Emerging biomarkers in HCC patients: Current status

Hepatocellular carcinoma (HCC) is the liver's most common primary malignancy, with over half a million new cases diagnosed each year and being the fourth leading cause of cancer death, worldwide. The poor prognosis of HCC is largely related to late diagnosis. Historically, serum alpha-fetoprotein and diagnostic imaging have been primary diagnostic modalities. However, the poor prognosis due to late diagnosis of HCC has proven unacceptable and, recently, significant efforts have been devoted to identifying patients with early stage HCC. Molecular biomarkers can provide additional and relevant information about the biological behavior of these tumors. Research in biomarker combinations may provide more accurate and valuable information for the future individualized HCC diagnosis and/or prognosis. Several biomarkers with prognostic significance have been identified, however all of them have been studied retrospectively. Furthermore, of all different molecular signatures that have been published, very few have been externally validated. The aim of this review is to analyze the most relevant emerging biomarkers of HCC.

Summary: This study compared the performance of artificial intelligence-enabled quantitative coronary computed tomography (AI-QCT) angiography analyses to conventional imaging and invasive tests for the detection and grading of coronary stenoses. The results demonstrated that AI-QCT analysis enables rapid and accurate identification and exclusion of high-grade stenosis, with a close agreement to quantitative coronary angiography.

Summary: By using a modified Delphi method, renowned international expert liver surgeons reached global consensus on the definition of TOLS in minimally invasive and open liver resection. This study provides the first international expert consensus-based definition of TOLS for assessing postoperative outcomes in liver surgery. The TOLS definition includes absence of intraoperative grade = 2 incidents, postoperative bile leakage grade B/C, postoperative liver failure grade B/C, 90-day major postoperative complications, 90-day readmission due to surgery-related major complications, 90-day/in-hospital mortality, and presence of R0 resection margin. This definition can be used to evaluate hospital performance and compare patient outcomes internationally.

Summary: This study identified three distinct subtypes of intrahepatic cholangiocarcinoma (ICC) based on tumor burden score (TBS), preoperative neutrophil-to-lymphocyte ratio (NLR), and CA19-9 levels. These subtypes were associated with significant differences in recurrence-free survival (RFS) and overall survival (OS). Despite having more favorable tumor burden and lymphocyte ratio, the inflammatory subtype of ICC had a higher incidence of recurrence.

Summary: This study investigated the postoperative oncologic outcomes in patients with early and intermediate hepatocellular carcinoma (HCC) and found that patients with intermediate HCC were less likely to achieve textbook oncologic outcomes. Factors such as tumor burden, aspartate transaminase-platelet ratio index, Charlson Comorbidity Index, major liver resection, and tumor stage were identified as influencing factors for textbook oncologic outcomes.

Summary: This study aimed to investigate the prognostic significance of margin status following hepatectomy of intrahepatic cholangiocarcinoma (ICC). The results showed that increasing margin width was associated with improved survival in patients with low or medium tumor burden, while margin status did not impact prognosis in patients with high tumor burden.

Summary: This study examined the prognostic role of tumor burden score (TBS) relative to albumin-bilirubin (ALBI) grade among patients undergoing curative-intent resection of intrahepatic cholangiocarcinoma (ICC). The results showed that decreased tumor burden was associated with better long-term outcomes. Patients with high TBS and high ALBI grade had worse overall survival and higher risk of early recurrence and death.

Summary: This study aimed to develop a holistic risk score incorporating preoperative tumor, liver, nutritional, and inflammatory markers to predict overall survival after hepatectomy for hepatocellular carcinoma. Patients who underwent surgery for hepatocellular carcinoma between 2000 and 2020 were included. A prognostic risk score model (PreopScore) was developed and validated using cross-validation.

Pancreas Rejection in the Artificial Intelligence Era: New Tool for Signal Patients at Risk

Summary: This study aimed to identify features that could indicate the risk of pancreas allograft rejection. 74 features were collected from 79 patients who underwent simultaneous pancreas-kidney transplantation, and predictive models were built using the Naive Bayesian Classifier and Support Vector Machine. The results showed that the classifiers had good predictive performance, with AUROC and classification accuracy values ranging from 0.96 to 0.97 and 0.87 to 0.94, respectively. These findings suggest that successful classifiers can be developed for graft rejection prediction, supporting clinical decision making.

Summary: This study evaluated the impact of the donor risk index (DRI), recipient, and perioperative factors on post liver transplant outcomes. The DRI was not found to be associated with 1-year graft failure, but higher SOFA score or lactate upon ICU admission post LT were independently associated with higher odds of 1-year graft failure.
