Patient-Specific Computational Analysis of Hemodynamics and Wall Mechanics and Their Interactions in Pulmonary Arterial Hypertension

Byron A. Zambrano, Nathan McLean, Xiaodan Zhao, Ju-Le Tan, Liang Zhong, C. Alberto Figueroa, Lik Chuan Lee, Seungik Baek

Summary: The study found that patients with pulmonary arterial hypertension (PAH) have stiffer and larger pulmonary artery walls, reduced relative area change, higher total resistance, and lower total compliance. Additionally, PAH patients have lower wall shear stress values at the main pulmonary arteries.

Imaging surveillance after open aortic repair: a feasibility study of three-dimensional growth mapping

Yunus Ahmed, Nitesh Nama, Ignas B. Houben, Joost A. van Herwaarden, Frans L. Moll, David M. Williams, C. Alberto Figueroa, Himanshu J. Patel, Nicholas S. Burris

Summary: The study explored the feasibility of a three-dimensional aortic growth assessment technique to quantify aortic growth in patients following open aortic repair. Vascular deformation mapping (VDM) was able to clearly depict localized aortic growth and provide a comprehensive assessment, showing a stronger correlation with targeted diameter change compared to clinical diameter change. VDM offers a more accurate assessment of aortic growth and could help identify post-surgical patients at high or low risk for future complications.

Characterization of Post-Operative Hemodynamics Following the Norwood Procedure Using Population Data and Multi-Scale Modeling

Jonathan Primeaux, Arash Salavitabar, Jimmy C. Lu, Ronald G. Grifka, C. Alberto Figueroa

Summary: The study combined population data and computational fluid dynamics (CFD) to analyze the hemodynamics of children with hypoplastic left heart syndrome (HLHS) immediately following and prior to surgical stages. Both post-stage I and pre-stage II models showed high-frequency flow components and increases in PA wall shear stress. These suboptimal conditions may be necessary to ensure adequate PA flow.

Inverse modeling framework for characterizing patient-specific microstructural changes in the pulmonary arteries

Reza Pourmodheji, Zhenxiang Jiang, Christopher Tossas-Betancourt, C. Alberto Figueroa, Seungik Baek, Lik-Chuan Lee

Summary: Microstructural changes in the pulmonary arteries associated with pulmonary arterial hypertension (PAH) were characterized using patient-specific inverse finite element modeling. Differences in stress and stiffness of elastin, collagen fibers, and smooth muscle cells were found between a PAH patient and a control subject. Elastin may play a key role in bearing load in the pulmonary arteries of both PAH patients and controls.

Noninvasive quantification of cerebrovascular pressure changes using 4D Flow MRI

David Marlevi, Jonas Schollenberger, Maria Aristova, Edward Ferdian, Yue Ma, Alistair A. Young, Elazer R. Edelman, Susanne Schnell, C. Alberto Figueroa, David A. Nordsletten

Summary: The study systematically evaluated the accuracy of image-based cerebrovascular relative pressure mapping, with results showing a close relationship between accurate estimations and spatial sampling. The nu WERP method demonstrated lower errors and higher correlation between resolutions, indicating its effectiveness in recovering relative pressures in cerebrovascular assessment.

Interventional Planning for Endovascular Revision of a Lateral Tunnel Fontan: A Patient-Specific Computational Analysis

Yunus Ahmed, Christopher Tossas-Betancourt, Pieter A. J. van Bakel, Jonathan M. Primeaux, William J. Weadock, Jimmy C. Lu, Jeffrey D. Zampi, Arash Salavitabar, C. Alberto Figueroa

Summary: A 2-year-old female with hypoplastic left heart syndrome (HLHS)-variant presented complications following single ventricle palliation, leading to elevated Fontan pathway pressures and significant dilation of the inferior Fontan pathway. Computational fluid dynamics (CFD) analysis revealed that the proposed endovascular revision would lead to unfavorable hemodynamic conditions, prompting the clinical team to reassess the patient's management. This study highlights the utility of CFD modeling in surgical planning for single ventricle CHD patients.

Jonas Schollenberger, Nicholas H. Osborne, Luis Hernandez-Garcia, C. Alberto Figueroa

Summary: This study presents a strategy to quantify cerebral hemodynamics in patients with CVOD using CFD and ASL. The feasibility and efficacy of this method were demonstrated through calibration and evaluation of blood flow distribution in two patients and a healthy control subject. Patient-specific CFD analysis showed advantages in assessing the hemodynamic impact of stenosis.

Comparative Study of Human and Murine Aortic Biomechanics and Hemodynamics in Vascular Aging

Sara E. Hopper, Federica Cuomo, Jacopo Ferruzzi, Nicholas S. Burris, Sara Roccabianca, Jay D. Humphrey, C. Alberto Figueroa

Summary: This study compared vascular wall mechanics and hemodynamics in humans and mice at two equivalent ages using fluid-solid-interaction (FSI) models, revealing species-specific differences in aortic compliance, resistance, and stiffness gradients as well as differences in response to aging. These results highlight the importance of recognizing key differences in vascular aging between human and mice when using mouse models for cardiovascular research.

Endovascular ascending aortic repair in type A dissection: A systematic review

Yunus Ahmed, Ignas B. Houben, C. Alberto Figueroa, Nicholas S. Burris, David M. Williams, Frans L. Moll, Himanshu J. Patel, Joost A. van Herwaarden

Summary: This study provides a comprehensive overview of catheter-based interventions in patients with acute type A aortic dissection, showing that endovascular repair is feasible with high technical success and acceptable short-term outcomes, despite higher rates of reintervention and endoleak. Further studies with longer follow-up and careful patient selection are needed before endovascular interventions can be widely introduced.
