (ICE) has emerged as a promising alternative for diagnosing IE and evaluating intracardiac infections, particularly when transthoracic echocardiography (TTE) is inconclusive and TEE is contraindicated. Additionally, ICE has been found to be useful in guiding transvenous lead extractions in infected implantable cardiac devices.
