BK Polyomavirus Evades Innate Immune Sensing by Disrupting the Mitochondrial Network and Promotes Mitophagy

Immune escape contributes to viral persistence, yet little is known about human polyomaviruses. BK-polyomavirus (BKPyV) asymptomatically infects 90% of humans but causes premature allograft failure in kidney transplant patients. Despite virus-specific T cells and neutralizing antibodies, BKPyV persists in kidneys and evades immune control as evidenced by urinary shedding in immunocompetent individuals. Here, we report that BKPyV disrupts the mitochondrial network and membrane potential when expressing the 66aa-long agnoprotein during late replication. Agnoprotein is necessary and sufficient, using its amino-terminal and central domain for mitochondrial targeting and network disruption, respectively. Agnoprotein impairs nuclear IRF3-translocation, interferon-beta expression, and promotes p62/SQSTM1-mitophagy. Agnoprotein-mutant viruses unable to disrupt mitochondria show reduced replication and increased interferon-beta expression but can be rescued by type-I interferon blockade, TBK1-inhibition, or CoCl2-treatment. Mitochondrial fragmentation and p62/SQSTM1-autophagy occur in allograft biopsies of kidney transplant patients with BKPyV nephropathy. JCPyV and SV40 infection similarly disrupt mitochondrial networks, indicating a conserved mechanism facilitating polyomavirus persistence and post-transplant disease.

Molecular Characterization of BK Polyomavirus Replication in Allogeneic Hematopoietic Cell Transplantation Patients

Summary: In this study, the replication of BK polyomavirus (BKPyV) was found to be associated with failing immune control and BKPyV-associated hemorrhagic cystitis in allogeneic hematopoietic cell transplantation (HCT). Molecular markers of BKPyV replication and disease were identified through quantitative nucleic acid testing (QNAT), DNase-I treatment, next-generation sequencing (NGS), and cell-mediated immunity testing. The fragmentation of BKPyV genome below 250 bp was found to impair the detection of BKPyV diversity and genotype-associated changes involved in immune escape.

Impact of nucleic acid extraction procedures on human papillomavirus (HPV) detection and genotyping

Summary: HPV infections are often asymptomatic, but certain genotypes can lead to cervical cancer. This study compared different nucleic acid extraction methods for detecting and genotyping HPV in cervical swabs with atypical cells. The results showed that centrifugation/enrichment improved HPV detection and genotyping accuracy. As multivalent HPV vaccine coverage increases, it is important to improve extraction methods and broaden genotype coverage to accurately detect the evolving HPV virome.

Amplicon size and non-encapsidated DNA fragments define plasma cytomegalovirus DNA loads by automated nucleic acid testing platforms: A marker of viral cytopathology?

Summary: Management of cytomegalovirus (CMV) in transplant patients relies on measuring plasma CMV-loads. The comparison between automated Roche-cobas (R) 6800-CMV and Roche-CAP/CTM-CMV with laboratory-developed Basel-CMV-UL54-95bp and Basel-CMV-UL111a-77bp showed qualitative concordance. DNase-I treatment significantly reduced CMV-loads for all assays. Non-encapsidated fragmented CMV-DNA is the major form of plasma CMV-loads measured by fully-automated platforms.

In the seventh guideline, they focused on respiratory syncytial virus (RSV) and provided answers to FAQs regarding epidemiology, clinical diagnosis, prophylaxis, and treatment.
