The Era of Cortical Bone Trajectory Screws in Spine Surgery: A Qualitative Review with Rating of Evidence

The use of cortical bone trajectory (CBT) pedicle screws for posterior fixation and fusion seems to constitute a viable alternative for spinal procedures, with the potential to mitigate risks, be minimally invasive, and cause less tissue damage than the traditional technique. This review analyzes the literature regarding CBT according to the rate of evidence of articles and their main focus. CBT has proved to be a safe and viable option for screw fixation in spine sup gery. Given the denser bone interception, high-quality biomechanics studies show equal or even better properties compared with classic pedicle screw fixation, depending on several factors such as screw size and length. Through the years, surgical technique has improved to gain a longer and safer trajectory than first described, Level 2 and 3 clinical studies suggest equal clinical and radiologic outcomes compared with pedicle trajectory fixation, but high-quality, level 1, randomized controlled trials are needed to confirm these results.

Pituitary surgery during Covid-19: a first hand experience and evaluation

Summary: The study discusses the features and applications of different types of mesenchymal stem cells in peripheral nerve regeneration after nerve injury. ADSCs are considered the most suitable MSCs for recovery from peripheral nerve lesions due to their excellent support and stimulation of axonal growth, remarkable paracrine activity, differentiation potential, low immunogenicity, and high survival rate.

Summary: Chronic traumatic encephalopathy (CTE) is a neurodegenerative disease caused by repetitive brain injuries, often seen in contact sports, which can lead to suicidal behavior. Two cases of retired professional athletes in high contact sports with similar clinical trajectories and severe suicidal behavior were reported. The findings can help improve the identification of CTE patients and provide mechanistic insights for the prevention of potential severe suicidal behavior.

Summary: This study aimed to compare the differences between free-hand three-dimensional (3D) planning-guided cortical bone trajectory (CBT) screw placement and 3D-printed template-guided CBT screw positioning in terms of accuracy, size of screws, and potential complications. Data from patients who underwent CBT screws for lumbar degenerative pathologies were analyzed and compared between the free-hand and template-guided groups. The template-guided group showed larger screws size and significant differences in accuracy parameters.

Modic Changes Increase the Cage Subsidence Rate in Spinal Interbody Fusion Surgery: A Systematic Review and Network Meta-Analysis

Summary: This study aimed to compare the effect of different Modic changes (MC) grades on the cage subsidence rate after spinal interbody fusion surgery. A meta-analysis was conducted on relevant randomized controlled trials and cohort studies. The results showed that MC may be associated with a higher cage subsidence rate. With the increase in MC grades, the incidence of subsidence decreased gradually, but it was always higher than that in the non-Modic changes (NMC). Oblique lumbar interbody fusion may be a better treatment choice for lumbar degenerative disease with MC.

A Medical Student's Guide to Online Resources in Neurosurgery

Summary: Early exposure to neurosurgery during medical school is critical for improving recruitment into the specialty. However, about 30% of medical schools in the U.S. lack a neurosurgery program, limiting students' exposure to the field. Virtual education, facilitated by webinars during the pandemic, has advantages such as global outreach and accessibility for international medical students. This review identifies and describes 16 educational resources, serving as a guide for medical students interested in neurosurgery.
