Smell and taste recovery in coronavirus disease 2019 patients: a 60-day objective and prospective study

Background The long-term recovery rate of chemosensitive functions in coronavirus disease 2019 patients has not yet been determined. Method A multicentre prospective study on 138 coronavirus disease 2019 patients was conducted. Olfactory and gustatory functions were prospectively evaluated for 60 days. Results Within the first 4 days of coronavirus disease 2019, 84.8 per cent of patients had chemosensitive dysfunction that gradually improved over the observation period. The most significant increase in chemosensitive scores occurred in the first 10 days for taste and between 10 and 20 days for smell. At the end of the observation period (60 days after symptom onset), 7.2 per cent of the patients still had severe dysfunctions. The risk of developing a long-lasting disorder becomes significant at 10 days for taste (odds ratio = 40.2, 95 per cent confidence interval = 2.204-733.2,p= 0.013) and 20 days for smell (odds ratio = 58.5, 95 per cent confidence interval = 3.278-1043.5,p= 0.005). Conclusion Chemosensitive disturbances persisted in 7.2 per cent of patients 60 days after clinical onset. Specific therapies should be initiated in patients with severe olfactory and gustatory disturbances 20 days after disease onset.

Summary: The aim of this study was to evaluate the prevalence of smell and taste dysfunction 2 years after mildly symptomatic SARS-CoV-2 infection, taking into account the background of chemosensory dysfunction in the non-COVID-19 population. The results showed that cases of SARS-CoV-2 infection still exhibited a significant excess of olfactory dysfunction 2 years after the infection when compared to matched controls.

Summary: Accurately fitted subperiosteal bone flaps can be a safe and effective solution for primary maxillary reconstructions in patients who cannot undergo free bone flap reconstructions. This type of custom-made implant provides support to the soft tissues of the middle-third of the face and a solid anchorage for dental prostheses or palatal obturators, improving the quality of life for patients.

Summary: Although scalp avulsion is rare, it poses challenges in terms of management and esthetic restoration due to involvement of neurovascular structures and scar retraction. Various algorithms and techniques are available for extensive scalp defect reconstruction, depending on the type of defect. In this study, we describe six cases of total scalp avulsion that underwent immediate combined reconstruction with split-thickness skin graft (STSG) and Integra(R) matrix.

Locoregional Flaps for the Reconstruction of Midface Skin Defects: A Collection of Key Surgical Techniques

Summary: This study reviewed common surgical techniques for midface reconstruction, including the bilobed flap, rhomboid flap, facial-artery-based flaps (nasolabial flap, island composite nasal flap, retroangular flap), cervicofacial flap, paramedian forehead flap, frontal hairline island flap, keystone flap, Karapandzic flap, Abbe flap, and Mustarde flap. The analysis found that studying facial subunits, determining the location and size of the defect, selecting the appropriate flap, and preserving vascular pedicles are key factors for optimal outcomes.

Minimally Invasive Harvesting Technique for Costal Cartilage Graft: Donor Site, Morbidity and Aesthetic Outcomes

Summary: This study describes a new technique for harvesting cartilage grafts in reconstructive surgery with a small incision size of less than 1.5 cm. The technique was successfully applied to 36 patients without major complications such as infections or chest wall deformities. The Vancouver Scar Scale was used to evaluate postoperative scarring, and the results showed minimal scarring at both 1 week and 6 months after the procedure. This minimally invasive method provides a valid and effective approach for cartilage grafts.

Summary: This study aimed to improve the effectiveness of pre-surgical diagnosis by using a machine learning tool to analyze inflammatory biomarkers and radiomic metrics extracted from MRI images to differentiate between benign and malignant salivary gland tumors. The results showed that both inflammatory biomarkers and radiomic features were capable of supporting a differential diagnosis between different types of tumors.

Summary: This study found that the prevalence of oropharyngeal squamous cell carcinomas (OPSCCs) driven by HPV infection is increasing in Northeastern Italy. The study also suggests that the prevalence of HPV infection impacts the positive predictive value of p16(INK4a) overexpression. Therefore, each institution should consider the subsite-specific prevalence rates of HPV-driven OPSCC when using p16(INK4a) overexpression as a surrogate marker for transforming HPV infection.

Tongue Reconstruction with Buccinator Myomucosal Island Flaps: Technical Considerations, Oncologic Safety, Functional Outcomes and QoL Assessment-A Retrospective Observational Study

Summary: The study investigated the effectiveness of buccinator myomucosal island flaps for tongue reconstruction after malignant tumor resections. A retrospective study on 52 patients showed successful transposition of all flaps without total loss, no cancer relapses, and a high recovery rate of touch, two-point, and pain sensations. The swallowing score was high with minor complaints and the quality of life assessments demonstrated high scores across all aspects. In conclusion, buccinator myomucosal island flaps are an effective option for tongue reconstruction with low morbidity and high quality of life.

Exploratory Study on Chemosensory Event-Related Potentials in Long COVID-19 and Mild Cognitive Impairment: A Common Pathway?

Summary: This study investigated chemosensory-evoked related potentials (CSERP) in patients with Long COVID-19 (LC) syndrome and Mild Cognitive Impairment (MCI) to understand if there is a common pathway for the similar symptoms observed in these disorders. The results showed reduced N1 amplitude in the left frontoparietal network, involved in working memory and attentional deficits, in both MCI and LC patients. LC patients also exhibited a reduction in P3 latency. These findings suggest that CSERPs could serve as valid biomarkers for assessing the progress of olfactory dysfunction (OD) and other impairments in LC and MCI.

Indications, Efficacy, Safety, and Clinical Outcomes of 585 nm Pulsed Dye Laser in Non-Malignant Laryngeal Lesions: A Systematic Review

Summary: This manuscript reviewed the use of a 585 nm pulsed dye laser (PDL) in non-malignant laryngeal lesions. The results showed that PDL can effectively improve vocal quality and treat vascular lesions, dysplasia/leukoplakia, Reinke's edema, granulomas, and recurrent respiratory papillomatosis. The procedure is well-tolerated and has minimal complications. However, there is a lack of consensus on laser settings and standard evaluation methods for vocal outcomes, indicating the need for further controlled studies.
