Is the use of ACE inb/ARBs associated with higher in-hospital mortality in Covid-19 pneumonia patients?

Introduction The present research aimed to determine the relation between the use of angiotensin-converting enzyme inhibitors (ACE inh) and angiotensinogen receptor blockers (ARBs) and in-hospital mortality of hypertensive patients diagnosed with Covid-19 pneumonia. Material and method In this retrospective study, we included 113 consecutive hypertensive patients admitted due to Covid-19 infection. In all patients, Covid-19 infection was confirmed with using reverse-transcription polymerase chain reaction. All patients were on ACE inh/ARBs or other antihypertensive therapy unless no contraindication was present. The primary outcome of the study was the in-hospital all-cause mortality. Results In total, 113 hypertensive Covid-19 patients were included, of them 74 patients were using ACE inh/ARBs. During in-hospital follow up, 30.9% [n = 35 patients] of patients died. The frequency of admission to the ICU and endotracheal intubation were significantly higher in patients using ACE inh/ARBs. In a multivariable analysis, the use of ACE inh/ARBs was an independent predictor of in-hospital mortality (OR: 3.66; 95%CI: 1.11-18.18;p= .032). Kaplan-Meir curve analysis displayed that patients on ACE inh/ARBs therapy had higher incidence of in-hospital death than those who were not. Conclusion The present study has found that the use of ACE inh/ARBs therapy might be associated with an increased in-hospital mortality in patients who were diagnosed with Covid-19 pneumonia. It is likely that ACE inh/ARBs therapy might not be beneficial in the subgroup of hypertensive Covid-19 patients despite the fact that there might be the possibility of some unmeasured residual confounders to affect the results of the study.

Serum Uric Acid to Albumin Ratio Can Predict Contrast-Induced Nephropathy in ST-Elevation Myocardial Infarction Patients Undergoing Primary Percutaneous Coronary Intervention

Summary: Contrast-induced nephropathy (CIN) is a common complication following primary percutaneous coronary intervention (pPCI) in ST-elevation myocardial infarction (STEMI) patients. A novel marker, serum uric acid to albumin ratio (UAR), has been found to be associated with acute kidney injury in intensive care unit patients. This study investigated the predictive value of UAR for the development of CIN in STEMI patients after pPCI and found that UAR was an independent predictor of CIN.

Summary: This study aimed to explore the relationship between serum uric acid/albumin ratio (UAR) and no-reflow (NR) in ST elevation myocardial infarction patients who underwent primary percutaneous coronary intervention. The study found that a high UAR was an independent risk factor for NR, and the AUC value of UAR in detecting NR was higher than that of its components, albumin and serum uric acid.

Summary: The protective effect of obesity, known as the obesity paradox, in acute coronary syndrome patients undergoing percutaneous coronary intervention or bypass surgery was investigated in this comprehensive meta-analysis. Overweight patients had a lower mortality risk compared to normal-weight patients for both short-term and long-term mortality. Additionally, low-weight patients had a higher risk of mortality than normal-weight patients.

Evaluation of Naples Score for Long-Term Mortality in Patients With ST-Segment Elevation Myocardial Infarction Undergoing Primary Percutaneous Coronary Intervention

Summary: The Naples score (NS), a composite of cardiovascular adverse event predictors, has been proven to be a prognostic risk score in cancer patients. This study aimed to investigate the predictive value of NS for long-term mortality in ST-segment elevation myocardial infarction patients. The results showed that NS was independently associated with long-term mortality, and adding NS to a baseline model improved the prediction and discrimination of long-term mortality. NS could be used for risk stratification of long-term mortality in STEMI patients undergoing primary percutaneous coronary intervention.

Predictive Value of the Naples Prognostic Score for Acute Kidney Injury in ST-Elevation Myocardial Infarction Patients Undergoing Primary Percutaneous Coronary Intervention

Summary: The purpose of this study was to investigate the association between the Naples prognostic score and the development of acute kidney injury (AKI) in ST-elevation myocardial infarction (STEMI) patients undergoing primary percutaneous coronary intervention (pPCI). A total of 2901 consecutive STEMI patients who underwent pPCI were included in the study. The Naples prognostic score was found to be the most significant predictor of AKI occurrence after considering factors such as admission creatinine, age, and contrast volume. The continuous Naples prognostic score model provided the best prediction performance and discriminative ability. The study suggests that the Naples prognostic score may be useful in predicting the risk of AKI in STEMI patients undergoing pPCI.

The association between frontal QRS-T angle and reverse dipper status in newly diagnosed hypertensive patients

Summary: This study investigated the association between the fQRS-T angle and reverse dipper hypertension in newly diagnosed hypertensive patients without LVH. The results showed that individuals with reverse dipper hypertension had significantly higher fQRS-T angles compared to those with normal and non-reverse dipper hypertension. Multivariate analysis also showed an independent association between the fQRS-T angle and reverse dipper hypertension.

Inadvertent misplacement of pacemaker lead presented with acute ischemic stroke

Summary: If a permanent pacemaker lead is misplaced in the left ventricle (LV), it can cause complications such as heart rhythm problems and blood clots. In this case, a 78-year-old patient presented with an embolic stroke caused by an LV lead passing through a patent foramen ovale and being misplaced in the LV. Thrombus regression was achieved with anticoagulation and lead extraction was planned. Lead extraction is a priority in acute cases, but not the primary approach for long-term leads misplaced in the LV. A patient-based individual approach should be preferred in such cases.

Summary: This study analyzed the influence of pre-implantation left ventricular mass index (LVMI) on pacing-induced cardiomyopathy (PICM) in patients with dual chamber permanent pacemakers (PPMs) due to complete AV block. The findings showed that higher LVMI was associated with an increased risk of long-term PICM. Overall, this study highlights the prognostic role of LVMI in predicting PICM.

Effect of ivabradine on ventricular arrhythmias in heart failure patients with reduced ejection fraction

Summary: This study investigated the effect of ivabradine on life-threatening arrhythmias and long-term mortality in heart failure patients with reduced ejection fraction. The results showed a lower likelihood of ventricular tachycardia in patients using ivabradine, as well as a lower presence of premature ventricular contractions and ventricular tachycardias. Additionally, ivabradine users had fewer appropriate implantable cardioverter-defibrillator shocks, and long-term mortality did not differ between the groups.
