Summary: Endometriosis, a common gynecologic disease, often leads to various pain symptoms such as dysmenorrhea, dyspareunia, and chronic pelvic pain. Inflammatory responses, nervous system sensitization, and altered distribution of nerve fibers may contribute to chronic pain in these patients. Understanding these mechanisms is crucial in guiding treatment decisions for endometriosis-related pain symptoms.

Summary: This study found that sacral neuromodulation (SNM) may be an effective treatment for refractory chronic pelvic pain in women with endometriosis. Patients experienced pain reduction and improved quality of life after SNM implantation.

Summary: This study aimed to evaluate the diagnostic accuracy of MRI in the planning of surgical treatment for deep pelvic endometriosis. The results showed that MRI had high accuracy in detecting deep pelvic endometriosis in specific locations, allowing for the localization of lesions with highly fibrotic components. Compared to other imaging methods, MRI provides better visualization.

Summary: Women with chronic pelvic pain and/or endometriosis have an increased risk of appendiceal endometriosis, but modern appendectomy during gynecologic surgery is safe with no associated complications. Our findings support the consideration of appendectomy as part of comprehensive surgical management of endometriosis.

Summary: Endometriosis-associated pain plays a crucial role in determining the need for surgical intervention. However, there is currently no quantitative method to diagnose the intensity of local pain, especially in cases of deep endometriosis. This study examines the clinical significance of a pain score system that utilizes a pelvic examination to assess endometriotic pain. The results show a significant decrease in the pain score postoperatively, suggesting the potential use of this score to develop surgical strategies for deep endometriosis.

Summary: Chronic pelvic pain is commonly associated with additional pathologies such as adenomyosis and endometriosis. A retrospective study found that 40% of patients with associated pathologies did not experience relief from pelvic pain at the early follow-up and 32% at the 12-month follow-up after laparoscopic niche repair. Therefore, careful patient selection is important for uterine scar repair in patients with concomitant adenomyosis and endometriosis.

Summary: In deep endometriosis, endometrium-like cells penetrate more than 5mm below the peritoneum, causing anatomical changes and posing a surgical challenge. The different manifestations of the disease can be found in the #Enzian classification. Successful treatment of deep endometriosis relies on appropriate indications, preoperative preparation, and experienced surgical teams.

Summary: The research revealed that patients undergoing endometriosis surgery generally believe that cognitive behavioural therapy (CBT) should be added to the treatment regimen to improve their quality of life. Participants preferred in-person individual or group therapy and emphasized the importance of custom-made treatment plans tailored to individual needs.

Summary: The study aimed to quantify postoperative opioid use after laparoscopic surgery for endometriosis or pelvic pain, and identify patient characteristics associated with greater postoperative opioid requirements. Results showed that most patients had a lower postoperative opioid requirement than prescribed, suggesting surgeons can prescribe fewer postoperative narcotics in this population. Patients with a previous surgery for pelvic pain, self-reported chronic pelvic pain syndrome, anxiety, and depression may represent a subset of patients with increased postoperative opioid requirements.

Summary: A study found that preoperative pelvic pain comorbidities were negatively associated with pain-related quality of life after endometriosis surgery. Depression and musculoskeletal/myofascial pain (abdominal wall pain and pelvic floor myalgia) were the most important factors. Therefore, these pelvic pain comorbidities should be considered in a formal prediction model for pain outcomes after endometriosis surgery.

Summary: Research on the effects of cannabis-based products on the female reproductive system, particularly in the context of endometriosis, is limited and inconclusive. While some studies suggest potential benefits, there is a lack of well-designed, robust studies and randomized controlled trials. National guidance in the UK currently does not recommend the use of cannabis-based products due to insufficient evidence of their benefits. Further comprehensive research is needed before their use can be recommended or prescribed.

Summary: This study aimed to assess the effects of one-year dienogest therapy on the clinical picture, pain patterns, psycho-emotional status, and quality-of-life indicators in adolescents with endometriosis after surgical treatment. Results showed that dienogest therapy significantly alleviated clinical symptoms, improved quality of life, reduced pain symptoms, and showed good tolerability and compliance.

Summary: This study aimed to identify metabolites in presurgical blood associated with the risk of persistent postsurgical pelvic pain in adolescent and young adult patients undergoing endometriosis surgery. The results indicate that dysregulation of multiple groups of lipid metabolites may play a role in the persistence of pelvic pain after surgery among young endometriosis patients.

Summary: This study identified 30 parameters and 6 numeric rating scale scores associated with endometriosis through questionnaires and examinations. Women with endometriosis had longer pain duration and may experience radiating pain to the thighs/legs and dysuria. A non-invasive clinical prediction model based on these parameters and scores can be used for diagnosis and surveillance of endometriosis.

Summary: This study analyzed the expression levels of microRNAs in endometrial samples from recurrent implantation failure patients and healthy controls, and found that certain microRNAs may be involved in the development of RIF. These microRNAs could potentially serve as biomarkers for early diagnosis of RIF patients.

Summary: This retrospective cohort study examined the impact of adenomyosis on the clinical and survival outcomes of patients with coexistent endometrial cancer (EC). Although adenomyosis was associated with prolonged disease-specific survival (DSS), it did not have prognostic importance for survival outcomes of EC patients.

Summary: This study aimed to assess whether dual trigger alters the ploidy chance of a blastocyst in PGT-A cycles, and found that dual triggering did not provide additional superiority on blastocyst euploidy rate compared to hCG triggering. Further studies in women with different infertility etiology are needed to evaluate the impact of triggering method on ploidy rates.

Summary: This study aimed to investigate the effect of endometrial thickness (ENT) change in response to progesterone on pregnancy outcomes in fresh oocyte donation (OD) recipients. The results showed that the change in ENT after 6 days of progesterone administration, whether increased or decreased, did not significantly affect the live birth rate (LBR) and clinical pregnancy rate (CPR) in fresh OD recipients.

Summary: This study compared the outcomes of pregnant women who underwent labor induction with intravaginal misoprostol or had spontaneous labor. The results showed that the group induced with misoprostol had higher rates of vaginal delivery within 12 hours and shorter time from the last dose to delivery. Intravaginal misoprostol appears to be a promising option for labor induction due to its effectiveness, convenience, and affordability.

Summary: This study aimed to compare the effects of early cord clamping (ECC), delayed cord clamping (DCC), and umbilical cord milking (MC) on maternal and neonatal outcomes in elective cesarean births. The results showed that DCC and MC are superior to ECC in terms of reducing intraoperative bleeding and shortening the duration of the operation, without negatively affecting maternal and neonatal outcomes.
