Expression of pro-angiogenic factors as potential biomarkers in experimental models of colon cancer

Purpose RAS mutational status in colorectal cancer (CRC) represents a predictive biomarker of response to anti-EGFR therapy, but to date it cannot be considered an appropriate biomarker of response to anti-VEGF therapy. To elucidate the function of K-Ras in promoting angiogenesis, the effect of conditioned media from KRAS mutated and wild type colon cancer cell lines on HUVECs tubule formation ability and the correspondent production of pro-angiogenic factors have been evaluated by a specific ELISA assay. Methods Ras-activated signaling pathways were compared by western blot analysis and RTq-PCR. In addition, VEGF, IL-8, bFGF and HIF-1 alpha expression was determined in K-RAS silenced cells. Furthermore, we conducted an observational study in a cohort of RAS mutated metastatic CRC patients, treated with first-line bevacizumab-based regimens, evaluating VEGF-A and IL-8 plasma levels at baseline, and during treatment. Results K-RAS promotes VEGF production by cancer cell lines. At the transcriptional level, this is reflected to a K-RAS dependent HIF-1 alpha over-expression. Moreover, the HIF-1 alpha, VEGF and FGF expression inhibition in KRAS knocked cells confirmed these results. Within the clinical part, no statistically significant correlation has been found between progression-free survival (PFS) and VEGF-A/IL-8 levels, but we cannot exclude that these biomarkers could be further investigated as predictive or prognostic biomarkers in this setting. Conclusion Our study confirmed the direct involvement of K-Ras in promoting angiogenesis into colon cancer cell lines.

Summary: We developed a drug-based score to predict prognosis and treatment response in advanced cancer patients receiving immune checkpoint inhibitors (ICIs), and validated its value in a population of metastatic renal cell carcinoma patients treated with ipilimumab plus nivolumab. The model considers the use of certain drugs before immunotherapy initiation and demonstrated significant and clinically meaningful predictions for overall survival and progression-free survival. Further development will involve analyzing the gut microbiome and its link to drug exposure and immune sensitivity.

Summary: Different treatment strategies after the progression of hepatocellular carcinoma (HCC) include continuing immune checkpoint inhibitors (ICIs), switching to tyrosine kinase inhibitors (TKIs), and stopping anticancer therapy. This study found that continuing ICI treatment and sequential treatment of ICI and TKI were effective strategies. However, intrahepatic growth and new vascular invasion were associated with poorer prognosis.

The results showed that high levels of TILs were associated with ICI treatment response and could be used for patient selection and precision therapy guidance.

Summary: This study aimed to investigate the impact of cholesterol efflux capacity on the clinical outcomes of patients with advanced non-small cell lung cancer and metastatic renal cell carcinoma treated with immune checkpoint inhibitors. The results showed that blood cholesterol levels and serum transporters were associated with patients' overall survival, progression-free survival, and clinical benefit. Passive diffusion demonstrated positive prognostic value in terms of these clinical outcomes.

Summary: The impact of type 2 diabetes mellitus (T2DM) on the clinical outcome of immune checkpoint inhibitors (ICI) in solid tumor patients is uncertain. Patients on glucose-lowering medications (GLM) for T2DM had worse overall survival (OS) and progression-free survival (PFS) independent of the number of GLM received. Among the GLM, metformin use was associated with increased risk of death and disease progression/death. Further studies are needed to understand the relative impact of metformin and preexisting T2DM diagnosis on outcomes in this population.

PETAL protocol: a phase Ib study of pembrolizumab after transarterial chemoembolization in hepatocellular carcinoma

Summary: Transarterial chemoembolization (TACE) is the preferred treatment for intermediate-stage hepatocellular carcinoma (HCC), and recent data suggest that TACE can enhance the effectiveness of anti-PD-1 immunotherapy. The authors present the protocol for PETAL, a phase Ib study that aims to evaluate the safety and bioactivity of pembrolizumab, an anti-PD-1 antibody, after TACE in HCC patients. The study will enroll up to 26 participants, with a run-in phase evaluating six patients to establish preliminary safety. Pembrolizumab will be administered three times weekly for 1 year or until disease progression, starting 30-45 days after TACE. The primary objective is to assess safety, while efficacy will be preliminarily evaluated through radiological responses every four cycles.

Summary: This study investigated the impact of BMI on the safety and efficacy of Atezo/Bev in unresectable HCC patients. The results showed no significant differences between overweight and non-overweight patients in terms of survival rates, progression-free survival, overall response rate, and disease control rate. Summary: Direct comparisons across first-line regimens for advanced hepatocellular carcinoma are lacking. This network metanalysis compared first-line systemic treatments for hepatocellular carcinoma and found that ICI + anti-vascular endothelial growth factor combinations, as well as double ICIs, showed the greatest overall survival benefit. However, ICI + kinase inhibitor regimens were associated with greater progression-free survival benefit at the cost of higher toxicity rates.
