The Regional Cerebral Oxygen Saturation Effect of Inotropes/Vasopressors Administered to Treat Intraoperative Hypotension: A Bayesian Network Meta-analysis

Anna Maria Bombardieri, Narinder P. Singh, Lauren Yaeger, Umeshkumar Athiraman, Ban C. H. Tsui, Preet Mohinder Singh

Summary: One of the main concerns of intraoperative hypotension is the adequacy of cerebral perfusion. Treatment options for intraoperative hypotension include the use of different drugs, such as inotropes and vasopressors, but there is no consensus on the preferred drug. A network meta-analysis was conducted to compare the effect of various inotropes/vasopressors on cerebral oxygen saturation (ScO2) in patients undergoing surgery under anesthesia. Results showed that dopamine, ephedrine, and norepinephrine had the lowest probability of decreasing ScO2.

Continuous interscalene versus phrenic nerve-sparing high-thoracic erector spinae plane block for total shoulder arthroplasty: a randomized controlled trial

Summary: This study compared the use of high-thoracic erector spinae plane block (HT-ESPB) and interscalene block for total shoulder arthroplasty. The results showed that the incidence of hemidiaphragmatic paralysis was significantly lower in the HT-ESPB group compared to the interscalene group. There were no significant differences in pain scores and opioid consumption between the two groups.

The effect of intraoperative lidocaine administration in a disrupted erector spinae fascial plane on intercostal transcranial motor evoked potentials

Summary: The study aimed to investigate the effects of local anesthesia administration through ESP catheters on intraoperative intercostal tcMEPs in pediatric patients undergoing posterior spine fusion. The results showed that lidocaine administration through ESP catheters did not cause any changes in intercostal tcMEPs in the 26 patients studied.

Role of neuromodulation in acute pain settings

Ban C. H. Tsui, Rajnish K. Gupta

Summary: Peripheral nerve stimulation (PNS), a neuromodulatory technique, is increasingly used for chronic pain syndromes and as an adjunct analgesic modality in acute pain settings. While PNS shows promise in perioperative analgesic control, there are significant obstacles that need to be addressed before it can be implemented into standard practice.

Utility of Electrical Stimulation for Correct Placement and Neuromodulation of the Erector Spinae Plane Block for Total Shoulder Arthroplasty: A Case Report

Jan Boublik, Richard K. Kim, Ban C. Tsui

Summary: The case report introduces a patient who underwent shoulder arthroplasty using a high thoracic-erector spinae plane (HT-ESP) catheter for electrical and chemical neuromodulation. This highlights the potential of electrical stimulation in identifying and treating the fascial plane level.

Catherine T. Byrd, Richard K. Kim, Pooja Manapat, Hao He, Ban C. H. Tsui, Joseph B. Shrager, Mark F. Berry, Leah M. Backhus, Natalie S. Lui, Douglas Z. Liou

Summary: This study investigated the incidence and causes of interruption of thoracic epidural analgesia in esophagectomy patients and its association with postoperative outcomes. Results showed that 35.7% of patients experienced epidural interruption, with hypotension being the most common cause. Patients with epidural interruption had a higher risk of complications such as heart failure, atrial fibrillation, delirium, sepsis, and severe anastomotic leak. Female gender was associated with epidural interruption. Rating: 8/10

Summary: Through a Delphi international consensus study, standardized names and anatomical descriptions for upper and lower limb peripheral nerve blocks were established, with strong consensus achieved for the majority of items. This framework will improve research, education, and clinical practice in regional anesthesia, ultimately benefiting patient care.
