Pyostomatitis vegetans in a pediatric patient with ulcerative colitis: case report of a rare pediatric inflammatory bowel disease extraintestinal manifestation and review of the literature

Inflammatory bowel diseases can occur with a wide range of extraintestinal symptoms, among which pyostomatitis vegetans, that is a rare but almost pathognomonic finding. We report the case of a 9-year-old female patient affected by ulcerative colitis and recently treated for an oral candidiasis, who experienced a relapse in her ulcerative colitis (PUCAI 50), preceded by the return of whitish lesions in the oral cavity, initially misdiagnosed and unsuccessfully treated as candidiasis and then recognized as pyostomatitis vegetans. Bloody diarrhea was treated with oral beclomethasone, with rapid remission of ulcerative colitis and disappearance of pyostomatitis vegetans. After 2 years, ulcerative colitis is in sustained remission with oral mesalamine and pyostomatitis vegetans has not recurred. Pyostomatitis vegetans is considered a marker of ulcerative colitis relapse among adult population, and although very few pediatric cases are described, it is very important to remember it in the differential diagnosis of the oral manifestations and comorbidities of inflammatory bowel diseases.

Disease Activity Patterns in the First 5 Years After Diagnosis in Children With Ulcerative Colitis: A Population-Based Study

Summary: The study aimed to define activity clusters in children with ulcerative colitis during the first 5 years after diagnosis and identify early prognostic risk factors. Results showed that more than one-third of children with UC had a chronically active or intermittent course in the first 5 years, with some experiencing sustained remission after initial active disease. Additionally, one-third of patients had well-controlled disease throughout the study period after initial treatment.

Summary: This study aimed to evaluate the epidemiological trends of pediatric inflammatory bowel diseases (IBD) in Italy from 2009 to 2018. The results showed that the minimal incidence rate of IBD in Italy seemed to have stabilized over the last two decades, with a slight predominance of ulcerative colitis (UC) over Crohn's disease (CD). The diagnostic delay for CD has significantly decreased, indicating an improved diagnostic capacity.

Summary: Significant differences are observed in the clinical presentation and outcomes of paediatric patients with L2, compared to patients with L1 and UC. L2 patients have lower rates of hematochezia and fever, skip lesions in the intestines, and lower antibody positivity. Response rates to exclusive enteral nutrition and oral steroids are comparable between L1 and L2 patients, but L1 patients are commenced on anti-TNF alpha earlier. Moreover, stricturing phenotype and need for colectomy are very rare in L2 patients.

Summary: This study aimed to evaluate the quality and amount of nutrient intake in pediatric patients with Crohn's Disease (CD). The results showed that the diet of CD patients was deficient in fibers, polyphenols, vitamin A, beta-carotene, and fatty acids, while it was rich in animal proteins, vitamin B12, and niacin. CD dietary habits reflected the so-called Western diet, possibly contributing to the pathogenesis of CD.

Summary: The survey of members of the Italian Society of Pediatric Gastroenterology, Hepatology and Nutrition showed high awareness and utilization of biosimilars in treating inflammatory bowel disease. Participants acknowledged similar efficacy and safety of biosimilars compared to originator drugs, with cost-saving being the main advantage. The majority reported switching from originator to biosimilars in their clinical practice with no significant adverse events, indicating a favorable and confident stance towards biosimilar use in pediatric IBD.

Nutraceuticals in the Modulation of the Intestinal Microbiota: Current Status and Future Directions

Summary: There is increasing interest in the pharmaceutical industry regarding the human intestinal microbiota due to its association with non-communicable diseases. While probiotics and prebiotics have been explored for modulating the gut microbiota, this review highlights other potential interventions such as postbiotics, essential oils, fungus, and officinal plants. Further research, especially clinical studies using fungus, officinal and aromatic plants or their extracts, is necessary to fully understand and harness the potential of these interventions in modulating the intestinal microbiota for improved pharmacological therapy of non-communicable diseases.

Summary: This study aimed to evaluate the efficacy and tolerance of thalidomide in children with very early onset inflammatory bowel disease (VEOIBD) compared with children with pediatric-onset IBD (pIBD). The results showed that thalidomide is an effective and tolerated treatment in children with VEOIBD. However, discontinuation due to lack of efficacy is more frequent, but adverse events are less common than in children with pIBD.
