Pigment Epithelium-derived Factor secreted by corneal epithelial cells regulates dendritic cell maturation in dry eye disease

Purpose: In this study, we quantify Pigment Epithelium-derived Factor (PEDF) secreted by corneal epithelial cells and evaluate its immunomodulatory functions in a murine model of dry eye disease (DED). Methods: We induced DED in female C57BL/6 mice using a controlled environment chamber for 14 days. We quantified mRNA expression of Serpinf1 gene and PEDF protein synthesis by corneal epithelial cells (CEpCs) using RT-PCR and ELISA. CEpCs from normal or DED mice were cultured with IFN gamma-stimulated-dendritic cells (DCs) for 24 h, and expression of MHC-II and CD86 by DCs was determined using flow cytometry. Next, we either added recombinant PEDF (rPEDF) or anti-PEDF antibody to co-culture, and DC expression of the above maturation markers was quantified. Lastly, we treated DED mice with either topical rPEDF, anti-PEDF Ab or murine serum albumin (MSA), and DC maturation, expression of pro-inflammatory cytokines, and DED severity were investigated. Results: Serpinf1 mRNA expression and PEDF protein production levels by CEpCs were upregulated in DED. CEpCs from DED mice exhibited an enhanced suppressive effect on the expression of MHC-II and CD86 by DCs, compared to normal mice. This effect was abolished by blocking endogenous PEDF with anti-PEDF Ab or enhanced by supplementing with rPEDF. Treatment with anti-PEDF antibody blocked the effect of endogenousPEDF and increased DC maturation, expression of pro-inflammatory cytokines in conjunctivae, and exacerbated disease severity in DED mice. Conversely, topical rPEDF enhanced the suppressive effect of endogenous PEDF on DC maturation, decreased expression of pro-inflammatory cytokines in conjunctivae, and reduced disease severity. Conclusions: The results from our study elucidate the role of PEDF in impeding DC maturation, and suppression of ocular surface inflammation, explicating a promising therapeutic potential of PEDF in limiting the corneal epitheliopathy as a consequence of DED.

Summary: Bacterial keratitis remains one of the leading causes of corneal blindness, and knowledge gaps hinder early and accurate diagnosis and management. The complex regulatory networks, unique virulence factors, and rapid secretion of harmful proteins and toxins in bacteria make treatment challenging. This review provides an overview of diagnostic techniques and therapies for bacterial keratitis, recent advancements in accurately diagnosing mono- and poly-bacterial corneal infections, and understanding antibiotic resistance at the sub-cellular genomic level. The development of novel therapies and drug delivery systems for bacterial keratitis management is also discussed.

Ocular complications of plasma cell dyscrasias

Summary: Plasma cell dyscrasias are a wide range of severe monoclonal gammopathies caused by over-secretion of abnormal monoclonal antibodies from pre-malignant or malignant plasma cells. This review examines ocular complications associated with various types of plasma cell dyscrasias. Wide-ranging ocular presentations have been identified in both the anterior and posterior segments of the eye, and early diagnosis can be facilitated by recognizing the presenting symptoms. It is important for ophthalmologists and oncologists to maintain a high clinical suspicion and promptly diagnose these ocular signs to prevent disease progression.

Clinical Characterization and Outcomes of Culture- and Polymerase Chain Reaction-Negative Cases of Infectious Keratitis

Summary: This retrospective case series evaluated the clinical history and presentation of culture and PCR-negative infectious keratitis cases. Out of 121 cases included, the most common presumed etiology at presentation was viral (38.01%) followed by bacterial (27.27%) and fungal (8.26%). Risk factors included ocular surface diseases and contact lens use. The visual outcomes were generally favorable, but a history of ocular surface disease and poor contact lens hygiene were associated with worse outcomes.

Analysing the change in contrast sensitivity post-travoprost treatment in primary open-angle glaucoma patients using Spaeth Richman contrast sensitivity test

Summary: The objective of this study was to assess the ability of the Internet-based SPARCS method to measure the change in contrast sensitivity (both central and peripheral) after treatment with travoprost. The results showed that both central and peripheral contrast sensitivity improved following treatment with travoprost, and the change in contrast sensitivity was significantly correlated with RNFL thickness and perimetric indices.
