Summary: Endometriosis, a common gynecologic disease, often leads to various pain symptoms such as dysmenorrhea, dyspareunia, and chronic pelvic pain. Inflammatory responses, nervous system sensitization, and altered distribution of nerve fibers may contribute to chronic pain in these patients. Understanding these mechanisms is crucial in guiding treatment decisions for endometriosis-related pain symptoms.

Summary: Chronic pelvic pain is commonly associated with additional pathologies such as adenomyosis and endometriosis. A retrospective study found that 40% of patients with associated pathologies did not experience relief from pelvic pain at the early follow-up and 32% at the 12-month follow-up after laparoscopic niche repair. Therefore, careful patient selection is important for uterine scar repair in patients with concomitant adenomyosis and endometriosis.

Summary: Research on the effects of cannabis-based products on the female reproductive system, particularly in the context of endometriosis, is limited and inconclusive. While some studies suggest potential benefits, there is a lack of well-designed, robust studies and randomized controlled trials. National guidance in the UK currently does not recommend the use of cannabis-based products due to insufficient evidence of their benefits. Further comprehensive research is needed before their use can be recommended or prescribed.

Summary: This study investigated the association between pelvic floor, abdominal, and uterine tenderness with central nervous system pain amplification among women with endometriosis or CPP. The results suggest that high pelvic floor tenderness may be a marker of heightened pain sensitivity and central nervous system pain amplification, impacting treatment response. Further research is needed to explore the predictive value of this clinical phenotype on response to medical and behavioral treatments for endometriosis and CPP.

Summary: This study assessed the differences in resting electroencephalogram (EEG) oscillatory amplitudes between women with endometriosis and healthy controls, and explored the relationship with chronic pelvic pain. The results showed that women with endometriosis had higher amplitudes in the Delta and Beta frequency bands, and lower amplitudes in the Alpha frequency band. Delta and Beta amplitudes were positively associated with pain severity, but only Beta amplitude maintained this association after controlling for Delta-Beta amplitude coupling.

Summary: This study reviewed clinical data of 46 patients with endometriosis and chronic pelvic pain unresponsive to conventional therapies. The findings suggest that a combination of neuromodulators, myofascial pain therapies, and S3 pulsed radiofrequency plus inferior hypogastric plexus blockade can be effective in managing pain in these patients.

Summary: In this study, the role of sodium channel Na(V)1.7 in endometriosis-induced chronic pelvic pain was examined. Selective inhibition of this channel can alleviate pain without affecting normal sensation. This research provides a new possibility for improving the quality of life for women with endometriosis.

Summary: Endometriosis is a heterogeneous disease that can cause chronic pain in and beyond the pelvis due to neurogenic sensitization. Coinciding pain and comorbidities require specific attention. We discuss the causes, comorbidities, and management of endometriosis-associated chronic pelvic pain, advocating for a multidisciplinary approach to develop more effective treatments.

Summary: This study aimed to compare the rates of mental disorders, sexual dysfunctions, and childhood maltreatment in women with endometriosis with different levels of pelvic pain, as well as to test two models to predict current mental disorders. The findings showed that women with chronic pelvic pain had higher rates of mental disorders and sexual dysfunctions. The study highlights the importance of pain relief and interdisciplinary care in the treatment of endometriosis.

Summary: This study aimed to compare the hypoalgesic effects of elbow flexion low-load resistance exercise and high-load resistance exercise on healthy individuals. The results showed that both low-load and high-load resistance exercise significantly reduced pain threshold, indicating a decrease in pain sensitivity. Furthermore, changes in blood pressure and perceived exertion did not mediate the changes in pain threshold.

Summary: Management of chronic pelvic pain remains a huge challenge, with unimodal therapeutic options often unsuccessful. Individualised multimodal management appears to be the most promising approach, leading to improvement for a large proportion of patients. The European Association of Urology Chronic Pelvic Pain Guideline Group provides an overview of important concepts for successfully diagnosing and treating this challenging disease.

Summary: Recently, a study found a positive correlation between protein expression of FOXJ1 and the 5-year survival rate of patients with tubo-ovarian high-grade serous carcinoma (HGSC). However, protein expression of GMNN was not significantly associated with patient survival. This study provides preliminary evidence for the prognostic value of FOXJ1 in HGSC and validates the prior mRNA-based prognostic association through immunohistochemistry.

Summary: The study aimed to investigate the gene expression (MAPK1 and CAPN2) and microRNA expression (miR-30a-5p, miR-7-5p, miR-143-3p, and miR-93-5p) involved in adhesion and apoptosis pathways in different types of endometriosis. Samples were collected from patients with superficial peritoneal endometriosis (SE), deep infiltrating endometriosis (DE), and ovarian endometrioma (OE), as well as control group without endometriosis. The results showed that SE had lower expression of pro-survival genes and miRNAs compared to DE and OE, indicating different pathophysiological mechanisms for SE.

Summary: This study aimed to evaluate the impact of the pandemic on the number of elective and urgent hysterectomies for benign gynecological pathologies and the need for blood transfusions in a single-care tertiary center in the State of Sao Paulo, Brazil. The analysis showed a significant decrease of 71.9% in planned hysterectomies during the pandemic, while urgent surgeries increased by 130%. Elective hysterectomies play a crucial role in improving women's quality of life and delaying treatment can have negative physiological, psychological, and social consequences.
