A case of COVID-19 and pneumonia returning from Macau in Taiwan: Clinical course and anti-SARS-CoV-2 IgG dynamic

A 46-year-old woman presented to the emergency department with 2-day fever and cough at seven days after returning from Macau. COVID-19 and pneumonia was diagnosed based on the positive real-time RT-PCR tests for oropharyngeal swab samples and the presence of anti-SARS-COV-2 IgG starting from the illness day 11 and post-exposure 18-21 days. Multisystem inflammatory syndrome in adults: Characteristics, treatment, and outcomes

Summary: Following the emergence of multisystem inflammatory syndrome in children (MIS-C), a similar clinical scenario called multisystem inflammatory syndrome in adults (MIS-A) has been observed. Although the prevalence of MIS-A is low and likely underdiagnosed, it can be associated with high mortality. MIS-A is a multisystem disease that affects the cardiovascular, respiratory, gastrointestinal, dermatologic, hematologic, and neurologic systems. The diagnosis of MIS-A requires clinical manifestations, laboratory evidence of inflammation, and SARS-CoV-2 infection. The appropriate treatment for MIS-A is still uncertain, but anti-inflammatory agents like intravenous immunoglobulin and corticosteroids are commonly used. Further research is needed to determine the true prevalence, pathogenesis, and effective treatment for MIS-A.

Coronavirus disease 2019 rebounds following nirmatrelvir/ritonavir treatment

Summary: Nirmatrelvir/ritonavir (NMV-r) is an effective antiviral agent for treating nonhospitalized COVID-19 patients. However, there have been rare cases of virologic and symptomatic rebound after NMV-r treatment, which is a cause for concern. This review provides comprehensive information on the frequency, mechanism, outcomes, and management of COVID-19 rebound after NMV-r. The incidence of rebound was less than 2% and most cases had mild symptoms, with favorable clinical outcomes.

Summary: COVID-19 is an emerging infectious disease caused by SARS-CoV-2, and has become a global pandemic with over 600 million infections and more than 6.6 million deaths as of November 25, 2022. The disease carries a high mortality rate, especially in severe cases. Co-infections and secondary infections with bacteria and fungi further complicate the diagnosis and management of COVID-19. This guideline provides recommendations for the management and treatment of COVID-19 associated bacterial and fungal infections, including CABI, CAPA, CAC, and CAM, based on the current evidence and using the GRADE methodology.

Characteristics of rectal chlamydia among men who have sex with men in southern Taiwan, 2020-2022: An emerging threat of rectal lymphogranuloma venereum L2b

Summary: The prevalence of rectal chlamydia among non-HIV men who have sex with men (MSM) in Taiwan is uncertain, and rectal lymphogranuloma venereum (LGV) has never been reported in the Far East. A study in southern Taiwan from 2020 to 2022 found that 6.2% of MSM had rectal chlamydia infection. Risk factors for rectal chlamydia included co-infection with rectal gonorrhea and multiple sexual partners. The study also identified different genotypes or genovariants of chlamydia, with L2b being the most prevalent in rectal LGV cases.

Psychological Distress and Physical Adverse Events of COVID-19 Vaccination among Healthcare Workers in Taiwan

Summary: The COVID-19 pandemic has caused psychological distress among healthcare workers (HCWs), and this study investigated the association between psychological distress and adverse events (AEs), as well as the determinants of protective behaviors in Taiwanese HCWs. The results showed that depression, distress, and anger were significantly associated with physical AEs, while suicidal thoughts were related to systemic AEs after COVID-19 vaccination. Older age and female gender were associated with more engagement in preventive behaviors, and suicidal thoughts were related to the occurrence of systemic AEs among HCWs. Doctors performed better at preventive behaviors compared to nurses and other HCWs, and HCWs experiencing anxiety and nervousness tended to avoid crowds.

Summary: This study evaluated the immune responses and safety of COVID-19 vaccination in patients with atypical hemolytic uremic syndrome (aHUS) in Taiwan. The results showed that the vaccines effectively protected aHUS patients from severe COVID-19 complications without significant safety concerns. Transplant aHUS patients had lower immune response titers, indicating a need for additional vaccine doses.
