Optimising genomic approaches for identifying vancomycin-resistant Enterococcus faecium transmission in healthcare settings

Summary: The researchers developed a standardized genomic method to identify transmission links of vancomycin-resistant Enterococcus faecium (VREfm). They found that clustering by core genome multilocus sequence type (cgMLST) provided more informative population structure than traditional MLST, and pairwise genome comparisons using split k-mer analysis (SKA) were effective in inferring patient-to-patient transmission. This study provides a standardized genomic framework for VREfm transmission detection, which can be globally deployed for outbreak detection and investigation.

Summary: This study is the largest to date in Australia to investigate the distribution and antibiotic susceptibility of Nocardia species. The susceptibility profiles varied among different species, and first-line therapeutic recommendations did not always reflect in vitro susceptibility. Ongoing susceptibility testing is warranted for targeted clinical use.

Correlation between monkeypox viral load and infectious virus in clinical specimens

Summary: This study aimed to correlate cell culture infectivity of clinical samples with viral loads. The results showed that viral loads in skin lesions were significantly higher than those in throat or nasopharyngeal samples, and viral loads in anal samples were also significantly higher than those in throat or nasopharyngeal samples. The viral culture success rate was 80%, and 50% of the samples were positive in viral culture at Ct 34.1.

Summary: In the past decade, advancements in HIV detection technology and testing regulations have greatly impacted laboratory testing for HIV in Australia. This article provides an update on the current issues concerning the laboratory detection and confirmation of HIV, including the impact of early treatment and prevention strategies, updated national HIV laboratory case definition, and novel strategies for HIV detection. These developments create an opportunity to develop a nationally consistent contemporary HIV testing algorithm for optimization and standardization of HIV testing in Australia.
