Ocular Manifestations in an Italian Cohort of Patients with Takayasu Arteritis

Rosanna Dammacco, Luca Cimino, Luca De Simone, Giovanni Alessio, Franco Dammacco

Summary: This study describes ophthalmic manifestations, therapy, and outcomes in 16 patients with Takayasu arteritis. Ocular manifestations were a common feature in this cohort and frequently led to severe visual deterioration. Glucocorticoids associated with immunosuppressive agents induced sustained remission, and the monoclonal antibody tocilizumab showed potential for relief in relapsing-refractory patients.

Assessment of the Anti-inflammatory Effects of NORFLO® ORO in Acute Relapses of HLA-B27-associated Autoimmune Uveitis: A Multicenter, Randomized, Placebo-controlled, Double-blind Clinical Study

Pia Allegri, Luca Cimino, Janet L. Davis, Ilknur Tugal-Tutkun

Summary: This study investigated the effectiveness and safety of PHBC in reducing the number and intensity of HLA-B27-related AAU relapses. The results showed that compared to the placebo group, the PHBC group had fewer relapses and a higher proportion of responders, with comparable severity of attacks. Therefore, PHBC treatment may be beneficial.

Exploring Gene Expression Profiles in Primary Central Nervous System Vasculitis

Carlo Salvarani, Jonas Paludo, Gene G. Hunder, Stephen M. Ansell, Caterina Giannini, Joseph E. Parisi, John Huston, Matthew J. Koster, Kenneth J. Warrington, Stefania Croci, Robert D. Brown

Summary: This study used RNA sequencing to explore the gene expression profile of primary central nervous system vasculitis (PCNSV). The results indicated dysfunction in endosomes, mitochondria, and ribosomes, alterations in protein synthesis, and involvement of noncoding RNAs in PCNSV. Pathway analysis revealed the activation of dendritic cell maturation, antigen processing, and neuroinflammation in PCNSV, while oxidative phosphorylation was inhibited. CIBERSORT estimation suggested higher prevalence of activated NK cells, M1 macrophages, memory B cells, and follicular helper T cells in PCNSV samples.

Risk of SARS-CoV-2 reinfection by vaccination status, predominant variant and time from prior infection: a cohort study, Reggio Emilia province, Italy, February 2020 to February 2022

Massimo Vicentini, Francesco Venturelli, Pamela Mancuso, Eufemia Bisaccia, Alessandro Zerbini, Marco Massari, Andrea Cossarizza, Sara De Biasi, Patrizio Pezzotti, Emanuela Bedeschi, Paolo Giorgi Rossi

Summary: This study evaluated the risk of SARS-CoV-2 reinfection based on vaccination status, predominant variant, and time after initial infection. The results showed that two doses of vaccine and prior infection could reduce the risk of reinfection, while natural immunity had low protection against the Omicron BA.1 variant. Combined vaccination and natural immunity were more effective in preventing reinfection than either alone.

Takayasu arteritis and large-vessel giant cell arteritis in Italian population. Comprehensive analysis from a single institutional cohort of 184 cases

Luigi Boiardi, Elena Galli, Pierluigi Macchioni, Francesco Muratore, Giulia Klinowski, Gene G. Hunder, Massimiliano Casali, Giulia Besutti, Lucia Spaggiari, Annibale Versari, Stefania Croci, Pamela Mancuso, Paolo Giorgi Rossi, Chiara Marvisi, Carlo Salvarani

Summary: This study compared the clinical and imaging characteristics of Italian patients with Takayasu arteritis (TAK) and large vessel-giant cell arteritis (LV-GCA). The results showed that LV-GCA patients had a higher prevalence of males and more frequently presented with cranial symptoms, fever, polymyalgia rheumatica, and hypertension. TAK patients had longer delay to diagnosis and presented more frequently with loss of pulses of large arteries, vascular bruits, limb claudication, myocardial infarction/angina, and hypertension induced by renal artery stenosis. In terms of treatment, TAK patients received higher doses of prednisone, had longer duration of prednisone therapy, and received traditional immunosuppressants and biological agents more frequently. Flares were more frequent in TAK patients, while no differences were observed for long-term remission. New vascular procedures were more frequently performed in TAK patients. Imaging at diagnosis showed that TAK patients had more frequent vascular stenosis/occlusion and a higher number of vessels with structural damage, while LV-GCA patients had a higher number of inflamed vessels.

Epidemiology and Prevention of Early Infections by Multi-Drug-Resistant Organisms in Adults Undergoing Liver Transplant: A Narrative Review

Giovanni Dolci, Giulia Jole Burastero, Francesca Paglia, Adriana Cervo, Marianna Meschiari, Giovanni Guaraldi, Johanna Chester, Cristina Mussini, Erica Franceschini

Summary: Invasive bacterial infections are a major cause of morbidity and mortality after liver transplant, especially in the first few months. Infections caused by multi-drug-resistant organisms (MDRO) are increasing in this setting. MDRO pre-transplant colonization in both donors and recipients is a risk factor for post-transplant MDRO infections. More research is needed to develop preventive and antibiotic prophylactic strategies to reduce MDRO infections in the post-transplant period. This review aims to provide a comprehensive insight on the epidemiology of MDRO colonization and infections in adult liver transplant recipients, donor-derived MDRO infections, and potential surveillance and prophylactic strategies.

Comparable cytokine release ex-vivo by whole blood from COVID-19 patients with and without non-invasive ventilation

Martina Bonacini, Ilaria Ferrigno, Alessandro Rossi, Nicola Facciolongo, Marco Massari, Romina Corsini, Veronica Galli, Alessandro Zerbini, Carlo Salvarani, Stefania Croci

Summary: This study aimed to identify differences in cytokine release by T cells ex-vivo between COVID-19 patients with diverse disease severity. The results showed differences in cytokine release between patients of different disease severity, but these differences do not seem to explain the need for non-invasive ventilation.

CD5-Negative Primary Mantle Cell Lymphoma Presenting with a Bilateral Conjunctival Mass: A Potential Diagnostic Pitfall

Magda Zanelli, Alberto Lugli, Andrea Palicelli, Francesca Sanguedolce, Maurizio Zizzo, Camilla Cresta, Samuele Biancafarina, Giovanni Martino, Barbara Crescenzi, Saverio Pancetti, Giuseppe Broggi, Rosario Caltabiano, Luca Cimino, Cristina Mecucci, Stefano Ascani

Summary: Mantle cell lymphoma (MCL) is rarely reported in the ocular adnexa, which is more commonly affected by extranodal marginal zone B-cell lymphoma. The diagnosis of MCL in this location can be challenging as it may be overlooked and misinterpreted as marginal zone B-cell lymphoma due to its rarity and CD5 negativity. Here, we present a case of primary bilateral CD5-negative MCL in the ocular adnexa and discuss potential diagnostic pitfalls.
