Neutrophil-to-lymphocyte ratio and platelet-tolymphocyte ratio in evaluation of inflammation in end-stage renal disease

Objective: To evaluate the relationship between neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), and inflammation in end-stage renal disease (ESRD) patients on maintenance hemodialysis (HD). Methods: 100 ESRD patients on maintenance HD (mean +/- SD age: 52.3 +/- 1.7 years, 52% were males) were included in this cross-sectional study. Data on patient demographics, dry weight, body mass index, duration of HD (months), etiology of ESRD, delivered dose of dialysis (spKt/V), complete blood count, blood biochemistry and inflammatory markers including hs-CRP (mg/L), TNF-alpha (pg/mL), NLR, and PLR were recorded in all patients and compared in patients with hs-CRP levels of <= 3 mg/L vs. > 3 mg/L. other study parameters were also recorded. Results: Compared to patients with lower hs-CRP levels, patients with hs-CRP levels of > 3 mg/L had significantly higher values for NLR (3.7 +/- 0.2 vs. 2.7 +/- 0.2, p < 0.01) and PLR (150.7 +/- 6.9 vs. 111.8 +/- 7.0, p < 0.001). Both NLR and PLR were positively correlated with hs-CRP (r = 0.333, p = 0.01 and r = 0.262, p = 0.001, respectively) and negatively correlated with transferrin saturation (%) (r = -0.418, p = 0.001 and r = -0.309, p = 0.002, respectively). Conclusion: Our findings in a cohort of ESRD patients on maintenance HD revealed higher values for NLR and PLR in patients with higher levels of inflammation along with a significant positive correlation of both NLR and PLR with hs-CRP levels. Being a simple, relatively inexpensive and universally available method, whether or not calculation of NLR and PLR offers a plausible strategy in the evaluation of inflammation in ESRD patients in the clinical practice should be addressed in larger scale randomized and controlled studies.

Can systemic immune inflammation index at admission predict in-hospital mortality in chronic kidney disease patients with SARS-CoV-2 infection?

Summary: This study found that patients with chronic kidney disease (CKD) who are infected with COVID-19 have a higher risk of severe disease and mortality. Hematological indices, including NLR, SII, PLR, and LCR, are associated with in-hospital mortality in CKD patients. Among these markers, NLR and SII have the best ability to distinguish patients at higher risk of death.

Daily variability of acid-base status and determinants of serum bicarbonate in hemodialysis patients

Summary: This study aimed to evaluate the daily variability of bicarbonate in hemodialysis patients and identify the factors that affect this issue. The results showed that relying solely on pre-dialysis tCO2 values to define metabolic acidosis may lead to misclassification, as the agreement between pre- and post-dialysis tCO2 and HCO3- was not good.

Prognostic value of histopathological scoring and grading in patients with renal AA amyloidosis

Summary: This study aimed to analyze the impact of histological characteristics on renal outcome in patients with biopsy-proven renal AA amyloidosis. The results showed that baseline renal function was closely linked to the extent of AA amyloid deposit in renal parenchyma, while only vascular amyloid load was a predictor of renal outcome in AAA patients.

Peritoneal Dialysis-related Peritonitis: Microbiological Profile and Outcome

Summary: This study investigated the incidence, causative agents, and outcomes of peritonitis episodes in patients undergoing peritoneal dialysis. The results showed that both Gram-positive and Gram-negative organisms were common causes of peritonitis, with a higher rate of catheter removals and lower resolution rate observed in Gram-negative cases. The findings suggest that knowledge of prevalent microbial agents and appropriate treatment adjustments are important in reducing the incidence of peritonitis.

Acute kidney injury in Turkey: epidemiological characteristics, etiology, clinical course, and prognosis

Summary: This study evaluated the etiologies, comorbidities, and outcomes of AKI in Turkey. The results showed that prerenal etiologies were more common, with nephrotoxic agent exposure being the most common renal etiology. A high proportion of patients required kidney replacement therapy, and the recovery rate at 6 months varied. Patients from the Eastern Anatolian region had a higher mortality rate.

Unusual occurrence of leptospirosis in a hemodialysis patient in a nonendemic region

Summary: This is a case report on a nonendemic occurrence of leptospirosis caused by Leptospira in a 44-year-old male living in a Syrian refugee camp in the arid city of Sanliurfa. The patient presented with jaundice and renal failure. This case serves as an example of leptospirosis in a nonendemic area, and provides a brief overview of relevant literature.

Prospective observational study of purse-string suturing of the anterior rectus fascia for urgent-start peritoneal dialysis

Summary: This study aimed to assess the efficacy and safety of a consistent percutaneous procedure for peritoneal dialysis (PD) catheter placement in initiating immediate-start PD in patients with end-stage kidney disease (ESKD). The results showed that it is feasible to start PD within 1 hour of catheter placement, with no cases of leakage, technique failure, or the need for hemodialysis. Although there were minor complications, it is of significant importance for initiating PD.

Sarcopenia and cardiovascular disease in patients with and without kidney disease: what do we know?

Summary: Cardiovascular disease is common in patients with chronic kidney disease and is the leading cause of mortality.
