Hyperlipidemia Caused by Voriconazole: A Case Report

Summary: This study reported a case of chronic pulmonary aspergillosis patient with significantly increased blood lipids after treatment with voriconazole. The patient's blood lipid levels returned to normal after discontinuation of voriconazole and switching to itraconazole treatment. The study suggested a positive causal relationship between voriconazole and dyslipidemia, which may be related to the trough concentration of voriconazole.

Clinical Features of Pulmonary Nocardiosis in Patients with Different Underlying Diseases: A Case Series Study

Summary: This study investigated the clinical features of pulmonary nocardiosis (PN) in patients with different underlying diseases. The results showed that PN mainly occurred in immunocompromised host patients, but also occurred in immunocompetent host cases to a lesser extent, especially in patients with bronchiectasis and/or COPD. Patients in the immunocompromised host group had a higher incidence of complicated extrapulmonary infections, and combination therapy with two antibiotics was commonly used in this group.

Summary: This study investigated the pathoadaptation mechanism of Acinetobacter baumannii in response to selective pressure exerted by pulmonary epithelial cells. Through experimental evolution, phenotypic characterization, and multi-omics analysis, the study revealed that A. baumannii exhibited mutations that altered its adhesion ability, anti-phagocytic ability, ribosome translation efficiency, and outer membrane permeability. These mutations also affected the susceptibility of A. baumannii to various antimicrobial agents.

The Clinical Impact of Metagenomic Next-Generation Sequencing (mNGS) Test in Hospitalized Patients with Suspected Sepsis: A Multicenter Prospective Study

Summary: We conducted a study to assess the diagnostic performance of metagenomic next generation sequencing (mNGS) in hospitalized patients with suspected sepsis and its role in guiding antimicrobial therapy. The mNGS test showed shorter turn-out time and higher sensitivity compared with blood culture, especially for fungal infections. It performed better for patients with mild symptoms, prior antibiotic use, and early stage of infection, and was capable of guiding antibiotic regimen modification and improving prognosis.

Clonal outbreak of NDM-1-producing Enterobacter hormaechei belonging to high-risk international clone ST78 with the coexistence of tmexCD2-toprJ2 and mcr-9 in China

Summary: This study reports the outbreak of NDM-1-producing Enterobacter hormaechei in a tertiary hospital in China. The strains belonged to a high-risk clone and co-harbored tmexCD2-toprJ2 and blaNDM-1, with 12 strains also carrying mcr-9. The study highlights the rapid evolution and potential for clonal dissemination of these strains, emphasizing the need for enhanced surveillance and control measures.

Zoonotic fungus Arthroderma multifidum causing chronic pulmonary infection

Summary: A rare case of Arthroderma multifidum infection occurred in a 63-year-old man with certain risk factors, including occupational exposure and immunosuppression following previous pulmonary tuberculosis and pneumothorax surgery. The pathogen was isolated and identified through gene sequencing, and its whole genome sequencing and analysis have been completed. This study provides important clinical insights on the ideal conditions for zoonotic fungal infections created by a combination of immune suppression and environmental exposure.

Antibacterial Activity of Eravacycline Against Carbapenem-Resistant Gram-Negative Isolates in China: An in vitro Study

Summary: In this study, the antibacterial activity of Eravacycline against carbapenem-resistant gram-negative bacteria (CRGNB) was analyzed. The results showed that Eravacycline had a broad-spectrum antimicrobial activity and a stronger bactericidal effect compared to tigecycline. This provides a theoretical basis for the future clinical treatment of drug-resistant bacterial infections.

Overexpression of blaGES-1 due to a strong promoter in the class 1 integron contributes to decreased ceftazidime-avibactam susceptibility in carbapenem-resistant Pseudomonas aeruginosa ST235

Summary: Sequence type 235 (ST235) Pseudomonas aeruginosa, associated with high morbidity and mortality, has a relatively high resistance to ceftazidime-avibactam (CZA), partly due to overexpression of blaGES-1 carried in a complex transposon. Overexpression of blaGES-1 combined with an efflux pump results in high-level CZA resistance, reducing therapeutic options for treating infections caused by ST235 P. aeruginosa. Surveillance initiatives are essential to prevent further dissemination of high-risk ST235 P. aeruginosa with CZA resistance.
