The Role of Positron Emission Tomography in Clinical Management of Intraductal Papillary Mucinous Neoplasms of the Pancreas

Intraductal papillary mucinous neoplasms (IPMNs) of the pancreas represent a heterogeneous group of tumors, increasingly diagnosed in clinical practice. An early differential diagnosis between malignant and benign lesions is crucial to patient management and the choice of surgery or observation. The therapeutic approach is currently based on a patient's clinical, biochemical, and morphological characteristics. The latest published International Consensus Guidelines (ICG) make no mention of the role of metabolic assessments of IPMNs. The aim of this study was to review the current literature, examining the role of 18-fluorodeoxyglucose (FDG) positron emission tomography (PET) in IPMN management. An extensive literature review was conducted according to the 2009 Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, and 10 articles were analyzed in detail, focusing on the value of PET as opposed to other standard imaging criteria. Data were retrieved on 419 patients. The 18-FDG-PET proved more sensitive, specific, and accurate than the ICG criteria in detecting malignant IPMNs (reaching 80%, 95%, and 87% vs. 67%, 58%, and 63%, respectively). Metabolic assessments may be used as an additional tool for the appropriate management of patients with doubtful imaging findings.

Summary: This study reports a rare case of duodenal lesion in a 41-year-old female patient, which was evaluated for malignant potential and treatment planning using clinical staging and (18)FDG PET/MRI. Robotic transduodenal excision was performed based on the findings. The study suggests that accurate staging with (18)FDG PET/MRI can be very useful in the management of duodenal lesions with uncertain malignant potential.

Impressive response to dabrafenib and trametinib plus silybin in a heavily pretreated IDH wild-type glioblastoma patient with BRAFV600E-mutant and SOX2 amplification

Summary: This study reports a case of a 42-year-old patient with BRAF(V600E) mutant and SOX2 amplification glioblastoma who showed a radiologic and metabolic complete response to personalized treatment with dabrafenib, trametinib, and silybin. Personalized treatment with BRAF and MEK inhibitors showed promising activity in patients with BRAF(V600E) mutant glioblastoma, and silybin may play an important role in reducing drug resistance, especially in patients with SOX2 amplification.

Summary: 18F-FDG PET/MRI may be the preferred diagnostic method for Hodgkin lymphoma patients due to its combination of metabolic information from PET and excellent soft-tissue contrast from MRI, while avoiding radiation exposure from CT. However, longer examination times are a major issue, especially for children requiring anesthesia. Therefore, a targeted selection of suitable whole-body MRI sequences is important to optimize the PET/MRI workflow.

Summary: Pancreatic metastases from other neoplasms are rare, and the role of surgery for this condition is unclear. This study investigated the role of resection in patients with pancreatic secondary lesions. Radical resection of metastases to the pancreas is feasible and safe, and may provide a survival benefit for selected patients, especially those with metastases from renal cell carcinoma.

[18F]FDG PET-MR in the Evaluation and Follow-Up of Incidental Bone Ischemic Lesions in a Mono-Center Cohort of Pediatric Patients Affected by Hodgkin's Lymphoma

Summary: Hodgkin's lymphoma (HL) has a good prognosis in children, adolescents, and young adults, but the development of adverse effects such as Bone Ischemic Lesions (BILs) is a concern. This study evaluated the role of [F-18]FDG-PET-MR in detecting BILs in pediatric HL patients. BILs were detected through PET-MR imaging as medullary lesions with a tortuous border. Among 53 HL patients, 18.9% developed BILs, with mostly multifocal lesions. PET-MR is a sensitive technique for detecting asymptomatic BILs, which can be monitored through clinical follow-up.

Nadir creatinine as a predictor of renal outcomes in PUVs: A systematic review and meta-analysis

Summary: This study conducted a systematic review with meta-analysis to analyze the predictive value of nadir creatinine on long-term renal function in infants with posterior urethral valves (PUVs). The results showed that nadir creatinine is the best prognostic factor for long-term renal function in PUV patients, and a value above the cutoff of 1 mg/dL should be considered a significant predictor for the risk of chronic kidney disease (CKD) and end-stage kidney disease (ESKD).
