Head, Neck, and Abdominopelvic Septic Thrombophlebitis: Current Evidence and Challenges in Diagnosis and Treatment

Septic thrombophlebitis (STP) is a complex, cross-disciplinary clinical condition that combines a localized infection with a neighboring venous thrombosis. STP can occur at several possible anatomic sites, such as dural sinuses, jugular vein (Lemierre syndrome), portal vein (pylephlebitis), and pelvic veins. Its high mortality in the preantibiotic era improved considerably with the introduction of modern antibiotics. However, little evidence exists to date to guide its clinical management. The incidence of STP or its risk factors may be increasing, and its mortality may still be considerable. These trends would have far-reaching implications, especially in the setting of increasing resistance to antimicrobial agents. No clinical assessment tools exist to support patient screening or guide treatment in STP. Few interventional studies exist on the efficacy and safety of anticoagulation. Recommendations on its indications, duration, and the agents of choice are mostly based on evidence derived from small observational studies. While all forms of STP pose similar challenges, future research may benefit from the distinction between bacteria-associated, virus-associated, and mycosis-associated thrombophlebitis. Addressing these gaps in evidence would enhance our ability to diagnose this condition and treat patients effectively.

Summary: Between 2008 and 2019, the age-standardized mortality rate related to PE in Veneto, Northern Italy decreased significantly. Proportionate mortality from PE remained higher in women, and the association between cancer and PE in deaths became stronger over time, indicating a change in risk factor distribution.

Preexisting Chronic Thromboembolic Pulmonary Hypertension in Acute Pulmonary Embolism

Summary: This study found that 15% of patients with acute pulmonary embolism (PE) showed signs of chronic PE or chronic thromboembolic pulmonary hypertension (CTEPH) on CT pulmonary angiography (CTPA). Specific radiologic parameters, such as intravascular webs, arterial narrowing or retraction, dilated bronchial arteries, and right ventricular hypertrophy, could help identify preexisting CTEPH. Early diagnosis of CTEPH can be facilitated by targeted follow-up strategies and risk-adapted CTEPH screening based on these parameters.

Modelling changes in glass transition temperature in polymer matrices exposed to low molecular weight penetrants

Summary: This study investigates the depression of glass transition temperature (T-g) in polymer matrices when in contact with low molecular weight compounds. The mechanical action of compressive stress exerted by the pressure of the fluid phase also affects T-g. A modeling approach based on the Gibbs-Di Marzio criterion is proposed to predict rubbery-glassy states maps of polymer-penetrant mixtures. The configurational entropy is modeled using the Non-Random Hydrogen Bond lattice fluid theory.

Septic Pulmonary Embolism. A Contemporary Profile

Summary: Septic pulmonary embolism (SPE) is a rare clinical condition that differs from non-septic thrombotic pulmonary embolism. The diagnosis of SPE relies on specific radiologic findings and prompt identification of the primary source of infection is important. Invasive treatment and broad-spectrum antimicrobial therapy may be necessary. SPE has a high in-hospital mortality rate and may be associated with the opioid epidemic and increased cases of septic thrombophlebitis.

The results showed that V/Q SPECT/CT has high sensitivity and specificity for the diagnosis of acute PE, while Q SPECT/CT has high sensitivity but limited specificity for the diagnosis of PE.

Summary: The maternal mortality rates related to pulmonary embolism in the United States did not decrease from 2003 to 2020, despite the decrease in mortality rates related to pulmonary embolism in the general population. There were differences in the rates across different maternal age groups and racial/ethnicity groups. Further research is needed to evaluate the effectiveness of venous thromboembolism prevention and pulmonary embolism diagnosis and management strategies in reducing deaths resulting from pulmonary embolism in this vulnerable population.

Navigating through the haemostatic paradox in kidney failure: A practical overview

Summary: Chronic kidney disease (CKD) affects approximately 9.1% of the global population, resulting in a significant health burden. Some individuals with CKD may require renal replacement therapy due to complete kidney failure. These patients are at increased risk of both bleeding and thrombosis, making their management challenging. Limited research has been conducted on the effects of antiplatelet agents and anticoagulants in this vulnerable subgroup of patients. This review aims to provide an overview of the current understanding of haemostasis in end-stage kidney disease and discuss evidence-based management strategies for common haemostasis challenges.

Cardiopulmonary exercise testing during follow-up after acute pulmonary embolism

Summary: This study investigated the association between exercise limitation assessed by cardiopulmonary exercise testing (CPET) and clinical, echocardiographic, laboratory abnormalities, and quality of life (QoL) after pulmonary embolism (PE). The results showed that abnormal exercise capacity of cardiopulmonary origin is common after PE and is associated with clinical and haemodynamic impairment, as well as long-term QoL reduction. CPET can be considered for selected patients with persisting symptoms after PE to identify candidates for closer follow-up and possible therapeutic interventions.

Anticoagulant therapy for splanchnic vein thrombosis: recent updates for patients with liver cirrhosis

Summary: Liver cirrhosis is often accompanied by changes in hemostasis, which contributes to the theory of rebalanced hemostasis. Splanchnic vein thrombosis (SVT) is a common complication in cirrhotic patients (17-26% of cases), with liver cirrhosis being a major risk factor for SVT (24-28% of SVT cases). This narrative review focuses on the current state of anticoagulant treatment for cirrhotic SVT, particularly the potential role of direct oral anticoagulants (DOACs) and recent guidelines in this area.

Summary: Cancer patients have an increased risk of developing venous thromboembolism in unusual areas such as the abdominal veins. Cancer-associated abdominal vein thrombosis is associated with high mortality rates and a high risk of recurrent thrombosis. The management of cancer-associated abdominal vein thrombosis follows general guidelines for venous thromboembolism.
