This case report describes the clinical characteristics of a 50-year-old woman that developed SARS-CoV-2 pneumonia and was admitted at the COVID-19 dedicated unit where she developed neurological symptoms 10 days after admission. After neurological examination, including a panel of blood cerebrospinal fluid biomarkers, a diagnosis of Miller Fisher syndrome (MFS) was hypothesized and intravenous immunoglobulin therapy (IVIG) was initiated. Fourteen days after the start of IVIG treatment, the patient has been discharged at home with the resolution of respiratory symptoms and only minor hyporeflexia at the lower limbs, without any side effect.

Lost diagnoses in not otherwise specified headache in Emergency Department

Antonio Granato, Laura D'Acunto, Maria Elisa Morelli, Giulia Bellavita, Franco Cominotto, Paolo Manganotti

Summary: The diagnosis of NOS headaches in the Emergency Department is common despite specialist visits, with only 37% of patients receiving a precise diagnosis. Neurologists made diagnoses slightly more frequently than non-neurologists, but the ED physician often did not report specialist diagnoses in the discharge sheet. This leads to an overestimation of NOS headaches and a loss of specific diagnoses.

Autonomic dysfunction in post-COVID patients with and witfhout neurological symptoms: a prospective multidomain observational study

Alex Buoite Stella, Giovanni Furlanis, Nicolo Arjuna Frezza, Romina Valentinotti, Milos Ajcevic, Paolo Manganotti

Summary: The study revealed that dysautonomia may be a possible complication in post-COVID individuals, presenting symptoms such as orthostatic intolerance, sudomotor dysfunction, gastrointestinal issues, and pupillomotor dysfunction. Patients with neurological symptoms had a higher COMPASS-31 score due to more severe orthostatic intolerance symptoms compared to those without neurological symptoms.

Application of Ultrasonography in Neurogenic Dysphagia: A Systematic Review

Summary: Swallowing disorders are common in neurological diseases, and ultrasound imaging has been suggested as a useful tool for evaluating dysphagia. It provides measures of both static and dynamic anatomical components, aiding in the identification and early rehabilitation of swallowing difficulties. Studies have shown that ultrasound imaging correlates well with clinical evaluation and has prognostic and rehabilitation potential. However, due to the variety of tools, methods, and techniques used in the reviewed studies, standard procedures and outcomes have yet to be established.

Summary: We present a case of a 70-year-old man who developed rapidly progressing CJD with SARS-CoV-2 co-infection, ultimately leading to his death after 15 days of hospitalization. This rare clinical presentation highlights the challenges in diagnosis.

Summary: This systematic review aims to assess the incidence of benign tumors, malignant tumors, and non-neoplastic lesions in asymptomatic postpuberal males with incidentally detected testicular lesions. The study found a higher incidence of benign lesions with a diameter of <5 mm. Further research is needed to better understand the biology and management strategy for small testicular lesions.

Cognitive deficit in post-acute COVID-19: an opportunity for EEG evaluation?

Giovanni Furlanis, Alex Buoite Stella, Francesco Biaduzzini, Giulia Bellavita, Nicolo Arjuna Frezza, Sasha Olivo, Alina Menichelli, Alberta Lunardelli, Milos Ajcevic, Paolo Manganotti

Summary: This study aimed to investigate EEG alterations in individuals with self-reported cognitive deficit after COVID-19. Among 90 patients attending a post-COVID neurology service, 20 patients with brain fog 4 weeks after non-severe COVID-19 infection were studied. EEG analysis revealed abnormalities in 65% of the sample, with 69% showing slowing activity and 31% presenting epileptic discharges in frontal areas. None of the patients exhibited DWI MRI lesions.

Summary: This study analyzed common refurbishment interventions applied to a social housing building in Italy and considered internal comfort during the summer season in the design phase. The results showed that both building insulation and climatic change affected heating consumption, but building insulation had little effect on internal comfort while climate change increased the percentage of hours of discomfort.
