Summary: This study aimed to determine MRI parameters predictive of molecular subtype and risk stratification in glioma and develop a structured reporting system. Reproducible imaging parameters such as presence of necrosis, T2/FLAIR mismatch, internal cyst, and predominant contrast enhancement were identified as significant predictors across readers. The highest diagnostic performance was found in predicting risk type 5 with predominant enhancement, followed by risk types 2 and 1 with specific imaging features.

The radiomic models, composed of selected radiomics features and logistic regression (LR), support vector machine (SVM), and LR least absolute shrinkage and selection operator (LASSO) classifiers, demonstrated good predictive performance. The study showed the potential of using MR modalities and radiomics models as noninvasive tools for predicting the IDH mutation status in glioma.
