Background Intestinal and diffuse gastric adenocarcinomas differ in clinical, epidemiological and molecular features. However, most of the concepts related to the intestinal-type are translated to gastric adenocarcinoma in general; thus, the peculiarities of the diffuse-type are underappreciated. Results Besides its growing importance, there are many gaps about the diffuse-type carcinogenesis and, as a result, its epidemiologic and pathogenetic features remain poorly understood. Conclusions Alternative hypotheses to explain these features are discussed, including the role of the gastric microbiota, medical therapies, and modifications in the stomach's microenvironment.

Summary: The cohort study showed that autoantibody positivity may indicate subclinical autoimmune gastritis in younger women, particularly in H pylori-seronegative individuals. The findings among young females and corpus subsite align with increasing cancer incidence trends for these groups, suggesting autoimmune gastritis may be replacing H pylori as the driving factor.

Knockdown of certain lncRNAs led to reduced cell proliferation, migration, and invasion. Gene co-expression network analysis revealed associations between deregulated lncRNAs and various biological processes, such as cell adhesion and DNA repair, with potential implications for therapeutic development.

Summary: Our study provides a comprehensive assessment of anti-H. pylori antibody profiles in gastric intestinal metaplasia (IM). The identified novel antibodies against target antigens, in addition to CagA, may play a role in the progression of IM. Specific H. pylori antibodies, along with other biomarkers, can help identify IM patients for surveillance.

Volatile Markers for Cancer in Exhaled Breath-Could They Be the Signature of the Gut Microbiota?

Summary: This study investigated the correlation between volatile organic compounds (VOCs) in exhaled breath and the fecal microbiota in gastric cancer patients and healthy controls. The results showed that this correlation differs between cancer patients and healthy controls. The volatile metabolites in exhaled breath were highly correlated with fecal bacterial taxa in cancer patients, while the correlation was lower in healthy controls.

Summary: This study found a potential association between proton-pump inhibitors (PPI) and gastric cancer, which may be mainly due to reverse causality.
