Hepatocellular carcinoma (HCC) is the liver's most common primary malignancy, with over half a million new cases diagnosed each year and being the fourth leading cause of cancer death, worldwide. The poor prognosis of HCC is largely related to late diagnosis. Historically, serum alpha-fetoprotein and diagnostic imaging have been primary diagnostic modalities. However, the poor prognosis due to late diagnosis of HCC has proven unacceptable and, recently, significant efforts have been devoted to identifying patients with early stage HCC. Molecular biomarkers can provide additional and relevant information about the biological behavior of these tumors. Research in biomarker combinations may provide more accurate and valuable information for the future individualized HCC diagnosis and/or prognosis. Several biomarkers with prognostic significance have been identified, however all of them have been studied retrospectively. Furthermore, of all different molecular signatures that have been published, very few have been externally validated. The aim of this review is to analyze the most relevant emerging biomarkers of HCC.

Ultra Deep Sequencing of Circulating Cell-Free DNA as a Potential Tool for Hepatocellular Carcinoma Management

Summary: This study investigated the value of detecting the most prevalent HCC mutations in plasma cfDNA as prognostic noninvasive biomarkers. The results showed that the total amount of cfDNA, number of mutated genes, and number of detectable mutations in cfDNA were significantly associated with mortality. Additionally, the number of mutated genes, detected mutations, and ratio between number of mutations and cfDNA amount were significantly related to recurrence. Detection of more than four mutations in cfDNA correlated with a higher risk of death. The study suggests that total cfDNA and detection of prevalent HCC mutations could have prognostic implications in early-stage HCC patients.

DNA methylation markers in the detection of hepatocellular carcinoma

Summary: Hepatocellular carcinoma (HCC) is a common primary liver malignancy with poor prognosis. Epigenetic modification, specifically DNA methylation, plays a significant role in HCC development by impacting cell differentiation, proliferation, and function. Methylated DNA markers have shown promise as specific biomarkers for early-stage HCC detection, offering higher sensitivity and specificity compared to current surveillance methods. However, the immediate application of these biomarkers is limited by various factors. This review provides a detailed rationale for using methylation biomarkers in HCC detection and summarizes recent studies on methylated DNA markers, emphasizing the importance of the underlying cause of liver disease in cancer mechanisms.

Machine learning-based integration develops a neutrophil-derived signature for improving outcomes in hepatocellular carcinoma

Summary: This study stratified HCC patients into subtypes based on neutrophil-related molecular characteristics and identified significant differences in prognosis, clinical pathological characteristics, inflammation-related pathways, levels of immune infiltration, and expression levels of immune genes among the subtypes. Furthermore, a risk model called NDS was constructed using machine learning, consisting of 10 essential genes, which showed superior accuracy and predictive value for HCC patient prognosis.

A prognostic model for hepatocellular carcinoma patients based on signature ferroptosis-related genes

Summary: An prognostic model based on ferroptosis-related genes was successfully established using RNA-seq and clinical data of HCC patients. The model incorporates risk score, TNM stage, and cirrhosis to predict the survival of HCC patients. The difference in overall survival between risk groups was found to be closely related to immune-related pathways, suggesting that an immune-suppressive tumor microenvironment may influence the prognosis of HCC patients.

Prognostic MicroRNA Fingerprints Predict Recurrence of Early-Stage Hepatocellular Carcinoma Following Hepatectomy

A panel of 23 microRNAs (HCCseek-23 panel) was consolidated based on their reported functions in HCC development. Quantitative PCR and machine learning random forest models were applied to develop diagnostic and prognostic models.

Summary: Non-alcoholic fatty liver disease (NAFLD) is a common cause of liver disease worldwide, with potential progression to hepatocellular carcinoma (HCC). The prevalence of NAFLD and consequently HCC is expected to increase in the coming years, posing a significant public health problem. Monitoring HCC in NAFLD patients has limitations, and new biomarkers and scores have been proposed for early diagnosis. This review explores the differences in HCC features between NAFLD and non-NAFLD patients, as well as the limitations of tumor surveillance in NAFLD patients and new biomarkers for early diagnosis.

Identification of DNA repair gene signature and potential molecular subtypes in hepatocellular carcinoma

Summary: DNA repair plays a critical role in tumor progression and affects various aspects such as tumor mutational burden, genome stability, PD-L1 expression, immunotherapy response, and tumor-infiltrating lymphocytes. A prognostic model based on DNA damage response genes was developed for hepatocellular carcinoma (HCC), and patients were stratified according to their risk scores. The model accurately predicted survival rates and showed potential as an independent factor for HCC prognosis. Gene set enrichment analysis revealed enriched pathways in high-risk HCC, and potential molecular subtypes were identified based on DDR gene expression. This study provides novel insights into HCC prognosis and molecular mechanisms.

Summary: By using a modified Delphi method, renowned international expert liver surgeons reached global consensus on the definition of TOLS in minimally invasive and open liver resection. This study provides the first international expert consensus-based definition of TOLS for assessing postoperative outcomes in liver surgery. The TOLS definition includes absence of intraoperative grade = 2 incidents, postoperative bile leakage grade B/C, postoperative liver failure grade B/C, 90-day major postoperative complications, 90-day readmission due to surgery-related major complications, 90-day/in-hospital mortality, and presence of R0 resection margin. This definition can be used to evaluate hospital performance and compare patient outcomes internationally.

Summary: This study identified three distinct subtypes of intrahepatic cholangiocarcinoma (ICC) based on tumor burden score (TBS), preoperative neutrophil-to-lymphocyte ratio (NLR), and CA19-9 levels. These subtypes were associated with significant differences in recurrence-free survival (RFS) and overall survival (OS). Despite having more favorable tumor burden and lymphocyte ratio, the inflammatory subtype of ICC had a higher incidence of recurrence.

Summary: This study investigated the postoperative oncologic outcomes in patients with early and intermediate hepatocellular carcinoma (HCC) and found that patients with intermediate HCC were less likely to achieve textbook oncologic outcomes. Factors such as tumor burden, aspartate transaminase-platelet ratio index, Charlson Comorbidity Index, major liver resection, and tumor stage were identified as influencing factors for textbook oncologic outcomes.

Summary: This study examined the prognostic role of tumor burden score (TBS) relative to albumin-bilirubin (ALBI) grade among patients undergoing curative-intent resection of intrahepatic cholangiocarcinoma (ICC). The results showed that decreased tumor burden was associated with better long-term outcomes. Patients with high TBS and high ALBI grade had worse overall survival and higher risk of early recurrence and death.

Patients who underwent surgery for hepatocellular carcinoma between 2000 and 2020 were included. A prognostic risk score model (PreopScore) was developed and validated using cross-validation.

Summary: This study aimed to identify features that could indicate the risk of pancreas allograft rejection. 74 features were collected from 79 patients who underwent simultaneous pancreas-kidney transplantation, and predictive models were built using the Naive Bayesian Classifier and Support Vector Machine. The results showed that the classifiers had good predictive performance, with AUROC and classification accuracy values ranging from 0.96 to 0.97 and 0.87 to 0.94, respectively. These findings suggest that successful classifiers can be developed for graft rejection prediction, supporting clinical decision making.

Summary: This study conducted cluster analysis for ICC subtypes to explore their association with long-term outcomes and recurrence patterns. Three distinct ICC subtypes were identified, with significant differences in overall survival and recurrence-free survival. Despite a more favorable disease profile, the inflammatory ICC subtype had a higher incidence of recurrence.

The Impact of Donor Risk Index, Recipients' and Operative Characteristics on Post Liver Transplant One-Year Graft Failure: A Cohort Analysis

Summary: This study evaluated the impact of the donor risk index (DRI), recipient, and perioperative factors on post liver transplant outcomes. The DRI was not found to be associated with 1-year graft failure, but higher SOFA score or lactate upon ICU admission post LT were independently associated with higher odds of 1-year graft failure.
