Summary: The objective of this population-based cohort study was to identify 5-year trajectories of prescription opioid use and examine the characteristics of each trajectory group. The results suggest that most individuals initiating treatment with prescription opioids had relatively low and time-limited exposure over a 5-year period. A small proportion of individuals had sustained or increasing use, and they were typically older with more comorbidities and higher use of psychotropic and other analgesic drugs.

Summary: Although multimodal management is recommended for chronic noncancer pain (CNCP), little is known about the long-term treatment utilization patterns among individuals using opioids. This study conducted in Australia found that many Australians using opioids for CNCP also used nonopioid pharmacological and nonpharmacological treatments. However, some treatment use was inconsistent with guidelines and the use of nonpharmacological treatments was associated with having private health insurance.

Summary: This study aimed to compare the effect of intravenous infusion of lidocaine with ultrasound-guided transverse abdominal plane (TAP) block on postoperative recovery and analgesia in patients undergoing bariatric surgery. The results showed that both lidocaine infusion and TAP block provided good postoperative recovery and analgesia, but lidocaine infusion had better analgesic effect at 12 hours and 24 hours postoperatively compared with TAP block.

Summary: This study evaluates the outcomes of maximal opioid reduction on top of an existing Enhanced Recovery after Surgery (ERAS) pathway in a bariatric surgery program. The results indicate that implementing an opioid-sparing protocol not only reduces opioid use in surgical patients, but also improves satisfaction with pain control.

It provides a conceptual framework to guide future research in optimizing analgesic outcomes in older adults while minimizing deleterious effects on cognition.

Summary: This study found no benefit for postoperative pain or opioid requirements in patients undergoing robotic-assisted Roux-en-Y gastric bypass (RA-RYGB) compared to laparoscopic Roux-en-Y gastric bypass (L-RYGB), and the RA-RYGB group was more likely to report pain at the two-week follow-up.

Summary: Effective interventions can improve analgesic use and appropriateness in long-term care facilities. Interventions involving prescribers, pharmacists, and nurses, with an educational component, are most effective. Combining education and decision support also shows promise. Medication review interventions can lead to changes in medication prescription.

Summary: Considerable individual differences are observed in the sensitivity to opioid analgesics. A study found that carriers of the G allele of the rs12496846 SNP in the C3orf20 gene region were significantly associated with higher postoperative analgesic requirements, and this SNP was also associated with the mRNA expression level of the C3orf20 gene.

Summary: The aim of this study was to identify potential predictive factors associated with increased or decreased opioid requirements after free tissue transfer (FTT) to the head and neck. The study found that preoperative opioid use was associated with higher postoperative opioid requirements. Multimodal pain management (MMPM) was not associated with a decreased opioid requirement.

Summary: This study aimed to evaluate the association between the radiological score and the failure of conservative management in adhesive small bowel obstructions (aSBO). The results showed that a history of previous surgery was not a significant risk factor for conservative treatment failure, while a previous episode of aSBO was protective against treatment failure. Additionally, an Angers CT score of >= 5 was identified as an individual risk factor.

Summary: This study aimed to evaluate the incidence and risk factors for permanent stomas after sphincter-preserving surgery for rectal cancer, as well as compare long-term survival outcomes. Out of 257 patients, 16.7% developed permanent stomas after a median follow-up of 4.8 years, with severe post-operative complications and age being significant risk factors. Patients with permanent stomas had significantly lower overall survival and disease-free survival compared to those without stomas.

Summary: The study aimed to analyze the impact of new histoprognostic factors on the survival of colon cancer patients and optimize their therapeutic management. The presence of isolated tumor deposits and an infiltrative type of tumor invasion significantly worsened overall survival and recurrence-free survival. High-grade budding was associated with a poor prognosis, but with no significant difference. In conclusion, these histoprognostic factors should be integrated into pathological reports of colon cancers to guide more aggressive treatments for patients.

Summary: In a tertiary referral center, a study was conducted to identify preoperative risk factors for failure to discharge after bariatric surgery on or before postoperative day two. The study found that a history of psychiatric disorder, insulin-dependent diabetes, use of anticoagulant medication, distance to the referral center greater than 100 km, gallbladder lithiasis, and planned additional procedures were independent risk factors for delayed discharge.

Summary: Gastrointestinal bleeding of undetermined origin (GBUO) refers to gastrointestinal bleeding without an identified cause or location despite endoscopic assessment. The majority of cases of GBUO occur in the small intestine due to vascular, inflammatory, or tumoral causes. Video-capsule endoscopy (VCE) is recommended as the first-intention study in Europe, with CT-enteroscopy performed if intestinal obstruction is suspected. Enteroscopy is used for therapeutic or diagnostic purposes depending on the results of VCE or CT-enteroclysis, and intraoperative enteroscopy (IOE) is reserved for cases where preoperative enteroscopy fails or for recurrent GBUO after other studies and explorations.

Summary: In France, there are social inequalities in health for patients with colorectal cancer (CRC). Underprivileged patients have a lower incidence of CRC but higher mortality rates. The reasons behind these social inequalities vary depending on the healthcare system. Factors such as reduced participation in screening campaigns and delayed diagnosis contribute to the disparities in survival rates. Disadvantaged patients also face challenges during treatment, including a higher risk of open surgery, severe complications, and delayed chemotherapy.

Summary: This study reports on the quality of life of patients with sigmoid diverticulitis. The results show that long-term quality of life does not differ significantly between patients who received antibiotic therapy and those who received symptomatic treatment alone. However, for patients who experienced recurrent events, elective surgery appears to improve their quality of life. In conclusion, assessment of quality of life is important in diverticular disease and should guide operative indications.

Summary: This study evaluated the tolerability and clinical, biological and radiological tumor response and survival rates in patients with unresectable neuroendocrine liver metastases treated by trans-catheter arterial embolization with conventional chemoembolization, using streptozocin, Lipiodol, and embolization microspheres. The results showed that this procedure using streptozocin is an effective and well-tolerated palliative option for patients with unresectable neuroendocrine liver metastases, with a high objective response rate and non-progressive disease rate. The overall survival rate at different time points was also encouraging.
