Summary: This study investigated the potential effect of postoperative adjuvant therapy with lenvatinib on the long-term prognosis of hepatitis B virus-related hepatocellular carcinoma with microvascular invasion (MVI). The results showed that patients who received postoperative lenvatinib had improved recurrence rates and survival rates compared to those who did not receive the treatment. Nomograms were also used to accurately predict long-term survival.

Summary: This study compared the efficacy of combining systemic therapy with transarterial-based therapy versus transarterial chemoembolization (TACE) alone in hepatocellular carcinoma (HCC) patients with portal vein tumour thrombus (PVTT). The combination therapy group had higher overall survival rates, longer progression-free survival, and more cases of tumor downstaging and pathological complete response after salvage liver resection. The adverse event rates were similar between the two groups. Therefore, combining therapy is a safe and promising treatment option for HCC patients with PVTT.

Summary: A retrospective analysis found that HCC patients with portal vein tumor thrombosis (PVTT) had significantly higher preoperative circulating tumor cell (CTC) counts compared to those without PVTT. Patients with CTC counts ≥ 2 had significantly lower overall survival and disease-free survival rates. Therefore, preoperative CTC counts have the potential to be a valuable clinical tool in predicting the prognosis of HCC patients with PVTT.

Summary: The objective of this study was to identify the most effective individual drug in a triple-drug therapy for preventing postoperative pancreatic fistula (POPF) using a rat model. The researchers compared the effects of a somatostatin analog, gabexate mesilate, and imipenem/cilastatin on POPF in rats and evaluated levels of serum amylase and lipase, ascitic amylase and lipase, intraperitoneal adhesion, and pancreatic inflammation. The results showed that the somatostatin analog was the most effective drug against POPF, as it significantly reduced levels of ascitic amylase and lipase and inflammation in the pancreas.

Summary: Portal vein thrombosis following liver transplantation can be treated with endovascular techniques. Trans-splenic access is an interesting option as it avoids liver graft damage, but it is not possible after splenectomy. We report a successful case of percutaneous intervention with direct puncture of the retropancreatic splenic vein for portal vein thrombosis following living donor liver transplantation with simultaneous splenectomy. Our technique proved effective with no recurrence of thrombosis for 2 years.

Summary: This study retrospectively analyzed 280 cases of adult living donor liver transplantation (LDLT) and found that a graft-to-recipient weight ratio (GRWR) of less than 0.6% was associated with an increased risk of small-for-size syndrome (SFSS). Although overall graft survival rates were similar, appropriate donor and recipient selection is crucial for successful LDLT with very small grafts.

Summary: We present a case of pathologic complete response in a patient with advanced hepatocellular carcinoma (HCC) complicated with portal venous tumor thrombus, treated with atezolizumab and bevacizumab, followed by radical resection. The patient showed significant tumor shrinkage and decrease in tumor marker levels after systemic therapy. Radical resection was performed and a complete response was observed in the pathological examination. This case highlights the potential efficacy of atezolizumab plus bevacizumab as a treatment option for advanced HCC.
