Summary: Cholangiocarcinoma (CCC) is a rare cancer with poor prognosis when diagnosed in advanced stages. Dickkopf-related protein 1 (DKK-1) has been shown to have diagnostic and prognostic value in intrahepatic CCC (ICC) patients, and its combination with carbohydrate antigen 19-9 (CA 19-9) can improve diagnostic performance. High levels of DKK-1 and CA 19-9 are associated with shorter overall survival in CCC patients, indicating their potential as prognostic markers.

Summary: Zn-Co-S@G nanocomposites were synthesized through a one-step hydrothermal method and showed promising performance in constructing an electrochemical immunosensor for CA19-9 detection. The strong interaction between Zn-Co-S and graphene facilitated the exposure of numerous electroactive sites, resulting in improved amplified electrochemical signals and a good linear relationship for detecting CA19-9.

Summary: This study found that in patients with LARC treated with nCRT, the number of elevated post-nCRT tumor markers is an independent prognostic factor. For patients with elevated pre-nCRT tumor markers, normalization of post-nCRT markers is a protective factor. Furthermore, persistent elevation of tumor markers post-surgery is associated with increased risk of recurrence and death, while normalization of these markers is linked to better survival outcomes.

Summary: This study compared the normalization of serum carbohydrate 19-9 (CA19-9) levels before and after surgery in patients with pancreatic adenocarcinoma (PDAC) who underwent neoadjuvant therapy (NT) versus surgery-first (SF). The results showed that normal pre/postoperative CA19-9 was associated with better recurrence-free survival (RFS) and overall survival (OS). Nearly 30% of SF patients failed to achieve postoperative CA19-9 normalization. NT should be considered for patients with elevated CA19-9 levels, and ideal SF patients may have normal CA19-9 levels at presentation.

Summary: This study aimed to investigate the role of carbohydrate antigen 19-9 (CA 19-9) in evaluating response among patients with intrahepatic cholangiocarcinoma (iCCA). The authors found that CA 19-9 response was associated with radiologic response and improved outcomes in iCCA patients. Achieving a CA 19-9 response should be a therapeutic objective for patients with iCCA after systemic therapies.

Summary: This paper reports a rare case of Mirizzi syndrome with a very high CA19-9 marker in an elderly patient without malignancy. The case highlights the challenges in distinguishing Mirizzi syndrome from cholangiocarcinoma, emphasizing the importance of maintaining a high index of suspicion to rule out malignancy and develop an appropriate management plan, especially in elderly patients.

Summary: This study aimed to determine the associations between CA19-9/GGT levels and postoperative neoplasm recurrence and long-term outcome of DCC patients. The high CA19-9/GGT ratio group showed higher CA19-9 levels and lymphatic metastasis rate compared to the low-ratio group, indicating a correlation with poor clinical outcomes. Multivariate analysis identified CA19-9/GGT, lymphatic metastasis, and tumor differentiation as independent risk factors for tumor recurrence and long-term prognosis of DCC.

Summary: This study aimed to evaluate the relationship between perioperative serum CEA/carbohydrate antigen 19-9 levels and prognosis in locally recurrent rectal cancer. The results showed that preoperative serum carbohydrate antigen 19-9 level was correlated with poorer prognoses, and postoperative serum CEA >= 5 ng/mL was significantly correlated with a worse prognosis. At the time of diagnosis of re-recurrence, there was a high rate of serum CEA elevation.

Summary: Improved patient care and immunosuppressive drugs have increased kidney transplantation (KT) rates, but also led to a higher incidence of cancer in KT recipients. Prostate and thyroid cancers were found to be the most common types, with Kaposi's sarcoma showing the highest standardized incidence ratio. Factors such as age over 39 and the use of prednisolone were associated with an increased risk of cancer development in KT patients under immunosuppressive conditions.

Summary: This study aimed to explore healthcare providers' experience and perspectives on end-of-life care decisions, including end-of-life discussions and physician's order of life-sustaining treatment documentation, in Korea. The study utilized a cross-sectional survey with participation from physicians and nurses. The results highlighted that physicians had a good understanding of terminal illness and physician's order of life-sustaining treatment, although there were uncertainties in terminal state diagnosis and disease trajectory. Factors related to healthcare providers' relationships and communications were identified as major obstacles to end-of-life discussions. It was suggested that simplification of the process and additional staff were needed to facilitate these discussions and documentation.

Summary: Serum creatinine levels can overestimate renal function in elderly patients with sarcopenia. This study examined calf circumference (CC) as a potential indicator to test for cystatin C instead of creatinine in elderly inpatients. The results showed a positive correlation between CC and the eGFRcys/eGFRcr ratio in both sexes, suggesting that CC could be a cost-effective screening tool for detecting hidden renal impairment in elderly male patients using cystatin C as an add-on test.

Summary: This study investigated the associations between transfusion of different types of red blood cell preparations and kidney allograft outcomes after kidney transplantation. The results showed that regardless of the type of red blood cell products, patients receiving transfusion had a significantly higher proportion of acute rejection and graft failure. Particularly, transfusion of packed red blood cells (pRBCs) significantly increased the risk of transplant rejection. Therefore, careful consideration of transfusion indications and selection of the appropriate type of red blood cells is necessary for high-risk patients.

The study found that UTI caused by Candida species was significantly associated with high 30-day mortality. Furthermore, considering the increasing age of patients and more frequent use of indwelling urologic devices, UT-BSIs caused by microorganisms other than Escherichia coli could become a more serious medical burden in the future.
