SARS-CoV-2 spread in Northern Italy: what about the pollution role?

The recent epidemic of the new SARS-CoV-2 in the northern regions of Italy is putting the organization of the Italian health system under serious attack. The current emergency requires all possible efforts to stem the spread of the virus. In this context, it is clear that we have the urgent need to rely upon etiopathogenetic data, in order to do all possible efforts to block the epidemic. However, observing the trend of the infections in China and the geographic areas of the main outbreaks, it could be hypothesized that air pollution plays a role. In particular, it has been previously demonstrated, in specific populations, a role of particulate matter in worsening clinical presentation of virus infection in airways. Without prejudice to the ascertained virus spread by air droplets or contaminated surfaces, the factors that could have favored its spread remain to be investigated. Moreover, if these observations were to be confirmed, when the health emergency is resolved, it will be mandatory to redesign an economic-productive model in balance with the environment.

Functional capacity assessment and Minimal Clinically Important Difference in post-acute cardiac patients: the role of Short Physical Performance Battery

Summary: This study assessed physical performance and rehabilitation outcomes in elderly patients after a cardiac event and identified the Minimal Clinically Important Difference (MCID) of the Short Physical Performance Battery (SPPB). The results showed that patients with lower SPPB scores had longer hospital stays and higher complication rates. An improvement of more than 1 point on the SPPB was determined as the MCID, which can be used as a goal for rehabilitation interventions.

Summary: This study aimed to verify whether cardiopulmonary exercise testing (CPET) can predict outcomes in asymptomatic left ventricular dysfunction (ALVD) patients and identify effective CPET parameters for this group. The study found that CPET can predict events in ALVD patients, but the risk stratification relies on different parameters than in chronic heart failure (HF) patients.

Air Pollution Role as Risk Factor of Cardioinhibitory Carotid Hypersensitivity

Summary: Little is known about the impact of air pollution on the neuroautonomic system. The study found that performing carotid sinus massage under high ozone concentrations may slightly increase the risk of a cardioinhibitory response, but other pollutants and outdoor temperature do not seem to affect autonomic control under basal resting conditions. Ozone appears to influence the response to carotid sinus massage in patients with carotid sinus hypersensitivity, possibly by increasing nerve acetylcholine release to facilitate a cardioinhibitory response. P -> PR coherence may be useful in predicting sinus cardioinhibitory hypersensitivity in cases where carotid sinus massage is contraindicated.

Summary: Metabolic syndrome is a clinical condition characterized by the aggregation of major risk factors for cardiovascular disease and type 2 diabetes, including dyslipidemia, high blood pressure, abnormal blood sugar, and abdominal obesity. These risk factors have an independent effect but become synergistic when combined, leading to a doubled risk of cardiovascular diseases and a 1.5-fold increase in all-cause mortality. This article explores gender differences in the epidemiology, etiology, pathophysiology, and clinical expression of metabolic syndrome components, as well as gender differences in new biochemical markers of metabolic syndrome and cardiovascular risk.

Summary: COVID-19 is a highly variable disease with different symptoms and signs. Men are more susceptible to the infection, with a higher rate of hospitalization and mortality compared to women. The impact of cardiovascular diseases on COVID-19 outcomes differs between men and women, potentially due to sex-related biological factors and behavioral traits. The renin-angiotensin system and the angiotensin-converting enzyme 2 play key roles in disease pathogenesis, showing hormone-dependent actions. Men also have a higher incidence of myocardial injury associated with inflammation and coagulation disorders. The relationship between cardiovascular diseases and COVID-19 during pregnancy is also discussed.

Summary: The recent pandemic has brought significant changes to the approach of preventing cardiovascular diseases in women. The changes caused by the pandemic and quarantine measures have had a substantial impact on women, affecting both preventive visits and screenings, as well as social and economic factors. It is important to adopt new cardiovascular prevention approaches, focusing on healthy lifestyles, stress reduction, and the use of modern tools like telemedicine and mobile applications, especially for young and adult women. However, a more traditional approach may be necessary for older women. This review discusses the new approach to cardiovascular prevention in women, taking into account the lifestyle changes during the pandemic, the increase in obesity, the effects of stress and depression on the cardiovascular system, and specific high blood pressure guidelines for women.

Electrocardiographic and other Noninvasive Hemodynamic Markers in Decompensated CHF Patients

Summary: This study aims to validate the use of QR interval or RpT as risk markers for identifying acutely decompensated chronic heart failure (adCHF). 140 patients with suspected adCHF were enrolled, and their ECG intervals were measured. The results showed that V5-, V6- RpT and QR(SD), QRS(SD), QT(SD), JT(SD), and Te-SD were significantly higher in the adCHF group. Logistic regression analysis demonstrated that the mean values of QT and Te were the most reliable predictors of in-hospital mortality.

Effect of Head-Up/-Down Tilt on ECG Segments and Myocardial Temporal Dispersion in Healthy Subjects

Summary: The autonomic nervous system (ANS) can modulate the oscillation of electrocardiogram segments and their intervals, including the myocardial repolarization phase. This study investigates the influence of head-up/-down tilt on ECG segments, suggesting that cardiopulmonary postural reflexes can modulate ECG interval oscillations. T wave amplitude decreases during head-up tilt and correlates with left ventricular end-systolic volume. Head-up/-down tilt acutely modifies the autonomic nervous system balance through deactivation of cardiopulmonary reflexes.
