Emergence of Cronobacter sakazakii in Cases of Neonatal Sepsis in Upper Egypt: First Report in North Africa

Background and Aim Cronobacter sakazakii (C. sakazakii) has attracted considerable attention as an emerging neonatal pathogen and has been associated with outbreaks of life-threatening septicemia, necrotizing enterocolitis, and meningitis in neonates and infants globally. No data about the role of C. sakazakii as a cause of neonatal sepsis in North Africa is availale. Herein, we aimed to study the incidence of C. sakazakii in cases of neonatal sepsis, its distribution in different food samples in Egypt, antimicrobial profile, and the ability of the strains to form biofilms. Methods A total of 100 positive blood cultures from cases of neonatal sepsis admitted to the neonatal ICU at Assiut University Children's Hospital, Egypt, were analyzed. In addition, 1,100 food samples, including 400 powdered infant formula (PIF), 500 herbs, and 200 water samples were screened for the presence of C. sakazakii. We evaluated the antimicrobial profile and the ability of the strains to form biofilms. Results Cronobacter sakazakii was detected in 12 out of 100 cases of neonatal sepsis. The organism was also isolated from PIF, herbs, and water in percentages of 17.5, 9.2, and 7.5%, respectively. Regarding the antimicrobial sensitivity, all strains were resistant to ampicillin, amoxicillin, ampicillin/sulbactam, clindamycin, cephalothin, and cephalexin. In addition, C. sakazakii strains showed the ability to form biofilms, but with variable degrees of cell density. Conclusion We reported, for the first time, cases of neonatal sepsis caused by the emerging life-threatening pathogen C. sakazakii in Egypt. The organism was also detected in contaminated PIF, herbs, and water, which may be sources of infection for neonates, especially in countries where natural herbs are widely used as an alternative medicine. Finally, collective efforts by the parents, manufacturers of PIF, and healthcare personnel are essential to prevent serious infections caused by C. sakazakii, particularly in infants.

Summary: Heparanase (HPSE) is an enzyme that cleaves heparan sulfate (HS), contributing to extracellular matrix degradation and remodeling. HPSE-induced cleavage of HS promotes autophagy and release of growth factors. The interaction between HPSE and HS triggers the release of chemokines and cytokines, leading to inflammatory response, cytokine storm, and coagulopathy. Targeting HPSE with specific inhibitors may reduce complications in SARS-CoV-2 and MPXV infections.

The study also identified immune expression patterns in affected lymph nodes, indicating the involvement of the NF-kappa B/p65 signaling pathway in the inflammatory response to control the pathogen.

Summary: The wide spread of antibiotic resistance poses a serious global hazard, leading to millions of deaths. Biofilms created by microorganisms further enhance bacterial resistance and the development of multidrug resistant infections. The lack of novel antimicrobial compounds has led to international efforts in developing new antimicrobial agents and innovative techniques for antibiotic administration. Nanotechnology has shown potential in combating multidrug resistance and biofilm infections by utilizing nanomaterial-based therapeutics and their ability to target biofilms and resistant pathogens.

Risks and Preventions for Pregnant Women and Their Preterm Infants in a World with COVID-19: A Narrative Review

Summary: The study aimed to summarize the existing literature on the effects of COVID-19 on pregnant women, preterm babies, and the incidence of preterm birth (PTB). Contradictory results were found regarding the prevalence of PTB during the pandemic compared to previous years, with some studies showing an increase and others showing a decline. COVID-19 infection during pregnancy can lead to complications such as cesarean section, stillbirth, ICU admission, preeclampsia/eclampsia, and higher mortality rates. Vaccination for COVID-19 in pregnant and lactating women was found to stimulate immune responses without notable negative reactions or outcomes for the mother or baby.

First Report on Abnormal Renal Function in Acute Hepatitis E Genotype 1 Infection

Summary: Impaired renal functions have been reported in patients with Hepatitis E virus (HEV) infections, particularly with genotypes 3 and 4. This study evaluated kidney function parameters in patients with HEV genotype 1 (HEV-1) during the acute phase of infection. Out of 31 HEV-1 patients, 16% had abnormal kidney function tests (KFTs), with some patients showing decreased glomerular filtration rates. These abnormalities resolved during convalescence. Monitoring kidney function and potential renal complications is important during HEV-1 infections.

The Role of Cluster of Differentiation 39 (CD39) and Purinergic Signaling Pathway in Viral Infections

Summary: CD39 is a marker of immune cells that hydrolyzes ATP into adenosine, exerting an immunosuppressive effect. CD39 plays a regulatory role in various immunologic cells through purinergic signaling pathways. CD39+ T cells are implicated in viral infections and their expression is an indicator of lymphocyte exhaustion. CD39 levels in T lymphocytes can predict disease progression and immune response to antiviral drugs, while the percentage of CD39 and CD73 in B lymphocytes and monocytes affects the status of viral infections.

Nanoparticles have emerged as a promising treatment for diseases caused by this newly discovered pathogen, and are currently being researched as an effective solution against C. auris.
