Hepatic consequences of COVID-19 infection. Lapping or biting?

The outbreak of coronavirus disease 2019 (COVID-19) starting last December in China placed emphasis on liver involvement during infection. This review discusses the underlying mechanisms linking COVID-19 to liver dysfunction, according to recent available information, while waiting further studies. The manifestations of liver damage are usually mild (moderately elevated serum aspartate aminotransferase activities), and generally asymptomatic. Few patients can still develop severe liver problems, and therapeutic options can be limited. Liver dysfunction may affect about one-third of the patients, with prevalence greater in men than women, and in elderly. Mechanisms of damage are complex and include direct cholangiocyte damage and other coexisting conditions such as the use of antiviral drugs, systemic inflammatory response, respiratory distress syndrome-induced hypoxia, sepsis, and multiple organ dysfunction. During new COVID-19 infections, liver injury may be observed. If liver involvement appears during COVID-19 infection, however, attention is required. This is particularly true if patients are older or have a pre-existing history of liver diseases. During COVID-19 infection, the onset of liver damage impairs the prognosis, and hospital stay is longer.

Effects of Biliary Phospholipids on Cholesterol Crystallization and Growth in Gallstone Formation

Summary: The prevalence of cholesterol gallstone disease is increasing globally due to obesity and insulin resistance. The role of biliary phospholipids in cholesterol gallstone formation has been neglected, but their reduction or deficiency leads to abnormal bile composition that predisposes to cholesterol supersaturation. Changes in biliary phospholipid concentrations influence cholesterol crystallization, resulting in different crystallization pathways. This review summarizes the chemical structures and functions of biliary phospholipids and discusses their role in cholesterol crystallization and gallstone formation.

Self-reported symptoms after COVID-19 vaccination. Distinct sex, age, and geographical outcomes in Lebanese and Italian cohorts

Summary: The study found that different COVID-19 vaccines were authorized in Italy and Lebanon in 2021, but the side effects and the impact of sex and age were not fully explored. A web-based questionnaire was designed to collect self-reported side effects after the 1st and 2nd dose of the vaccine in Italian and Lebanese cohorts. The study revealed that females had higher rates and severity of post-vaccinal symptoms compared to males, and the symptoms rates and severity decreased with increasing age.

NAFLD, MAFLD, and beyond: one or several acronyms for better comprehension and patient care

Summary: Non-alcoholic fatty liver disease (NAFLD) is the most common type of chronic liver disease characterized by excessive hepatic fat storage in patients with no or little alcohol consumption. The etiology and pathogenesis of NAFLD are largely unknown, and there is currently no definitive therapy. Due to its close association with metabolic dysfunctions, efforts are being made to change the acronym NAFLD to MAFLD, which is expected to improve disease classification, awareness, understanding, and choice of personalized therapeutic approaches. Other sub-classifications of fatty liver disease have also been proposed to address the heterogeneity of causes.

The Impact of Za'atar Antioxidant Compounds on the Gut Microbiota and Gastrointestinal Disorders: Insights for Future Clinical Applications

Summary: Given the important role of gut microbiota in host homeostasis and energy balance, changes in its composition can lead to immune-mediated inflammatory disorders and increased intestinal permeability. Za'atar, a popular plant-based food in the Eastern Mediterranean region, contains antioxidant compounds and fiber that have shown health benefits in cardiovascular and metabolic diseases. Studies have also explored the effects of Za'atar on gut microbiota and gastrointestinal diseases, especially its polyphenols. However, more research is needed on the potential clinical applications of Za'atar as a nutraceutical in gastrointestinal disorders.

Portal Hypertension in Nonalcoholic Fatty Liver Disease: Challenges and Paradigms

Summary: Portal hypertension in cirrhosis is defined as an increase in the portal pressure gradient (PPG), and it is traditionally estimated by the hepatic venous pressure gradient (HVPG). Nonalcoholic fatty liver disease (NAFLD) also leads to portal hypertension, which is more common in this condition than previously believed. A better understanding of liver mechanobiology in NAFLD may help in the development of novel pharmaceutical targets for prevention and treatment.

Metabolic dysfunction-associated gallstone disease: expecting more from critical care manifestations

Summary: About 20% of adults worldwide have gallstones, with 80% of them being composed of cholesterol. Gallstone disease is a condition caused by gallbladder stones, which requires medical treatment, endoscopic procedures, or cholecystectomy. It is one of the most prevalent digestive disorders in Western countries and a common reason for hospital admissions in Europe. The incidence of gallstone disease is increasing due to the global epidemic of insulin resistance, type 2 diabetes, obesity, and metabolic syndrome. Gallstones can be considered a metabolic dysfunction-associated disease, requiring specific and systemic preventive measures.

Trace Elements Levels in Major Depressive Disorder-Evaluation of Potential Threats and Possible Therapeutic Approaches

Summary: This article reviews the literature on the relationship between the concentration of selected elements in the serum of patients with major depressive disorder (MDD) and the onset and progression of this psychiatric condition. According to the results, metallomics may serve as a potential screening tool for patients with MDD, providing assessment of symptom severity. Deficiencies or excessive amounts of selected elements may be associated with the progression of depressive symptoms or even the onset of the disease in individuals predisposed to MDD.

Are children with gallstone disease more overweight? Results of a matched case-control analysis
