During the past two decades human exposure to bisphenol A (BPA) and phthalates such as di-iso-butyl phthalate (DiBP), di-n-butyl phthalate (DnBP), butylbenzyl phthalate (BBzP) and di-(2-ethyl-hexyl) phthalate (DEHP) has received substantial interest due to widespread population exposures and potential endocrine disrupting effects. Therefore, these chemicals have gradually been restricted and phased out through legislation. However, humans are still exposed to a wide range of other less studied phthalates, phthalate substitutes and SPA analogues as well as other polychlorinated and phenolic substances. In this study, we investigated human exposure to these chemicals over the past decade. Three hundred urine samples collected in 2009, 2013 and 2017 (100 samples each year) from young Danish men of the general population, participating in a large on-going cross-sectional study, were selected for the present time trend study. The urinary concentration of metabolites of 15 phthalates, di-2-ethylhexyl terephthalate (DEHTP) and di-iso-nonyl-cyclohexane-1,2-di-carboxylate (DINCH), seven bisphenols including BPA, bisphenol S (BPS) and bisphenol F (BPF), as well as triclosan, triclocarban, benzophenone-3, three chlorophenols and two phenylphenols were analyzed by two new sensitive LC-MS/MS methods developed and validated for the present study. A significant decrease in urinary concentrations over time was observed for the majority of the chemicals. Median concentrations of BPA and the metabolites of DiBP, DnBP, BBzP and DEHP were more than halved from 2009 to 2017. Similar decreases were observed for triclosan and the chloro- and phenylphenols. In contrast, metabolites of the two phthalate substitutes DEHTP and DINCH increased more than 20 and 2 times, respectively. The potential BPA substitutes; BPS and BPF also increased, but only slightly. Despite these new exposure patterns, the exposure to the old well-known chemicals, such as DiBP, DnBP, BBzP, DEHP and BPA was still higher in 2017 compared to the exposure level of the new substitutes such as DEHTP, DINCH, BPS and BPF. A significant decrease in internal exposure to most of the common phthalates and BPA over the past decade was observed, reflecting market changes and regulatory measures implemented in EU. Despite increasing exposures to some of the known phthalate substitutes and BPA analogues, the total amount of each measured chemical group (original and substitute analytes combined) was lower in the more recently collected samples. This indicates only partial direct substitution or substitution by chemicals not covered in this approach, or a general decline in the exposure to these chemical/product groups over the last decade.

Pituitary-testis axis dysfunction following adjuvant androgen deprivation therapy

Summary: Men with high-risk, non-metastatic prostate cancer receive adjuvant androgen deprivation therapy (ADT) for at least 2 years according to Danish guidelines. It remains unclarified if patients regain the function of the pituitary-testis axis after cessation of ADT. Our study investigated the function of the pituitary-testis axis following adjuvant ADT and found that persistent testosterone deficiency (TD) is a common long-term consequence, equally distributed between pituitary and testicular dysfunction. The study emphasizes the necessity for systematic follow-up of full pituitary-testis axis function in patients receiving adjuvant ADT.

Testicular torsion and subsequent testicular function in young men from the general population

Summary: This study aimed to investigate whether prior testicular torsion is associated with testicular function in young men from the general population. The results showed no differences in semen parameters between men who had experienced testicular torsion and controls. However, subtle alterations in reproductive hormones were observed, with higher FSH and lower inhibin B levels in men with prior testicular torsion. Follow-up studies are needed to assess any long-term consequences for fertility.

Summary: This study reveals a significant decline in sperm count among men from all continents, with a more pronounced decrease observed post-2000. The decline in sperm count is not limited to North America-Europe-Australia, but also seen in South/Central America-Asia-Africa. Urgent research and action are needed to investigate the causes and prevent further disruption of male reproductive health.

Result indicators provide information on chemical exposure in different demographic groups and allow comparisons to identify differences and inform policy measures. Summary: It has been observed over the past 25 years that exposure to certain phthalates can have negative effects on human health, such as disrupting the endocrine system. As a result, the EU implemented regulations which led to a decrease in production of restricted phthalates and an increase in the production of substitutes. A recent study examined the impact of these regulations on exposure patterns in two European populations.

Prenatal and current phthalate exposure and cognitive development in 7-year-old children from the Odense child cohort

Summary: This study investigated the association between prenatal and concurrent exposure to phthalates and IQ in 7-year-old children. The results showed that high concentrations of metabolites of di(2-ethylhexyl) phthalate were associated with decreased IQ in children. This suggests that exposure to phthalates may be harmful to child neurodevelopment.

Summary: Adult patients with Klinefelter syndrome (KS) exhibit variable phenotypes and a high risk of metabolic disorders. Most adults require testosterone replacement therapy (TRT), but its use during puberty is debated. This study found that patients with KS have unfavorable body composition and impaired bone mineral status already during childhood and adolescence. Further research is needed to evaluate the effects of TRT during puberty on these parameters.

Biochemical identification of prepubertal boys with Klinefelter syndrome by combined reproductive hormone profiling using machine learning

Summary: This study aimed to characterize the clinical profiles of prepubertal boys with Klinefelter syndrome (KS) and develop a novel biochemical classification model to identify KS before puberty. Retrospective longitudinal data from 15 prepubertal boys with KS and 1475 controls were used to train a machine learning model for the detection of KS. The study showed that applying machine learning models can improve the identification of prepubertal boys with KS.
