The COVID-19 outbreak has led to 80,409 diagnosed cases and 3,012 deaths in mainland China based on the data released on March 4, 2020. Approximately 3.2% of patients with COVID-19 required intubation and invasive ventilation at some point in the disease course. Providing best practices regarding intubation and ventilation for an overwhelming number of patients with COVID-19 amid an enhanced risk of cross-infection is a daunting undertaking. The authors presented the experience of caring for the critically ill patients with COVID-19 in Wuhan. It is extremely important to follow strict self-protection precautions. Timely, but not premature, intubation is crucial to counter a progressively enlarging oxygen debt despite high-flow oxygen therapy and bilevel positive airway pressure ventilation. Thorough preparation, satisfactory preoxygenation, modified rapid sequence induction, and rapid intubation using a video laryngoscope are widely used intubation strategies in Wuhan. Lung-protective ventilation, prone position ventilation, and adequate sedation and analgesia are essential components of ventilation management.

Pressure-Regulated Ventilator Splitting for Disaster Relief: Design, Testing, and Clinical Experience

Micha Sam Brickman Raredon, Clark Fisher, Paul M. Heerdt, Robert B. Schonberger, Alyssa Nargi, Steven Nivison, Elaine Fajardo, Ranjit Deshpande, Shamsuddin Akhtar, Allison M. Greaney, Joseph Belter, Thomas Raredon, Joseph Zinter, Andrew McKee, Mark Michalski, Pavlina Baevova, Laura E. Niklason

Summary: The COVID-19 pandemic has highlighted the need for an affordable and safe method to expand ventilator capacity. This study presents a ventilator-splitting circuit that allows for individualized ventilatory support for multiple patients while insulating them from each other's condition changes.

Risk factors and outcomes for patients with bleeding complications receiving extracorporeal membrane oxygenation: An analysis of the Chinese Extracorporeal Life Support Registry

Chenglong Li, Tong Cai, Haixiu Xie, Liangshan Wang, Feng Yang, Zhongtao Du, Xiaoyang Hong, Chengbing Zhou, Yimin Li, Haibo Qiu, Zhaohui Tong, Hui Zeng, Zhiyong Peng, Xin Li, Xiaotong Hou

Summary: This study analyzed the occurrence, risk factors, and clinical outcomes of bleeding complications in patients supported with extracorporeal membrane oxygenation (ECMO). The results showed that 18.1% of ECMO patients in the Chinese Society of Extracorporeal Life Support (CSECLS) registry experienced bleeding complications, which were associated with higher in-hospital mortality, especially in patients who received extracorporeal cardiopulmonary resuscitation (ECPR), patients on circulatory support, and pediatric patients.

Mesenchymal Stem Cell-Secreted TGF-β1 Restores Treg/Th17 Skewing Induced by Lipopolysaccharide and Hypoxia Challenge via miR-155 Suppression

Summary: This study found that under hypoxic and inflammatory conditions, the transforming growth factor-β1 (TGF-β1) secreted by mesenchymal stem cells (MSCs) can regulate the Treg/Th17 skewing induced by lipopolysaccharide (LPS) and hypoxia by inhibiting the expression of miR-155. These findings provide evidence for future T lymphocyte-targeted cell therapy in specific conditions.

Beneficial effect of Xuebijing against Pseudomonas aeruginosa infection in Caenorhabditis elegans

Le Zhang, Yuxing Wang, Chang Cao, Yike Zhu, Wei Huang, Yi Yang, Haibo Qiu, Songqiao Liu, Dayong Wang

Summary: In this study, the potential of Xuebijing treatment against bacterial infection and the underlying mechanisms were investigated using Caenorhabditis elegans as an animal model. The results showed that Xuebijing could inhibit the reduction of lifespan caused by Pseudomonas aeruginosa infection by inhibiting colonization of the bacteria in the intestinal lumen. Xuebijing treatment also suppressed the increase in antimicrobial gene expression induced by Pseudomonas infection.

Regional anesthesia for transplantation surgery - A white paper part 1: Thoracic transplantation surgery

Michael Ander, Neal Mugve, Cara Crouch, Cale Kassel, Kyota Fukazawa, Robert Izaak, Ranjit Deshpande, Charles McLendon, Jiapeng Huang

Summary: Transplantation surgery has benefited from advances in technique and technology, particularly in the use of regional anesthesia to provide analgesia and reduce opioid use. However, standardized practices for regional anesthesia in transplantation surgery are lacking, and no formal guidelines exist. To address this, a task force reviewed available literature and found that regional anesthesia is effective in controlling postoperative pain after transplantation surgeries. Part 1 of their review focuses on thoracic transplantation surgeries, while part 2 covers abdominal transplantations.

Summary: This study investigated hospital-acquired bloodstream infections (HA-BSI) in 2600 adult patients from 333 ICUs in 52 countries. HA-BSI were frequently caused by Gram-negative, carbapenem-resistant, and difficult-to-treat pathogens. Antimicrobial resistance led to delays in adequate antimicrobial therapy, resulting in high mortality rates.
