Herpes simplex virus: global infection prevalence and incidence estimates, 2016

Objective To generate global and regional estimates for the prevalence and incidence of herpes simplex virus (HSV) type 1 and type 2 infection for 2016. Methods To obtain data, we undertook a systematic review to identify studies up to August 2018. Adjustments were made to account for HSV test sensitivity and specificity. For each World Health Organization (WHO) region, we applied a constant incidence model to pooled prevalence by age and sex to estimate the prevalence and incidence of HSV types 1 and 2 infections. For HSV type 1, we apportioned infection by anatomical site using pooled estimates of the proportions that were oral and genital. Findings In 2016, an estimated 491.5 million people (95% uncertainty interval, Ul: 430.4 million-610.6 million) were living with HSV type 2 infection, equivalent to 13.2%of the worlds population aged 15-49 years. An estimated 3752.0 million people (95% UI: 3555.5 million-3854.6 million) had HSV type 1 infection at any site, equivalent to a global prevalence of 66.6% in 0-49-year-olds. Differing patterns were observed by age, sex and geographical region, with HSV type 2 prevalence being highest among women and in the WHO African Region. Conclusion An estimated half a billion people had genital infection with HSV type 2 or type 1, and several billion had oral HSV type 1 infection. Millions of people may also be at higher risk of acquiring human immunodeficiency virus (HIV), particularly women in the WHO African Region who have the highest HSV type 2 prevalence and exposure to HIV.

Impact of diabetes mellitus on tuberculosis epidemiology in Indonesia: A mathematical modeling analysis

Summary: This study investigated and forecasted the impact of diabetes mellitus (DM) on tuberculosis (TB) epidemiology in Indonesia. The findings suggest that DM will play an increasingly significant role in the incidence and mortality of TB over the next three decades. Sensitivity and uncertainty analyses confirmed the predictions, and joint management and programs are proposed to reduce TB incidence and mortality.

Analyzing inherent biases in SARS-CoV-2 PCR and serological epidemiologic metrics

The results show that prolonged PCR positivity leads to biased epidemiological measurements, while the time delay in detecting antibodies underestimates the true prevalence of infection.

Summary: The performance of the fluorescence LFIA Finecare (TM) 2019-nCoV S-RBD test was evaluated and found to have high sensitivity and specificity, with consistent results compared to reference assays. This test can be used for quantitative measurement of SARS-CoV-2 antibodies and serves as an alternative method to assess antibody response after infection or vaccination in small or non-laboratory settings.

Summary: A study in Qatar showed that booster vaccination significantly reduced the incidence of infection with the omicron variant, providing strong protection against Covid-19-related hospitalization and death. Booster effectiveness was observed for both mRNA vaccines, BNT162b2 and mRNA-1273, with lower infection rates and reduced severity of cases among those who received the booster dose.

Estimated economic burden of genital herpes and HIV attributable to herpes simplex virus type 2 infections in 90 low- and middle-income countries: A modeling study

Summary: This study estimated the economic and quality-of-life losses due to genital herpes in low- and middle-income countries (LMICs) and the potential contribution of herpes simplex virus type 2 (HSV-2) infections to HIV transmission. The findings showed that the economic losses caused by genital herpes can be substantial, and the quality-of-life losses outweigh the spending on treatment and productivity reduction. If HSV-2 has indeed contributed to the spread of HIV, then a significant proportion of the costs associated with HIV treatment and wage losses can be attributed to HSV-2. Therefore, developing a vaccine against HSV-2 should be a global priority.

Summary: This study characterized the epidemiology of herpes simplex virus type 2 (HSV-2) infection in the Middle East and North Africa (MENA) region. The prevalence of HSV-2 infection in MENA was found to be lower compared to other world regions, but the infection rate may be increasing. Despite conservative sexual norms, 1 in 20 adults in MENA is chronically infected. These findings highlight the need for expanded surveillance and monitoring of HSV-2 infection in MENA.
