Background Polycystic ovary syndrome (PCOS) is the most common endocrine disorder in women during reproductive age. It is characterised clinically by oligo-ovulation or anovulation, hyper-androgenism, and the presence of polycystic ovaries. It is associated with an increased prevalence of metabolic syndrome, cardiovascular disease and type 2 diabetes. The onset of PCOS has been associated to several hereditary and environmental factors, but insulin resistance plays a key pathogenetic role. We sought to investigate the effects of a ketogenic diet (KD) on women of childbearing age with a diagnosis of PCOS. Methods Fourteen overweight women with diagnosis of PCOS underwent to a ketogenic Mediterranean diet with phyoextracts (KEMEPHY) for 12 week. Changes in body weight, body mass index (BMI), fat body mass (FBM), lean body mass (LBM), visceral adipose tissue (VAT), insulin, glucose, HOMA-IR, total cholesterol, low density lipoprotein (LDL), high density lipoprotein (HDL), triglycerides (TGs), total and free testosterone, luteinizing hormone (LH), follicle stimulating hormone (FSH); dehydroepiandrosterone sulfate (DHEAs), estradiol, progesterone, sex hormone binding globulin (SHBG) and Ferriman Gallwey score were evaluated. Results After 12 weeks, anthropometric and body composition measurements revealed a significant reduction of body weight (- 9.43 kg), BMI (- 3.35), FBM (8.29 kg) and VAT. There was a significant, slightly decrease of LBM. A significant decrease in glucose and insulin blood levels were observed, together with a significant improvement of HOMA-IR. A significant decrease of triglycerides, total cholesterol and LDL were observed along with a rise in HDL levels. The LH/FSH ratio, LH total and free testosterone, and DHEAS blood levels were also significantly reduced. Estradiol, progesterone and SHBG increased. The Ferriman Gallwey Score was slightly, although not significantly, reduced. Conclusions Our results suggest that a KD may be considered as a valuable non pharmacological treatment for PCOS. Longer treatment periods should be tested to verify the effect of a KD on the dermatological aspects of PCOS.

Validity and reliability of an unstable board for dynamic balance assessment in young adults

Alex Rizzato, Erica Gobbi, Antonio Paoli, Giuseppe Marcolin

Summary: Scientific literature emphasizes the importance of dynamic balance in fall prevention. This study investigated the validity and reliability of an instrumented unstable board for objective assessment of dynamic balance in young adults. The results showed significant correlations between the center of pressure (CoP) and kinematic parameters, and good to excellent test-retest reliability for the kinematic parameters. The instrumented unstable board was found to be a valid and reliable tool for on-site dynamic balance assessment.

A Novel Mix of Polyphenols and Micronutrients Reduces Adipogenesis and Promotes White Adipose Tissue Browning via UCP1 Expression and AMPK Activation

Francesca Pacifici, Gina Malatesta, Caterina Mammi, Donatella Pastore, Vincenzo Marzolla, Camillo Ricordi, Francesca Chiereghin, Marco Infante, Giulia Donadel, Francesco Curcio, Annalisa Noce, Valentina Rovella, Davide Lauro, Manfredi Tesauro, Nicola Di Daniele, Enrico Garaci, Massimiliano Caprio, David Della-Morte

Summary: This study investigated the effects of a natural mixture of polyphenols and micronutrients (A5(+)) on counteracting white adipogenesis and promoting fat browning. The results showed that A5(+) significantly reduced lipids accumulation, inhibited cellular proliferation, and decreased the release of pro-inflammatory cytokines. Additionally, A5(+) promoted fat browning and fatty acid oxidation by increasing the expression of genes related to brown adipose tissue.

Investigating the Link between Ketogenic Diet, NAFLD, Mitochondria, and Oxidative Stress: A Narrative Review

Antonio Paoli, Giuseppe Cerullo

Summary: With the global increase in obesity and metabolic syndrome, the prevalence of nonalcoholic fatty liver disease (NAFLD) has risen dramatically. NAFLD is currently the most common chronic liver disease, ranging from fat accumulation to more severe forms like nonalcoholic steatohepatitis (NASH) that can lead to cirrhosis and hepatocellular carcinoma. NAFLD is characterized by altered lipid metabolism and mitochondrial dysfunction, which worsens oxidative stress and inflammation, ultimately causing hepatocyte death and severe NAFLD. A ketogenic diet (KD), very low in carbohydrates (<30 g/die), has been shown to reduce oxidative stress and restore mitochondrial function. This review aims to analyze the evidence for the therapeutic role of KD in NAFLD, focusing on the interplay between mitochondria and the liver, the effects of ketosis on oxidative stress pathways, and the impact of KD on liver and mitochondrial function.

Sustainable Strategies for Increasing Legume Consumption: Culinary and Educational Approaches

Isaac Amoah, Angela Ascione, Fares M. S. Muthanna, Alessandra Feraco, Elisabetta Camajani, Stefania Gorini, Andrea Armani, Massimiliano Caprio, Mauro Lombardo

Summary: Legumes are nutrient-dense crops with health-promoting benefits, but their consumption is hindered by various barriers. Pre-treatment methods like soaking and sprouting can reduce anti-nutritional factors and cooking time for legumes. Extrusion technology can be used to develop innovative legume-enriched products, while culinary skills such as making legume salads and soups can promote legume consumption. This review highlights the nutritional effects of legume consumption and suggests strategies to improve digestibility and promote intake through educational and culinary approaches.

Very Low-Calorie Ketogenic Diet (VLCKD) as Pre-Operative First-Line Dietary Therapy in Patients with Obesity Who Are Candidates for Bariatric Surgery

Luigi Barrea, Ludovica Verde, Luigi Schiavo, Gerardo Sarno, Elisabetta Camajani, Antonio Iannelli, Massimiliano Caprio, Vincenzo Pilone, Annamaria Colao, Giovanna Muscogiuri

Summary: Bariatric surgery is the most effective method for long-term weight loss in severe obesity patients. The pre-operative diet plays a crucial role in determining patients' suitability for surgery and their post-operative outcomes. Nutritional management of bariatric patients requires specialized expertise.
