Summary: Background: Monoallelic variants in the KIF1A gene are associated with various clinical phenotypes involving the central and peripheral nervous system. Methods: In a multicenter study, 28 index cases with heterozygous variants in KIF1A were analyzed for clinical, genetic, and neuroradiological features. Results: Nine novel monoallelic variants and one copy number variation encompassing KIF1A were identified. Mutations mainly occurred de novo and in the motor domain, with most patients showing a spectrum of ataxia-spasticity disorders and some presenting secondary impairment of oxidative metabolism. Ubiquinol supplementation therapy showed subjective benefit in a subset of patients. Conclusions: This study expanded our understanding of KIF1A-related disorders, highlighting the heterogeneous nature of manifestations and the potential therapeutic role of ubiquinol supplementation in certain patients.

Summary: Arthrogryposis is a condition characterized by multiple congenital contractures affecting two or more areas of the body. This study identified the underlying pathogenic variants in individuals with arthrogryposis and found that prenatal findings were specific for one subtype. The phenotype of certain subtypes became more characteristic at later ages. Clinical evaluation combined with Next Generation Sequencing analysis may allow for early diagnosis and suggest a favorable prognosis in certain subtypes.

Novel CTNNB1 variant leading to neurodevelopmental disorder with spastic diplegia and visual defects plus peripheral neuropathy: A case report

Summary: This study reports on a 15-year-old patient with complex hereditary spastic paraplegias, exotropia, dyskinesia, and cerebellar signs. The patient also exhibited an unreported demyelinating polyneuropathy in the lower limbs. Genetic analysis revealed a novel likely pathogenic variant in the CTNNB1 gene. The findings suggest a potential causative link between peripheral neuropathy and CTNNB1 variants.

Clinical Features in Aromatic L-Amino Acid Decarboxylase (AADC) Deficiency: A Systematic Review

Summary: AADC deficiency is a rare metabolic disorder caused by genetic variants in the DDC gene. This systematic review examined the phenotypic spectrum of the disorder and found that most patients showed symptoms within the first 6 months of life, with hypotonia and developmental delay being the main signs. Oculogyric crises, hypokinesia, and ptosis were also commonly reported symptoms.

Homozygous DDC pathogenetic variants are associated with lower CSF levels and higher Parkinson's disease rating scale scores.

These conditions often involve epilepsy or developmental and epileptic encephalopathies (DE-EE) in childhood, highlighting the importance of long-term follow-up in individuals with intellectual disability or developmental delay (ID/DD) to identify those at increased risk of later Parkinsonism.
