Red cell alloimmunisation in patients with different types of infections

Red cell alloantigen exposure can cause alloantibody-associated morbidity. Murine models have suggested that inflammation modulates red cell alloimmunisation. This study quantifies alloimmunisation risks during infectious episodes in humans. We performed a multicentre case-control study within a source population of patients receiving their first and subsequent red cell transfusions during an 8-year follow-up period. Patients developing a first transfusion-induced red cell alloantibody (N=505) were each compared with two similarly exposed, but non-alloimmunised controls (N=1010) during a 5-week alloimmunisation risk period' using multivariate logistic regression analysis. Transfusions during severe' bacterial (tissue-invasive) infections were associated with increased risks of alloantibody development [adjusted relative risk (RR) 134, 95% confidence interval (95% CI) 097-185], especially when these infections were accompanied with long-standing fever (RR 306, 95% CI 157-596). Disseminated viral disorders demonstrated a trend towards increased risks (RR241, 95% CI 089-653), in apparent contrast to a possible protection associated with Gram-negative bacteraemia (RR 058, 95% CI 013-114). Simple' bacterial infections, Gram-positive bacteraemia, fungal infections, maximum C-reactive protein values and leucocytosis were not associated with red cell alloimmunisation. These findings are consistent with murine models. Confirmatory research is needed before patients likely to develop alloantibodies may be identified based on their infectious conditions at time of transfusion.

Anti-glycoprotein antibodies and sequestration pattern of indium-labeled platelets in immune thrombocytopenia

Functional Impact of Risk Gene Variants on the Autoimmune Responses in Type 1 Diabetes

Summary: Type 1 diabetes is an autoimmune disease caused by the interaction of genetic and environmental factors. In addition to HLA gene composition, over 60 non-HLA gene regions also contribute to the susceptibility of the disease. Combining these risk genes into a score significantly improves the prediction of disease progression. Many minor-risk SNPs associated with immune genes have been identified, but their impact on gene and protein expression and their functional changes at a cellular level require further investigation.

Summary: ITP is an autoimmune bleeding disorder characterized by platelet clearance in the spleen and liver. Thrombopoietin (TPO) levels remain low in ITP. The role of GPIb antibodies in relation to platelet hepatic sequestration and TPO levels in ITP remains unclear. This study found a positive association between GPIb antibodies and TPO levels as well as platelet hepatic sequestration in patients with severe thrombocytopenic ITP.

Summary: Acute graft-versus-host disease (aGVHD) is a potentially fatal complication of stem cell transplantation. This study used mass cytometry to analyze the immune cells in children with severe aGVHD and compared the results with control groups. The onset of aGVHD was associated with the appearance of specific myeloid and lymphoid cells in the blood and affected organs such as the skin and gastrointestinal tract. The findings highlight the importance of immune cell profiling for evaluating clinical response and managing severe aGVHD.

Summary: The Netherlands Armed Forces have been successfully using deep-frozen thrombocyte concentrate (DTC) for the treatment of bleeding trauma patients since 2001. However, there is a lack of evidence for the effectiveness and safety of DTCs. The MAFOD trial aims to compare the haemostatic effect of DTCs versus room temperature-stored platelets (RSP) in surgical bleeding treatment. If proven non-inferior, frozen platelets may be used in civilian settings to improve logistics and availability for bleeding trauma patients.

Summary: Abnormal hemoglobin concentration significantly affects a person's quality of life and physiology. Current tools for evaluating hemoglobin-related outcomes are lacking, leading to uncertainty in optimal hemoglobin levels and treatment targets. Reviews show that increasing hemoglobin improves patient-reported and physical outcomes, particularly at lower hemoglobin levels. However, there is a lack of high-quality evidence in this area.

Summary: Acute lung injury after cardiac surgery is associated with an unfavorable outcome. Prior to surgery, cardiac surgery patients who developed lung injury showed higher platelet activatability and indications for higher neutrophil turnover. Further research is needed to determine the relationship between these factors and the development of lung injury after cardiac surgery.
