Complications in Post-mastectomy Immediate Breast Reconstruction: A Ten-year Analysis of Outcomes

Breast reconstruction offers cosmetic and psychological advantages for women who choose to have a mastectomy. This study aims to evaluate the complications of the various options in 243 immediate breast reconstruction procedures. The reoperative complication rate was low at 6.6%. Diabetes mellitus, obesity, and a free pedicle autologous reconstruction were identified as risk factors for complications. Introduction: Immediate breast reconstruction offers cosmetic and psychological advantages post-mastectomy. There are various options of reconstruction, and this study aims to evaluate the associated complications. Patients and Methods: This is a single-center retrospective study analyzing data from January 1, 2008 to December 31, 2017 for immediate breast reconstruction procedures post-mastectomy performed at an academic breast unit. Procedures included expander and implant insertion, latissimus dorsi, pedicle transverse rectus abdominis musculocutaneous (TRAM), free TRAM, and deep inferior epigastric perforator. Complications and reoperative complications (defined as those requiring a reoperation within the first 30-day period), and associated risk factors were investigated using separate logistic regressions, and odds ratios (ORs) were calculated. Results: A total of 243 post-mastectomy immediate breast reconstruction procedures and complications rates were analyzed. The overall complication rate was 27.6%, comprised mainly of reconstruction-specific complications such as post-TRAM hernia or bulges, fat necrosis, and implant capsular contracture and leakage. The rate of reoperative complications was relatively low at 6.6%. The flap failure rate was similar between expander/implant reconstruction and autologous reconstruction methods at 3.3% and 5.6%, respectively (P = .60). Logistic regression identified significantly higher risks associated with diabetes mellitus (OR, 5.21; P = .022), obesity (OR, 5.80; P = .016), and free pedicle autologous reconstruction (OR, 3.975; P = .046) for reoperative complications. Conclusion: Different methods of immediate breast reconstruction post-mastectomy are feasible and safe. However, patient variables and procedure choice should be taken into consideration when counseling patients on reconstructive options, as they are strong predictors for postoperative complications. Prospective Clinical Trial on Expanding Indications for Cryosurgery for Early Breast Cancers

Summary: This study is a prospective single-arm clinical trial on cryosurgery for early breast cancers, evaluating the expanded criteria for tumors larger than 1.5 cm and non-luminal breast cancers. The results showed that residual cancer was found in 46.7% of patients, all of which were located at the periphery of the cryoablation zone. Careful pre-operative planning and intra-operative monitoring are crucial to ensure complete cryoablation.

Summary: The purpose of this study is to evaluate the effectiveness of the Chinese version of the ConquerFear-HK intervention in improving fear of cancer recurrence. A randomized controlled trial will be conducted with 174 eligible Chinese cancer survivors who will be assigned to either the ConquerFear-HK or BasicCancerCare intervention. Various measures will be used to assess the intervention's effects and psychological impacts.

Use of Superparamagnetic Iron Oxide (SPIO) Versus Conventional Technique in Sentinel Lymph Node Detection for Breast Cancer: A Randomised Controlled Trial

Summary: The study compares the efficacy of superparamagnetic iron oxide (SPIO)-guided and standard techniques for detecting sentinel lymph nodes (SLN) in early breast cancer. Multiple inferiority trials have shown that SPIO is not inferior to conventional techniques in detecting SLNs. The results indicate that single-tracer SPIO-guided SLNB is as effective as the dual technique (radioisotope and blue dye) and can safely replace the gold standard for SLN mapping in early breast cancer.
