COVID-19 pneumonia in lung transplant recipients: Report of 2 cases

Coronavirus disease 2019 (COVID-19) has been declared pandemic since March 2020. In Europe, Italy was the first nation affected by this infection. We report anamnestic data, clinical features, and therapeutic management of 2 lung transplant recipients with confirmed COVID-19 pneumonia. Both patients were in good clinical condition before the infection and were receiving immunosuppression with calcineurin inhibitors (CNI), mycophenolate mofetil, and corticosteroids. Whereas mycophenolate mofetil was withdrawn in both cases, CNI were suspended only in the second patient. The first patient always maintained excellent oxygen saturation throughout hospitalization with no need for additional oxygen therapy. He was discharged with a satisfactory pulmonary function and a complete resolution of radiological and clinical findings. However, at discharge SARS-CoV-2 RNA could still be detected in the nasopharyngeal swab and in the stools. The second patient required mechanical ventilation, had a progressive deterioration of his clinical conditions, and had a fatal outcome. Further insight into SARS-CoV-2 infection is eagerly awaited to improve the outcome of transplant recipients affected by COVID-19 pneumonia.

Autologous Blood Pleurodesis: What Is the Optimal Time Interval and Amount of Blood?

Summary: This study compared the effects of autologous blood patch pleurodesis using different blood volumes on patients with prolonged air leak after lung resection. The results showed that injecting 120mL of blood can significantly reduce chest tube removal time, length of hospital stay, and complications, indicating its potential as an effective treatment option.

Successful Lung Transplantation in a Patient With History of Lobectomy for Small Cell Lung Cancer: A Case Report

Summary: A recent history of malignancy is a strict contraindication for lung transplantation, but a 5-year disease-free interval is required in most cases. There are currently no specific guidelines for selecting recipients with a previous history of lung cancer. A case is reported of a patient who underwent lung transplantation for emphysema after undergoing lobectomy and treatment for small cell lung cancer, with no recurrence of neoplastic disease at present.

P14/ARF-Positive Malignant Pleural Mesothelioma: A Phenotype With Distinct Immune Microenvironment

Summary: The CDKN2A gene is important in the pathogenesis of malignant pleural mesothelioma (MPM). The study found that p14/ARF-positive epithelioid mesotheliomas may represent a more aggressive pathological phenotype, while p14/ARF-negative tumors seem less sensitive to immune checkpoint inhibitors. The association between p14/ARF-positive MPMs and PD-L1 expression suggests a possible interaction between the two pathways.

Diagnostic Challenges in Epithelioid Pleural Mesothelioma: Case Series with Support from Electron Microscopy

Summary: The histological diagnosis of pleural epithelioid mesothelioma can be challenging in cases of rare variants or patients with previous or concomitant tumors. Immunohistochemical reactions play a role in obtaining a sensitive and specific diagnosis of malignant pleural mesothelioma but sometimes the final interpretation remains inconclusive. Ultrastructural examination has historically been a useful tool in defining the mesothelial nature of neoplastic cells.

Lobar size reduction in lung transplantation: A propensity score study

Summary: This study examined the use of lobar reduction in lung transplantation for small-sized recipients. The results showed that lobar reduction had comparable early and long-term outcomes to standard lung transplantation, with no significant impact on patients' morbidity and survival.

Surgical management of post-transplant bronchial stenoses: a single-center experience

Summary: Bronchial stenoses are challenging complications after lung transplantation, with 7.0% of patients experiencing it in this study. While endoscopic management was effective in most cases, 1 patient required re-transplantation and 6 patients underwent bronchoplasty or sleeve resection. Of these patients, 2 remained alive with preserved pulmonary function at a median of 12 months post-surgery.

Spontaneous Regression of an Epidermal Growth Factor Receptor-mutant Left Upper Lobe Adenocarcinoma

Summary: This article presents a case of spontaneous regression of thoracic malignancy in a patient with epidermal growth factor receptor-mutant adenocarcinoma. The patient experienced contralateral aspiration pneumonia before surgery, but showed complete pathologic regression without any preoperative oncologic treatment. Immune system activation and the role of tumor microenvironment in disease progression and treatment are highlighted.

Summary: This study investigated the outcomes of recipients bridged to a first-time retransplantation (ReTx) by extracorporeal life support (ECLS). The results showed that the overall survival of the ECLS-ReTx group was significantly worse compared to the regular-ReTx and index-Tx groups. Risk factors such as recipient age, intertransplant interval, primary graft dysfunction as transplant indication, and type of ECLS were identified as significant factors affecting mortality in ECLS-ReTx recipients.
