OBJECTIVE. The purpose of this article is to review the natural history and management of bladder cancer, with insight into MRI applications for the assessment of muscle invasiveness of bladder cancer using the newly developed Vesical Imaging Reporting and Data System (VI-RADS) score. CONCLUSION. Multiparametric MRI and the VI-RADS score have been consistently validated across several different institutions as appropriate tools for local staging of bladder cancer and have been proven to contribute to the diagnostic workup and management of urinary bladder cancer.

Summary: This study compared clinical outcomes of transfemoral access (TFA), transthoracic access (TTA), and nonthoracic transalternative access (TAA) in TAVR patients with severe peripheral artery disease (PAD). The study found that both TFA and TAA were associated with lower 30-day and 1-year rates of major adverse events (MAE) compared with TTA. However, TAA had a higher risk of stroke/TIA compared with TFA at 1 year.

Summary: Comparing different diagnostic strategies for initial assessment of suspected stable CAD, CCTA shows similar health effects to direct ICA referral and better health outcomes compared to exercise ECG and SPECT-MPI. However, it is associated with more index revascularization compared to exercise ECG.

Quantitative flow ratio modulated by intracoronary optical coherence tomography for predicting physiological efficacy of percutaneous coronary intervention

Daixin Ding, Shengxian Tu, Yingguang Li, Chunming Li, Wei Yu, Xun Liu, Antonio Maria Leone, Cristina Aurigemma, Enrico Romagnoli, Rocco Vergallo, Carlo Trani, William Wijns, Francesco Burzotta

Summary: This study evaluates the potential of a novel method for fast computation of Murray law-based quantitative flow ratio (mu QFR) from coregistered optical coherence tomography (OCT) and angiography (OCT-modulated mu QFR, OCT-mu QFR) in predicting the physiological efficacy of percutaneous coronary intervention (PCI). The results show that simulated residual OCT-mu QFR has good correlation, agreement, and diagnostic concordance with actual post-PCI OCT-mu QFR. In OCT-guided procedures, the pressure drop of OCT-mu QFR in the stent is low and can be predicted by pre-PCI vessel/plaque characteristics.

Summary: The aim of this study was to evaluate the use of left atrial strain values as compared with traditional echocardiographic indexes in improving the non-invasive estimation of left ventricular and diastolic pressure in the acute phase of Takotsubo syndrome (TTS). The study also aimed to predict in-hospital adverse outcomes in TTS patients. Results showed that left atrial reservoir and pump strain values had a stronger correlation with LVEDP compared to other echocardiographic indexes. Furthermore, left atrial reservoir strain was an independent predictor of worse in-hospital outcomes.

Summary: There is no consensus on the optimal stent implantation parameters that enhance clinical outcome during percutaneous coronary interventions (PCIs) using optical coherence tomography (OCT) guidance. This study retrospectively examined the predictive accuracy of different suboptimal stent implantation definitions for long-term risk of device-oriented cardiovascular events (DoCE). The results showed that the definitions proposed by CLI-OPCI II and ILUMIEN-IV OPTIMAL PCI studies were predictive of DoCE, while the FORZA trial definition was not.

Chest CT features of COVID-19 in vaccinated versus unvaccinated patients: use of CT severity score and outcome analysis

Summary: The impact of vaccination on SARS-CoV-2 infection and COVID-19 pneumonia was evaluated by assessing lung disease severity using the CT severity score. Vaccinated patients had significantly lower CT scores compared to unvaccinated patients, and unvaccinated patients had worse clinical outcomes. Vaccination status had the strongest association with the severity of COVID-19, more than age, gender, and number of doses administered.

Summary: This study aimed to compare the diagnostic performance of Whole Body MRI and 18F-FDG-PET/CT in lymphoma staging and assess the relationship between metabolic parameters from 18F-FDG-PET/CT and ADC values. The results showed a high agreement between Whole Body MRI and 18F-FDG-PET/CT in identifying nodal and extra-nodal lesions. Furthermore, a strong negative correlation was found between ADCmean and SUVmean in nodal lesions.

Summary: This study aims to introduce DWI and quantitative ADC evaluation in the O-RADS MRI system and observe how it impacts diagnostic performance. It also assesses the validity and reproducibility among readers with different experience in female pelvic imaging and evaluates the correlation between ADC value and histotype in malignant lesions. The findings demonstrate the prognostic potential of DWI and ADC values for better radiological standardisation and characterisation of AMs in the O-RADS MRI classification.

Summary: The diagnostic work-up and risk stratification in bladder cancer patients are important for optimizing management and improving outcomes. MRI has been suggested as a non-invasive technique for staging and assessing response to therapy. The Vesical Imaging-Reporting And Data System (VI-RADS) has been developed to standardize MRI image acquisition and interpretation for accurate prediction of muscle-wall invasion. However, more high-quality evidence is needed before MRI can be recommended as a first-line test for bladder cancer.
