Carbohydrate antigen 19-9 elevation without evidence of malignant or pancreatobiliary diseases

Although carbohydrate antigen 19-9 (CA 19-9) may be elevated in benign diseases, elevated CA 19-9 may cause a fear of cancer and unnecessary follow-up studies. Research on how to approach systematically in this case is very limited. The purpose of this study was to analyze the clinical features and the causes of CA 19-9 elevation without evidence of malignant or pancreatobiliary diseases. We retrospectively reviewed the medical records of patients who had CA 19-9 elevation (>= 80 U/mL) and were found to be unrelated to cancer after follow-up. After exclusion, 192 patients were included in this study. The median level of CA 19-9 was 136.5 U/mL. The causes of CA 19-9 elevation were determined in 147 (76.6%) patients, and that was unknown in 45 (23.4%). The estimated causative diseases were hepatic diseases in 63 patients, pulmonary diseases in 32, gynecologic diseases in 38, endocrine diseases in 13, and spleen disease in 1. Of 45 patients with unknown cause, 35 had normalization of CA 19-9 and 10 had persistently elevated CA 19-9. In conclusion, CA 19-9 elevation without malignancies or pancreatobiliary diseases should be systematically evaluated and followed up. We suggest an algorithm to investigate the causes and follow up these patients.

Prediction of urine culture results by automated urinalysis with digital flow morphology analysis

De Novo Cancer Incidence after Kidney Transplantation in South Korea from 2002 to 2017

Summary: Improved patient care and immunosuppressive drugs have increased kidney transplantation (KT) rates, but also led to a higher incidence of cancer in KT recipients. Prostate and thyroid cancers were found to be the most common types, with Kaposi's sarcoma showing the highest standardized incidence ratio. Factors such as age over 39 and the use of prednisolone were associated with an increased risk of cancer development in KT patients under immunosuppressive conditions.

Experience and perspectives of end-of-life care discussion and physician orders for life-sustaining treatment of Korea (POLST-K): a cross-sectional study

Summary: This study aimed to explore healthcare providers' experience and perspectives on end-of-life care decisions, including end-of-life discussions and physician's order of life-sustaining treatment documentation, in Korea. The study utilized a cross-sectional survey with participation from physicians and nurses. The results highlighted that physicians had a good understanding of terminal illness and physician's order of life-sustaining treatment, although there were uncertainties in terminal state diagnosis and disease trajectory. Factors related to healthcare providers' relationships and communications were identified as major obstacles to end-of-life discussions. It was suggested that simplification of the process and additional staff were needed to facilitate these discussions and documentation.

Calf Circumference as an Indicator for Cystatin C Testing in Hospitalized Elderly Male Patients for Detecting Hidden Renal Impairment

Summary: Serum creatinine levels can overestimate renal function in elderly patients with sarcopenia. This study examined calf circumference (CC) as a potential indicator to test for cystatin C instead of creatinine in elderly inpatients. The results showed a positive correlation between CC and the eGFRcys/eGFRcr ratio in both sexes, suggesting that CC could be a cost-effective screening tool for detecting hidden renal impairment in elderly male patients using cystatin C as an add-on test.

Associations of Perioperative Red Blood Cell Transfusion With Outcomes of Kidney Transplantation in Korea Over a 16-Year Period

Summary: This study investigated the associations between transfusion of different types of red blood cell preparations and kidney allograft outcomes after kidney transplantation. The results showed that regardless of the type of red blood cell products, patients receiving transfusion had a significantly higher proportion of acute rejection and graft failure. Particularly, transfusion of packed red blood cells (pRBCs) significantly increased the risk of transplant rejection. Therefore, careful consideration of transfusion indications and selection of the appropriate type of red blood cells is necessary for high-risk patients.

Impact of urinary tract infection-causative microorganisms on the progression to bloodstream infection: A propensity score-matched analysis

Summary: This study aimed to determine the risk factors for the progression of UTI to BSI and evaluate mortality-associated factors in patients with UT-BSI. The study found that UTI caused by Candida species was significantly associated with high 30-day mortality. Furthermore, considering the increasing age of patients and more frequent use of indwelling urologic devices, UT-BSIs caused by microorganisms other than Escherichia coli could become a more serious medical burden in the future.

Perioperative blood usage and therapeutic plasma exchange in kidney transplantation during a 16-year period in South Korea
